Platelet activation plays a key role in atherothrombosis in type 2 diabetes mellitus (T2DM) and increased in vivo platelet activation with enhanced thromboxane (TX) biosynthesis has been reported in patients with impairment of glucose metabolism even in the earlier stages of disease and in the preclinical phases. In this regards, platelets appear as addresses and players carrying and transducing metabolic derangement into vascular injury. The present review critically addresses key pathophysiological aspects including (i) hyperglycemia, glycemic variability and insulin resistance as determinants and predictors of platelet activation, (ii) inflammatory mediators derived from platelets, such as soluble CD40 ligand, soluble CD36, Dickkopf-1 and probably soluble receptor for advanced glycation-end-products (sRAGE), which expand the functional repertoire of platelets from players of hemostasis and thrombosis to powerful amplifiers of inflammation by promoting the release of cytokines and chemokines, cell activation, and cell-cell interactions; (iii) molecular mechanisms underpinning the less-than-expected antithrombotic protection by aspirin (ASA), despite regular antiplatelet prophylaxis at the standard dosing regimen, and (iv) stratification of patients deserving different antiplatelet strategies, based on the metabolic phenotype. Taken together, these pathophysiological aspects may contribute to the development of promising mechanism-based therapeutic strategies to reduce the progression of atherothrombosis in diabetic subjects. Summary: This study evaluated the impact of heparin administration on survival in a large cohort of Italian COVID-19 patients and found that in-hospital heparin treatment was associated with a lower mortality rate, especially in severely ill patients and those with strong coagulation activation. Further results from randomized clinical trials are needed to provide clear recommendations.

Summary: This study found that age, non-alcoholic fatty liver disease (NAFLD), and lnHOMA-IR together explained 18% of the variance in memory function, with NAFLD and lnHOMA-IR contributing 8% and 9.1% respectively. Preventing and managing insulin resistance may benefit in slowing down future cognitive decline.

Summary: The efficacy of hydroxychloroquine in treating SARS-CoV-2 infection remains controversial. A retrospective study of 4,396 Covid-19 patients in Italy identified two patient clusters with differing characteristics and responses to HCQ treatment. HCQ showed a beneficial effect in low risk Covid-19 patients, while its impact on high risk individuals was limited.

Summary: In a multicenter observational study involving 33 Italian hospitals, the use of LPV/r treatment did not change death rate in COVID-19 patients, while DRV/c was associated with increased mortality. Within the limits of an observational study, these data do not support the use of LPV/r or DRV/c in COVID-19 patients.

Summary: Liraglutide treatment can reduce sST2 levels and possibly have a positive effect on cardiac fibrosis in obese patients with prediabetes or early type 2 diabetes mellitus. Plasma Gal-3 levels before liraglutide treatment may predict the response to the drug in terms of beta cell function improvement.

Summary: Prevention of cardiovascular disease with low-dose aspirin may be less effective in patients with faster recovery of platelet cyclooxygenase-1 activity. Patients with accelerated COX-1 recovery exhibit reduced platelet glycoprotein Ib alpha shedding, leading to increased thrombopoietin production and the formation of platelets that are not inhibited by aspirin over 24 hours. Clinical markers including age, non-alcoholic fatty liver disease, visceral obesity, and TPO/GC ratio can accurately predict faster COX-1 recovery and suboptimal aspirin response. The TPO/GC ratio may serve as a simple tool to identify patients who would benefit from more frequent aspirin dosing.

Summary: Allergic reactions to COVID-19 vaccine components are rare but should be considered. Skin tests combined with the basophil activation test (BAT) using PEG2000, PEG4000, and DMG-PEG2000 can help evaluate allergic reactions to the vaccine. The study suggests that even when the BAT is negative, a high stimulation index (SI) in basophil activation may indicate a low-grade reaction to PEG.

Summary: This study found that in patients on chronic low-dose aspirin treatment, the release of extracellular vesicles (EVs) was inhibited within 10 hours after aspirin administration, but returned to normal after 24 hours. This effect may be COX-1 dependent, and EVs may affect the rate of COX-1 recovery and the duration of aspirin effect.

Summary: This study aimed to evaluate the protective role of nanocurcumin as an antioxidant and anti-inflammatory agent, and its synergistic effects with Donepezil in the treatment of Alzheimer's disease in rats. The results showed that treatment with nanocurcumin improved memory, locomotion, and neuronal differentiation by activating the PI3K/AKT/GSK-3 beta pathway.
