Summary: The clinical and neuroimaging characteristics of rheumatoid meningitis (RM) in Chinese patients were analyzed retrospectively. Among 933 patients with rheumatoid arthritis (RA), six patients were identified with RM. Headache, hyperacute focal neurological deficits, and seizures were the most common symptoms. Meninges were affected in all patients and cerebral parenchyma in one patient. The lesions were generally located in the frontoparietal region. Immunotherapy, including tocilizumab, showed good response in most patients. RM in Chinese patients share similar characteristics and respond well to immunotherapy.

Summary: Rheumatoid arthritis (RA) is an autoimmune and inflammatory disease that causes joint destruction. The development of RA is believed to be influenced by various factors, including hereditary factors, immune system defects, gender, infectious agents, nicotine, and stress. Epigenetic changes, particularly the involvement of sirtuins, have been found to play a role in the aggressive phenotype of RA. Sirtuins, enzymes found in all living organisms, can slow down aging processes, reduce cell death, counteract inflammation, and regulate metabolism. This review discusses the role of all sirtuins (1-7) in the pathogenesis of rheumatoid arthritis.

Summary: Rituximab appears to be an effective and safe therapeutic option for rheumatoid arthritis-associated peripheral ulcerative keratitis. Most patients showed controlled corneal inflammation and good articular responses after treatment. Long-term follow-up did not show other manifestations of rheumatoid vasculitis or serious adverse events related to rituximab.

Summary: Rheumatoid arthritis is an autoimmune disorder that can lead to vasculitis. Cerebral rheumatoid vasculitis is a rare subtype of systemic rheumatoid vasculitis, causing strokes. There are currently no formal recommendations or guidelines for the treatment and prevention of stroke caused by cerebral rheumatoid vasculitis.

Summary: The study found that initiating glucocorticoids in steroid-naive RA patients is associated with an increased risk of cardiovascular events, especially at daily doses of >= 5mg, cumulative doses exceeding 1100mg, and durations longer than 81 days. No increased risk was found at lower doses or shorter durations.

Summary: RA patients have a 1.5 times higher risk of developing cardiovascular diseases compared to the general population. This is partly due to systemic inflammation causing increased atherogenesis, and an increased prevalence of traditional cardiovascular risk factors.

Summary: This study aims to evaluate whether the incidence of rheumatoid arthritis has increased after the introduction of the updated ACR/EULAR classification criteria. A systematic review found that the incidence of rheumatoid arthritis has increased over time since the implementation of the new criteria. However, due to significant heterogeneity among the included studies, no definitive conclusions can be drawn regarding this time effect.

Summary: Spinal subarachnoid haemorrhage is rare in cases of subarachnoid haemorrhage and even rarer with spinal rheumatoid vasculitis. The pathogenesis of this condition remains unknown. This case study describes a 52-year-old woman with rheumatoid arthritis who developed spinal subarachnoid haemorrhage, secondary to isolated spinal vasculitis, and achieved good therapeutic outcomes.

Summary: This large-scale nationwide cohort study investigated the association between cardiovascular disease (CVD) risk and rheumatoid arthritis (RA) and factors that may increase CVD risk. The study found that RA patients had a 50% higher risk of myocardial infarction (MI) and a 20% higher risk of stroke. The association between RA and CVD was more prominent in females, and the association between RA and MI risk was significant in individuals without diabetes.

Summary: This study aimed to estimate the short-term risks of acute coronary syndrome (ACS) in patients with rheumatoid arthritis (RA) based on their current disease activity, including remission. The results showed that failure to reach remission is associated with an increased risk of ACS, highlighting the importance of optimizing cardiovascular risk factors in these patients.

Summary: This study evaluated the effect of combination therapy with methotrexate and adalimumab versus methotrexate alone on cartilage status in patients with early rheumatoid arthritis using delayed gadolinium-enhanced magnetic resonance imaging of cartilage (dGEMRIC). The results showed that combination therapy resulted in improved cartilage composition, while methotrexate alone did not have any effect on cartilage composition.

Summary: This review included 14 studies and found that female patients were more common, with ages ranging from 47 to 61 years. Regular exercise was associated with a decrease in systolic blood pressure, while methotrexate was linked to a decreased risk of hypertension. Prednisone and COX-2 inhibitors may increase the risk of hypertension.
