Summary: Post-liver transplant biliary strictures are associated with higher rates of allograft rejection, failure, infections, and readmissions. Compared with ERCP, using IR or surgical interventions for these patients results in significantly higher rates of allograft failure and prolonged hospitalization.

Summary: This study identified positive HCV, biliary drain insertion, donor's BMI, and serum cholesterol level as independent risk factors for developing bile stones on top of biliary strictures. Biliary stones were associated with high episodes of acute graft rejection, and they could be successfully managed by the ERCP modality.

Summary: Biliary stricture (BS) is a significant complication after pediatric living donor liver transplantation (LDLT) for biliary atresia (BA). This study found that longer arterial ischemia time (AIT) was the only independent risk factor for the development of BS. AIT ≥40 minutes increased the 5-year cumulative risk of BS, highlighting the importance of minimizing AIT to reduce BS.

Summary: Bloodstream infection after pediatric liver transplantation is a common and severe complication that affects patient survival. This study evaluated the impact of post-transplantation bloodstream infection on clinical outcomes in children with biliary atresia, and found that bloodstream infection is associated with increased vascular complications and reoperations.

Summary: This study evaluated the long-term outcomes of endoscopic metal stenting for anastomotic biliary stricture (ABS) in patients who underwent deceased donor liver transplantation (DDLT). Among 465 patients, 41 developed ABS and endoscopic treatment was technically successful in 95.1% of cases. After a mean follow-up of 6.9 years, 22% of patients required surgical intervention.

Summary: This study aims to describe the cholangiographic features and endoscopic management of BCS after liver transplantation. Results showed that BCS patients have high rates of recurrence, secondary strictures, and poorer prognosis compared to non-AS and AS patients.

Summary: This study retrospectively investigated the occurrence of benign biliary strictures (BBS) in patients with acute biliary pancreatitis (ABP). The results showed that the majority of patients showed improvement in the stricture within 3 months after endoscopic management, and temporary stenting prevented complications during this period.

Summary: In this systematic review and meta-analysis, we identified 7 prognostic factors for the development of non-anastomotic biliary strictures (NAS) following orthotopic adult liver transplantation (OLT). These findings may potentially contribute to the development of diagnostic algorithms for better risk stratification of patients at risk for developing NAS.

Summary: This study compared the initial success rate and effectiveness of ERCP and PTC for anastomotic biliary stricture after liver transplantation. ERCP is the gold standard method for diagnosis and intervention, but PTC may be more effective for certain patients with special bile duct conditions.

Summary: This study found that the use of antimigration fully covered metal stents (A-FCMS) can significantly reduce stent migration rates in patients with anastomotic biliary strictures (ABS) following orthotopic liver transplantation (OLT), but the clinical benefit of reduced stent migration is unclear.

Summary: The study reported a single-institution experience involving a Glissonian sheath-to-duct method for biliary reconstruction in living donor liver transplantation, which showed lower biliary stricture rates compared to duct-to-duct reconstructions. The GD technique was suggested as an alternative method for biliary reconstruction in LDLT.

Summary: Invasive fungal infections (IFIs) are a serious complication of liver transplantation, with Candida, Aspergillus, and Cryptococcal infections being the most common. Prophylaxis is considered a promising mechanism for reducing the risks associated with these infections in liver transplant recipients.

Summary: The study found that both ACEI and ARB use were associated with decreased risk of community-acquired pneumonia infection, regardless of age and sex. Patients using ARB for a shorter duration exhibited a greater reduction in risk, while those using ACEI for 121-450 days were more likely to have reduced risks of community-acquired pneumonia.

Summary: This study investigated the trend of mortality and developed predictive models for adult patients with bacteremia at emergency departments. The study found a decrease in the cumulative incidence and mortality of bacteremia, but the mortality burden remains critical. The proposed ED-BM model had significantly better performance in predicting short-term mortality compared to other scoring systems.
