mTORC1 activation in childhood ependymoma and response to sirolimus

Recurrent ependymomas are considered rarely responsive to chemotherapy and often have a dismal prognosis after tumor progression. Below is a brief report of a 6 year old child with a multiply progressive ependymoma whose tumor had a near complete response to sirolimus that was durable for 18 months. Immunohistochemistry for phosphorylated S6, which has been reported to be associated with tumor sensitivity to mTORC1 inhibitors, was positive in this patient's tumor.

Summary: This study identified a new association between loss of polymerase proofreading and MSI, especially when both components are lost. Analysis of indels in microsatellites revealed five distinct signatures, providing insight into the mechanism of indel formation. The research demonstrates that MS-sigs can be a powerful clinical tool for diagnosing replication repair deficiency and predicting immunotherapy response.

Summary: Exposure to cranial radiotherapy is associated with an increased risk of subsequent CNS neoplasms among childhood, adolescent, and young adult (CAYA) cancer survivors. Surveillance for subsequent neoplasms is important, but there is currently insufficient evidence to establish whether early detection reduces morbidity and mortality. More research is needed to inform survivorship care.

In Vivo Estimation of Ketogenesis Using Metabolic Flux Analysis-Technical Aspects and Model Interpretation

Summary: The process of ketogenesis in the liver mitochondria and the measurement methods of endogenous ketone production in vivo using tracer approaches are discussed in the study. A two-pool model with simultaneous regression of liquid chromatography-tandem mass spectrometry ketone isotopologues and tracer infusion rates was implemented, along with H-1 NMR monitoring for AcAc tracer and analyte stability to ensure accurate flux calculations. The approach quantified AcAc and BHB pool sizes and their rates of appearance, disposal, and exchange, providing confidence intervals and detecting potential errors in experimental data, with complications for the physiological interpretation of individual ketone fluxes also discussed.

The findings highlight the importance of cell competition in shaping metastatic latency and relapse.

Phase II study of everolimus for recurrent or progressive pediatric ependymoma

Summary: This phase II clinical trial evaluated the efficacy of everolimus, an mTOR pathway inhibitor, for children with recurrent or progressive posterior fossa-A (PF-A) ependymomas. The study showed that everolimus did not lead to objective tumor responses in these patients. Adverse events were generally mild, with only one case of grade 3 pneumonia.
