Lower Cortical Porosity and Higher Tissue Mineral Density in Chinese American Versus White Women

Asian women have lower rates of hip and forearm fractures compared to other racial groups despite lower areal bone mineral density (aBMD). We have demonstrated microarchitectural differences, including greater cortical thickness (Ct.Th) and cortical volumetric BMD (Ct.BMD), in Chinese American versus white women. Yet it is not known whether greater Ct.BMD in Chinese American women is a result of greater tissue mineral density (TMD) or reduced cortical porosity (Ct.Po). Using an advanced segmentation algorithm based on high-resolution peripheral quantitative computed tomography (HR-pQCT) images, we tested the hypothesis that Chinese American women have better cortical skeletal integrity owing to lower Ct.Po and higher Ct.TMD compared with white women. A total of 78 Chinese American women (49 premenopausal and 29 postmenopausal) and 114 white women (46 premenopausal and 68 postmenopausal) were studied. Premenopausal Chinese American versus white women had greater Ct.Th, Ct.BMD, and Ct.TMD at both the radius and tibia, and decreased Ct.Po (p<0.05). A similar pattern was observed between postmenopausal Chinese American and white women. As expected, postmenopausal versus premenopausal women had lower Ct.BMD at the radius and tibia in both races (p<0.001). Ct.Po largely increased between premenopausal and postmenopausal women, whereas Ct.TMD decreased by 3% to 8% (p<0.001) in both races. Age-related differences in Ct.Po and Ct.TMD did not differ by race. In summary, both reduced Ct.Po and greater Ct.TMD explain higher Ct.BMD in Chinese American versus white women. Thicker and preserved cortical bone structure in Chinese American women may contribute to greater resistance to fracture compared to white women. Summary: Postmenopausal osteoporosis is influenced by reduced estrogen levels during menopause, impacting bone remodeling, bone mass, and fracture risk. Peak bone mass and microstructure are important predictors of osteoporosis risk, with thicker trabeculae being associated with lower rates of bone loss in response to estrogen deficiency. Thicker trabeculae are less likely to disconnect or perforate, leading to a lower degree of bone loss.

This study identified Gli1(+) cells as important contributors to meniscus development and repair, showing high progenitor activities under the control of Hh signal. Stimulating these cells or using an Hh agonist can accelerate healing and reduce osteoarthritis symptoms.

Lactation alters fluid flow and solute transport in maternal skeleton: A multiscale modeling study on the effects of microstructural changes and loading frequency

Summary: The study used a multiscale poroelastic modeling technique to investigate the effects of lactation on the physicochemical environment and mechanosensing of osteocytes in murine tibiae. The results showed that lactation-induced LCS expansion and PCM reduction promoted solute transport and osteocyte mechanosensing, while higher loading frequency and sparser PCM enhanced flow-mediated mechanosensing and nutrient transport.

Summary: This study demonstrated that maternal bone undergoes improvements in structure in response to dynamic loading, regardless of reproductive status, with lactating rats showing enhanced mechano-responsiveness in both trabecular and cortical bone compartments. Additionally, lactation-induced activities in osteocyte peri-lacunar/canalicular remodeling may lead to increased mechano-responsiveness in maternal bone during lactation. Findings emphasize the importance of weightbearing exercise on maternal bone health during reproduction and postpartum.

Summary: Despite the potent effect of intermittent parathyroid hormone (PTH) treatment on promoting new bone formation, bone mineral density (BMD) rapidly decreases upon discontinuation of PTH administration. The responses to PTH treatment withdrawal vary in intact and estrogen-deficient rats, with the latter experiencing significant bone loss and microarchitecture deterioration. An extended anabolic period is observed in estrogen-deficient rats upon early PTH withdrawal, followed by adverse changes in bone cell activities.

Structural role of osteocyte lacunae on mechanical properties of bone matrix: A cohesive finite element study

Summary: This study examined the structural role of osteocyte lacunae on the mechanical properties of the bone matrix, revealing significant linear relationships between lacunar area, density, size, and orientation with elastic modulus and ultimate strength. Parametric studies also showed strong correlations between elastic modulus and strength with lacunar density, size, and orientation. Additionally, simulations demonstrated that osteocyte lacunae guide crack propagation through local stress concentrations.

Summary: This study investigated the temporal response of newly formed bone tissue and preexisting bone tissue to withdrawal from intermittent parathyroid hormone (PTH) treatment in an ovariectomized rat model. The results showed that modeling-based bone formation (MBF) sites had a greater contribution to the extended anabolic effect upon early withdrawal from PTH compared to remodeling-based bone formation (RBF) sites. Furthermore, newly formed bone tissue from MBF had greater resistance to PTH discontinuation-induced bone loss than those from RBF and preexisting bone.
