Summary: This study investigates the mechanisms of gastro-esophageal reflux induced by spirometry and finds that spirometry can cause an elevation in impedance values and a reduction in resting pressure of the lower esophageal sphincter. The study also observes a decline in spirometric parameters during the second spirometry session when compared to the first session, providing compelling evidence for the association between spirometry maneuvers and gastro-esophageal reflux.

Summary: The study evaluated the mechanisms associated with reflux events after sleeve gastrectomy (SG). The results showed a strong association between reflux and SG, and identified three unique categories of reflux. The study also found that elevated gastro-esophageal pressure gradient was the main mechanism of reflux.

Summary: This study observed that clinical evaluation for aspiration relied more heavily on transbronchial biopsy findings. Although LLMI may retain clinical utility in some scenarios, reevaluation of the clinical value of ORO testing would be prudent.

Summary: Gastro-esophageal reflux disease (GERD) is a common condition that primary care physicians and gastroenterologists frequently encounter. Improving the management of GERD is vital for optimizing patient care and resource utilization. This review aims to provide a comprehensive update on the clinical management of GERD patients through a literature review on diagnosis and treatment.

Summary: GERD is increasingly diagnosed in infants worldwide, leading to more infants being treated. Non-pharmacological treatments are effective and safe, while acid-suppressive medications may have adverse effects. In infants, GERD is often nonacid related.

Summary: This study aimed to determine the discriminant factors of symptomatic reflux and establish diagnostic thresholds for GERD following SG. The results indicated that reflux score and supine acid exposure were the strongest discriminant factors for symptomatic reflux. The symptomatic cohort typically showed higher levels of acid exposure.

Summary: The study found that concurrent GT insertion and LNF is a common practice in children with GERD and failure to thrive. Children born preterm or with esophageal atresia are at high-risk for LNF failure after LNF/GT. Prospective, multicentric studies are needed to evaluate the best GT technique in children undergoing LNF.

Summary: This study evaluated the appropriateness of drug prescription for GERD in older people and found that only 41.2% of patients received appropriate prescriptions. Therefore, educational interventions should be implemented for physicians to improve prescription quality and prevent unnecessary health risks and costs.

Summary: Despite the large number of patients with pandemic-associated pernio, establishing a causal link to SARS-CoV-2 remains a challenge. Pathophysiological studies suggest that some children with pernio may have abnormalities in IFN-1 immunity, while excessive immune responses to SARS-CoV-2 are seen in the skin of pernio patients.

Summary: SARS-CoV-2 infection is generally mild, but in around 10% of cases, it can lead to severe pneumonia and potentially death. Genetic factors, such as inborn errors of type I interferons, and pre-existing auto-antibodies can increase the risk of critical illness. The production of type I interferons is essential for host defense against the virus.

Summary: SARS-CoV-2 infections can vary greatly between individuals, with some being asymptomatic while others experience life-threatening disease. While 20% of critical COVID-19 cases are due to inborn errors or autoantibodies, the genetic and immunological factors for resistance to infection itself remain unknown. Studies have shown that autosomal recessive deficiencies in certain genes or enzymes can confer resistance to diseases, suggesting a potential strategy for identifying individuals naturally resistant to SARS-CoV-2 infection.

Summary: Findings of the study showed that parents of children with chronic diseases experienced higher levels of anxiety, with COVID-19-specific stressors and parental psychological experiences being associated with increased anxiety, depression, and sleep problems. Moreover, parental burn-out was identified as a mediator for the impact of COVID-19-specific stressors on the well-being outcomes. This highlights the importance of interventions targeting parental burn-out in this vulnerable group.

Summary: This study analyzed the clinical, genetic, and immunological characteristics of 3 patients with uncharacterized AGS. RNU7-1 gene mutations were found to impair the processing of RDH pre-mRNA. A distinct IFN-I signature was observed in peripheral blood, and MCP-1 (CCL2) and CXCL10 were upregulated in cerebrospinal fluid. Histopathological analysis showed thrombotic microangiopathy in kidney tissue. Dysregulated STAT phosphorylation was observed upon cytokine stimulation.

Summary: This paper assessed the alignment of Belgian clinical practice in hereditary angioedema (HAE) with the updated international guideline and identified opportunities for optimization. The main action points to further optimize the Belgian clinical practice of HAE include working towards total disease control, informing patients about new long-term prophylactic therapies, ensuring availability of on-demand therapy, implementing a more comprehensive assessment in daily clinical practice, and expanding the existing patient registry.
