Wound dressings in chronic wound therapy

Introduction: Poor wound healing is a growing cost factor in the public health care system. The aim of professional wound management is to break the cycle of chronic healing to enable physiological wound repair. Method: A selective literature search was conducted in the databases AWMF, PubMed und Cochrane Library with the key words chronic wound or ulcer in combination with the terms alginates, hydrofibre, hydrogel, collagen, foam. Results: The wound management products available today do not enable us to achieve physiological regeneration of the skin instead of scarred repair of chronic wounds. Before starting any form of wound treatment the possible causes of poor wound healing must be identified, and causal treatment should be given whenever possible. A wide selection of different wound dressings is available for the topical treatment of chronic wounds. Thorough knowledge of these wound treatment agents enables their differentiated use, and is thus the prerequisite for tailoring them for use according to the healing phase. Conclusion: The current study data barely enable any conclusions to be drawn regarding the superiority of individual wound dressings with regard to the endpoint wound healing. Although the paucity of the evidence makes the decision for or against any individual product more difficult for the clinician, it also shows that expensive wound dressings are not necessarily superior to the more reasonably priced alternatives.

Summary: The study demonstrated that local administration of triamcinolone significantly reduced postabdominoplasty seroma accumulation and led to a decrease in levels of inflammatory mediators IL-6 and MMP-9. The suppression of inflammatory markers correlated clinically with a decrease in seroma accumulation.

Monitoring free flaps and replanted digits via perfusion index - A proof of concept study

Summary: This study evaluated the feasibility of using the perfusion index (PI) as a monitoring tool for free flaps and replanted digits. The results showed a significant intra- and interindividual range of PI and pulse oxygen saturation (SpO2). Trends in intraindividual PI changes may be a promising monitoring tool for free flaps and replanted digits.

Summary: Oral squamous cell carcinoma (OSCC) growth into mandible presents challenges in head and neck surgery, impacting aesthetics, function, and patient's quality of life. Investigating the role of MMP-27, Osteoprotegerin, and RANKL in bone invasion, it was found that reduced MMP-27 expression was strongly associated with invasive tumor growth and mandibular bone invasion. Further studies are needed to elucidate the molecular mechanisms of mandibular bone invasion in OSCC.

Expression of pH-Sensitive TRPC4 in Common Skin Tumors

Summary: TRPCs are important in tumor biology, particularly in Ca2+ homeostasis in cancer cells. TRPC4, a pH-sensitive protein, may function as a signaling molecule in the altered tumor microenvironment due to the reversed pH-gradient in solid tumors. The study investigated TRPC4 expression profiles in common skin cancers, revealing that SCC, NCN, and MM show positive TRPC4-expression, while BCC only does so in about half of the analyzed samples. This finding suggests TRPC4 as an immunohistochemical marker to distinguish SCC and BCC and warrants further research on its role in tumor progression and metastasis.

Summary: A significant number of patients with oral squamous cell carcinoma do not have the typical risk factors of smoking and alcohol consumption. These patients are more likely to be female and older at the time of diagnosis, and have a higher incidence of oral potentially malignant disorders and a better prognosis. Never-smokers/never-drinkers also have distinct characteristics and a more immunogenic microenvironment, which may help in improving the treatment of oral squamous cell carcinoma in this population.
