Nonceliac gluten sensitivity

Summary: This review article summarizes recent advances in nonceliac gluten sensitivity (NCGS), a disorder characterized by varied symptoms and frequent irritable bowel syndrome (IBS)-like manifestations. The recent discovery of wheat components other than gluten as disease triggers suggests that nonceliac wheat sensitivity (NCWS) may be a better term. Self-reported NCWS is common, especially in gastroenterology clinic patients, but many diagnoses are not confirmed by standard clinical criteria. A biomarker for NCWS is lacking, although subtle histological features at small intestinal biopsy may aid in diagnosis. Treatment of NCWS is based on the gluten-free diet (GFD), which has proven effective for a significant proportion of NCWS-related IBS patients. Dietary therapies, including the GFD, should be offered by dietitians based on individual patient choice. Studies are underway to test the therapeutic efficacy of supplemental gluten-digesting enzymes in NCWS patients. Recent studies have provided insights into the pathophysiology and clinical features of NCWS, but many questions remain unanswered regarding its epidemiology, biomarkers, and natural history.

Summary: This review examines the clinical associations of celiac disease (CeD), non-celiac gluten sensitivity (NCGS), and IBD, focusing on the therapeutic efficacy of gluten-free diets (GFD) or low-gluten diets in the therapy of IBD. While some cross-sectional reports suggest that a GFD may improve symptoms in IBD patients, current data do not support the universal use of a GFD in IBD due to a lack of high-quality prospective clinical studies.

The final definition will help researchers and clinicians better understand and support individuals with misophonia.

Definition of reaction pathways for catalytic aquathermolysis of Liaohe heavy crude oil

Symptoms of gluten ingestion in patients with non-celiac gluten sensitivity: A randomized clinical trial

Gluten Degradation by the Gut Microbiota of Ulcerative Colitis Patients

Summary: Several studies have shown that a gluten-free diet can improve disease symptoms in ulcerative colitis (UC) patients. This study hypothesized that UC-associated enteric microbes metabolize dietary gluten differently, producing immunogenic products that promote inflammation. The study found that UC patients did not consume gluten differently from healthy controls, but the profile of gluten-degrading bacteria in their stool was distinct. The products of gluten degradation by certain bacteria induced inflammation and affected barrier function in colonocyte cultures. These preliminary findings suggest a need for further investigation into the mechanisms by which gut microbiota contribute to UC pathogenesis through gluten degradation.

The need for a clinical case definition in test-negative design studies estimating vaccine effectiveness

Summary: Test-negative studies are widely used to estimate the effectiveness of COVID-19 vaccines. Selection bias can occur if the probability of participation is related to vaccination or COVID-19. However, using a clinical case definition can mitigate this bias. A systematic review and simulation found that studies using a clinical case definition had lower estimates of vaccine effectiveness compared to those that did not. Additionally, simulations showed a positive bias in vaccine effectiveness estimates when there was a higher proportion of healthy, vaccinated non-cases.

Cerebellar Symptoms in Gluten Sensitivity: a Systematic Review of the Effect of a Gluten-Free Diet on Brain Imaging

Summary: Neuroimaging studies have shown that cerebellar manifestations are prevalent in patients with gluten sensitivity, with cerebellar atrophy and white matter abnormalities being common findings. Magnetic resonance imaging, single-photon emission computed tomography, and magnetic resonance spectroscopy have been useful tools in diagnosing and monitoring brain changes in these patients, although more prospective studies are needed to further explore the neurological manifestations of gluten sensitivity on a gluten-free diet.

The Perception of Lactose-Related Symptoms of Patients with Lactose Malabsorption

The Effect of a Gluten-Free Diet on Vitamin D Metabolism in Celiac Disease: The State of the Art

Summary: Celiac disease is a chronic autoimmune disorder affecting the small intestine, leading to villous atrophy, crypt hyperplasia, and an increase in intraepithelial lymphocytes. This condition often causes disturbances in bone mass and mineral metabolism, including changes in serum levels of vitamin D. A gluten-free diet can reverse these abnormalities, raising questions about the necessity of vitamin supplementation for all patients. Currently available data do not support widespread use of vitamin supplementation, and a thorough evaluation of bone and mineral metabolism is needed to determine which patients require this approach.

The presence of CD, peripheral lymphocytosis, advanced Lugano clinical stage, and the histological subtype ITCL-NOS were significantly associated with worse survival.
