A computational exploration of helical arterio-venous graft designs

Although arterio-venous grafts (AVGs) are the second best option as long-term vascular access for hemodialysis, they suffer from complications caused by intimal hyperplasia, mainly located in vessel regions of low and oscillating wall shear stress. However, certain flow patterns in the bulk may reduce these unfavorable hemodynamic conditions. We therefore studied, with computational fluid dynamics (CFD), the impact of a helical AVG design on the occurrence of (un)favorable hemodynamic conditions at the venous anastomosis. Six CFD-models of an AVG in closed-loop configuration were constructed: one conventional straight graft, and five helical designed grafts with a pitch of 105 mm down to 35 mm. At the venous anastomosis, disturbed shear was assessed by quantifying the area with unfavorable conditions, and by analyzing averaged values in a case-specific patch. The bulk hemodynamics were assessed by analyzing the kinetic helicity in and the pressure drop over the graft. The most helical design scores best, being instrumental to suppress disturbed shear in the venous segment. There is, however, no trivial relationship between the number of helix turns of the graft and disturbed shear in the venous segment, when a realistic closed-loop AVG model is investigated. Bulk flow investigation showed a marked increase of helicity intensity in, and a moderate pressure drop over the AVG by introducing a lower pitch. At the venous anastomosis, unfavorable hemodynamic conditions can be reduced by introducing a helical design. However, due to the complex flow conditions, the optimal helical design for an AVG cannot be derived without studying case by case. (C) 2012 Elsevier Ltd. All rights reserved.

Central blood pressure estimation in type 1 diabetes: impact and implications of peripheral calibration method

Summary: The central blood pressure values estimated by radial artery tonometry in T1DM patients depend significantly on the method used for calibration of the radial waveforms. Using brachial MAP/DBP calibration resulted in higher central blood pressure compared to using brachial SBP/DBP calibration, indicating the importance of calibration approach for accurate estimation of central blood pressure using noninvasive methods.

Summary: Shear wave elastography (SWE) is a promising technique for assessing myocardial stiffness non-invasively. The study found that the transmural wave speed gradient can distinguish between intrinsic myocardial stiffening and stiffening caused by increased pressure. This gradient may have potential as a new diagnostic parameter for future clinical studies.

Pulse wave velocity: A clinical measure to aid material parameter estimation in computational arterial biomechanics

Summary: This study investigates whether pulse wave velocity (PWV) can guide in determining appropriate parameter values for the Gasser-Ogden-Holzapfel (GOH) constitutive model. The results show that PWV80 and PWV110 are complementary measures that can reduce the parameter space in arterial models, particularly for media- and collagen-related parameters.

Assessment of Early Diastolic Intraventricular Pressure Difference in Children by Blood Speckle-Tracking Echocardiography

Summary: This study developed and validated the calculation of intraventricular pressure difference (IVPD) using blood speckle-tracking (BST) in children and investigated its potential role in assessing diastolic function. The results showed that BST is highly feasible and provides information on diastolic suction and early filling in children with heart disease. The IVPD was significantly reduced in children with dilated cardiomyopathies (DCMs) and hypertrophic cardiomyopathies (HCMs) compared to controls.

The impact of geometry, intramural friction, and pressure on the antegrade longitudinal motion of the arterial wall: A phantom and finite element study

Summary: Longitudinal motion of the carotid arterial wall, as measured with ultrasound, is a promising indicator of vascular health, but its underlying mechanisms are not fully understood. Blood pressure has a strong relation to the antegrade longitudinal displacement in early systole. Tapered geometry and intramural friction between two parts of a vessel wall also influence the longitudinal displacement.

Arterial pulse wave modeling and analysis for vascular-age studies: a review from VascAgeNet

Summary: Arterial pulse waves (PWs), such as blood pressure and photoplethysmogram (PPG) signals, are rich sources of information for assessing vascular age and identifying individuals at elevated cardiovascular risk. This review explores the possibilities and limitations of reduced-order biophysical models and theoretical and empirical methods for analyzing PW signals and extracting clinically relevant information. Detailed mathematical derivations are provided, demonstrating the relationships between these models and methods. Suggestions for future research are also outlined to fully utilize the potential of modeling and analyzing PW signals for accurate vascular age assessment.

Summary: This study developed a computational model to simulate the thickening process of dissected aorta wall, considering the effects of stress and inflammation on elastin and collagen. The results showed that the model with transient inflammation pattern was able to reproduce the clinically observed membrane thickening rates, diameter expansion rates, and adaptations in microstructure.

In Vivo Material Properties of Human Common Carotid Arteries: Trends and Sex Differences

Summary: This study provides an estimation method for material parameters in the study of continuous diseases using non-invasively measured human carotid artery data. Both different material models are able to simulate the behavior of the arteries, but the structural model provides more realistic results. Gender has a greater impact on mechanical properties, and there is a stiffening trend in arteries with age, body mass index, and blood pressure, although not strong. Meanwhile, there is a high intersubject variability.

Image-based patient-specific Computational Fluid Dynamics (CFD) models provide detailed information about cardiac blood flow, supporting clinicians in diagnosis and treatment planning. This study developed a Chimera-based method to build a patient-specific 3D CFD model of the heart, considering the challenges of left ventricle and mitral valve motion. The results suggest that the mitral valve plays a crucial role in intraventricular flow development.

Individuals with rotator cuff tears unsuccessfully treated with exercise therapy have less inferiorly oriented net muscle forces during scapular plane abduction

Summary: Exercise therapy fails in about 25.0% of cases for individuals with rotator cuff tears, and one reason for this failure may be the inability to strengthen and balance the muscle forces that keep the humeral head in the correct position. This study developed computational musculoskeletal models to compare the net muscle force before and after exercise therapy between successfully and unsuccessfully treated patients. The study found that unsuccessfully treated patients had less inferiorly oriented net muscle forces, which may increase the risk of impingement.

Differences in intra-foot movement strategies during locomotive tasks among chronic ankle instability, copers and healthy individuals

Summary: This study aimed to explore the intra-foot biomechanical differences among individuals with chronic ankle instability (CAI), copers, and healthy individuals during dynamic tasks. The study found that copers and CAI individuals had smaller dorsiflexion angles and copers presented a more eversion position compared to healthy participants. Copers also had greater dorsiflexion angles in the metatarsophalangeal joint and more inversion moments in the subtalar joint during certain tasks. These findings can help in designing interventions to restore ankle joint functions in CAI individuals.

Acute effects of robot-assisted body weight unloading on biomechanical movement patterns during overground walking
