Summary: Hepatitis C virus (HCV) is a significant cause of chronic liver diseases worldwide and is associated with negative consequences, including cirrhosis, hepatocellular carcinoma, and increased risk of mortality. In addition to liver-related morbidities, HCV is also associated with several extrahepatic manifestations. Despite the introduction of direct-acting antiviral drugs, a considerable number of patients still develop extrahepatic complications.

Summary: Hepatitis E virus (HEV) infection has various extrahepatic manifestations affecting multiple organ systems, indicating it should be considered as a systemic disease. Different genotypes of HEV can induce different extrahepatic manifestations, and the severity of these diseases does not necessarily correlate with the severity of HEV infection. Antiviral therapy may alleviate or improve the extrahepatic manifestations related to HEV infection.

Through key publications, particularly in the Journal of Oral Pathology and Medicine, strong epidemiologic evidence supporting the association and a plausible pathogenic link between HCV and OLP were discovered. With the potential curability of HCV infection using modern direct antiviral agents, OLP in HCV-infected patients can also be effectively cured.

Summary: HCV infection in patients can lead to a range of rheumatologic manifestations and other systemic diseases, and the development of autoimmune phenomena and lymphoproliferative disorders are mainly mediated by immunological mechanisms. The presence of auto-antibodies is associated with chronic infectious triggers, and direct-acting antiviral therapies have been effective in reducing damage caused by immune activation. Nevertheless, identifying whether a primary rheumatic pathology coexists with HCV infection can complicate differential diagnosis.

The disease presents with various clinical manifestations and symptoms, ranging from discomfort to severe pain. The first-line treatment for symptomatic flares is topical superpotent corticosteroids, while a wide range of second/third-line treatments are available. Follow-up of patients is important to identify the transformation into squamous cell carcinoma, which occurs in approximately 1% of patients.

However, in patients with hepatitis C virus-related cirrhosis, there is no significant relationship between diabetes and HCC occurrence in studies with less than 48-month follow-up. Furthermore, diabetes also increases the risk of HCC in patients with hepatitis B virus-related cirrhosis.
