Summary: This study aimed to investigate the anti-allergic effects of a mixed extract of wheatgrass and aronia (TAAR) on mast cell activation and cutaneous anaphylaxis. The TAAR extract decreased the release of mediators related to mast cell degranulation and regulated oxidative-stress-related factors through the Nrf2 signaling pathway. In vivo experiments showed that TAAR extract improved symptoms in a mouse model of IgE-mediated allergic inflammation. Overall, TAAR extract shows potential as a natural product for the treatment of allergic inflammation and oxidative-stress-related diseases.

Summary: This article reports the first case of cerebral venous thrombosis in close temporal relation with an immunoglobulin-E mediated anaphylactic reaction. The patient presented with headache, language, and mental disturbances. Magnetic Resonance Imaging revealed a venous infarct and thrombosis in the deep venous system. The patient was treated with anticoagulation and showed improvement, suggesting that the anaphylactic reaction could have been a precipitating factor for cerebral venous thrombosis.

Summary: Acute anaphylaxis to small molecule drugs is believed to be predominantly antibody-mediated with IgE and IgG involvement, along with activation of mast cells and neutrophils. The MRGPRX2 receptor has also been identified as a potential trigger for anaphylactic reactions to various drugs, independent of antibody recognition. Understanding the role of MRGPRX2 in anaphylaxis could be crucial for future drug development and exploration as a potential drug target in broader clinical settings.

Summary: Based on a systematic review, it is unlikely that cross-contamination of major food allergens below 0.5 mg/100 g will endanger most food allergic patients when consuming a standard portion of food. The suggestion is to use a voluntary declaration on processed food packages stating that the product contains named allergens and may contain traces of other contaminations at concentrations less than 0.5 mg per 100 g of the product, which can be technically achieved in most processed foods. Further research on threshold levels is encouraged for a legally binding solution in the future.

Summary: Hymenoptera venom-induced anaphylaxis is a common and potentially fatal allergic reaction, with mastocytosis being an important risk factor. Undiagnosed underlying clonal mast cell disorder (cMCD) was found in some cases of fatal anaphylaxis. Screening high-risk individuals for cMCD using molecular methods is crucial for proper management and prevention of unnecessary deaths.

Summary: This review discusses the characteristics, synthesis modulation, and function of local IgE in chronic rhinosinusitis with nasal polyps (CRSwNP). The elevated levels of local IgE in polyps are independent of serum IgE levels and atopic status. Local IgE, associated with type 2 inflammation, is polyclonal and functional. Its production is triggered by not only allergens but also microbial colonization. Various factors, including lymphocytes, cytokines, and transcription factors, regulate the production of local IgE.

Summary: This study reports on the workflow established at the institution for evaluating allergic reactions to COVID-19 vaccines and summarizes the types and frequency of adverse reactions. The results show that most vaccine reactions occurred in women, with respiratory allergies predominantly observed in women with a history of allergies and asthma. Furthermore, a higher incidence of dermatological reactions was observed in men. Overall, most individuals who reacted to the initial vaccine dose could safely tolerate the second dose, and intradermal PEG testing had limited utility for evaluating allergic reactions to mRNA vaccines.

Summary: The purpose of this study was to compare the incidence of rescue medication utilization with varying doses of paclitaxel in patients who experienced a hypersensitivity reaction and required rescue medications. The results indicated a potentially meaningful increase in rescue medication utilization in patients who underwent an infusion rate escalation compared to those who continued on the standard infusion rate. However, there was no significant correlation between rescue medication utilization and disease stage, dosing regimen, or asthma diagnosis.

Summary: Biologicals are commonly used for rheumatologic diseases, cancers, and chronic inflammatory conditions, but they may lead to the development of anti-drug antibodies (ADA), limiting their clinical usage due to safety concerns. Strategies such as concomitant immunosuppressive treatment and regular infusion regimens can help reduce the risk of immediate hypersensitivity reactions (HSRs) associated with ADA. Drug desensitization (DD) may be considered for patients needing reintroduction of the biological culprit, especially in cases of IgE-mediated HSRs, potentially inhibiting mast cell degranulation and cytokine production.

Summary: The alpha-Gal syndrome is widely recognized across the Southeast, Midwest, and Atlantic Coast of North America, commonly acquired in adulthood as a consequence of tick bites. The distribution of cases generally conforms to the reported range of the lone star tick, with an inverse relationship between alpha-Gal cases and cases of imported fire ant anaphylaxis.
