Alternatively Activated (M2) Macrophage Phenotype Is Inducible by Endothelin-1 in Cultured Human Macrophages

Background Alternatively activated (M2) macrophages are phenotypically characterized by the expression of specific markers, mainly macrophage scavenger receptors (CD204 and CD163) and mannose receptor-1 (CD206), and participate in the fibrotic process by over-producing profibrotic molecules, such as transforming growth factor-beta1 (TGFbeta1) and metalloproteinase (MMP)-9. Endothelin-1 (ET-1) is implicated in the fibrotic process, exerting its profibrotic effects through the interaction with its receptors (ETA and ETB). The study investigated the possible role of ET-1 in inducing the transition from cultured human macrophages into M2 cells. Methods Cultured human monocytes (THP-1 cell line) were activated into macrophages (M0 macrophages) with phorbol myristate acetate and subsequently maintained in growth medium (M0-controls) or treated with either ET-1 (100nM) or interleukin-4 (IL-4, 10ng/mL, M2 inducer) for 72 hours. Similarly, primary cultures of human peripheral blood monocyte (PBM)-derived macrophages obtained from healthy subjects, were maintained in growth medium (untreated cells) or treated with ET-1 or IL-4 for 6 days. Both M0 and PBM-derived macrophages were pre-treated with ET receptor antagonist (ET(A/B)RA, bosentan 10(-5) M) for 1 hour before ET-1 stimulation. Protein and gene expression of CD204, CD206, CD163, TGFbeta1 were analysed by immunocytochemistry, Western blotting and quantitative real time polymerase chain reaction (qRT-PCR). Gene expression of interleukin(IL)-10 and macrophage derived chemokine (CCL-22) was evaluated by qRT-PCR. MMP-9 production was investigated by gel zymography. Results ET-1 significantly increased the expression of M2 phenotype markers CD204, CD206, CD163, IL-10 and CCL-22, and the production of MMP-9 in both cultures of M0 and PBM-derived macrophages compared to M0-controls and untreated cells. In cultured PBM-derived macrophages, ET-1 increased TGFbeta1 protein and gene expression compared to untreated cells. The ET-1-mediated effects were contrasted by ETA/BRA treatment in both cultured cell types. Conclusion ET-1 seems to induce the M2 phenotype in cultured human macrophages, a process apparently contrasted by the action of the ETA/BRA, suggesting possible clinical implications in those fibrotic diseases characterized by increased ET-1 concentrations, such as systemic sclerosis but also type 2 diabetes.

Different Methods to Improve the Monitoring of Noninvasive Respiratory Support of Patients with Severe Pneumonia/ARDS Due to COVID-19: An Update

Summary: The latest guidelines for the hospital care of patients with COVID-19-related acute respiratory failure recommend the use of noninvasive respiratory support (NIRS) instead of early intubation. Different clinical-radiological phenotypes of severe COVID-19 pneumonia have been categorized, which can benefit from the application of external positive end-expiratory pressure (PEEP) during noninvasive mechanical ventilation. Excessive negative pleural pressure swings (P-pl) can play a critical role in lung and diaphragm damage in patients treated with noninvasive respiratory support. Routine respiratory monitoring is necessary to prevent the worsening of respiratory condition and consider early initiation of invasive mechanical ventilation. Further research is needed to apply different monitoring methods in clinical practice and optimize ventilatory management for patients with acute respiratory distress syndrome due to SARS-CoV-2 infection.

Summary: This paper reviews the correlation between microvascular damage, endothelial dysfunction, and internal organ involvement, particularly pulmonary changes in SSc. While many studies have demonstrated the link between structural microcirculatory damage and pulmonary involvement, there is a scarcity of research on the correlation between microvascular functional impairment and internal organ damage. Overall, the literature supports the correlation between organ involvement and functional microcirculatory impairment in SSc patients. However, further investigation is needed to establish the potential of morphological and functional techniques as biomarkers in SSc.

Summary: The tyrosine kinase receptors of the TAM family and their ligand Gas6 play a crucial role in innate immunity and inflammatory response. The Gas6/TAM system is involved in the recognition of apoptotic debris by immune cells, which has been exploited by viruses like COVID-19 for cell entry and infection. Understanding the mechanisms underlying COVID-related fibrosis, particularly in the lungs, is still limited, and this review aims to integrate the current knowledge of the Gas6/TAM axis with the pathophysiological mechanisms of COVID-19, focusing on the development of a fibrotic phenotype.

Methylprednisolone is the most effective GC molecule, likely due to its ability to penetrate the lungs. Future research should focus on identifying markers to guide personalized GC treatment.

Further research is needed to optimize and standardize the procedure, as well as study the histological and molecular characteristics of PF.

Summary: This study aimed to describe the clinical characteristics and predictive factors for progression of ILD patients with autoimmune features. The results showed that IPAF patients had a lower prevalence of UIP pattern compared to CTD-ILD and UIPAF patients. ILD progression was less frequent in the IPAF group compared to CTD-ILD and UIPAF groups. The definition of UIPAF should be separated from ILD classification and include sicca syndrome for better prognosis prediction.
