Persistent versus transient depressive symptoms in relation to platelet hyperactivation: A longitudinal analysis of dementia caregivers

Background: Depressive symptoms and caregiving stress may contribute to cardiovascular disease (CVD) via chronic platelet activation; however, it remains unclear whether this elevated activation constitutes a trait or state marker. The primary objective was to investigate whether persistent depressive symptoms would relate to elevated platelet activation in response to acute psychological stress over a three-year period. Methods: Depressive symptoms (Brief Symptom Inventory) were assessed among 99 spousal dementia caregivers (52-88 years). Platelet P-selectin expression was assessed in vivo using flow cytometry at three time-points over the course of an acute stress test: baseline, post-stress, and after 14 min of recovery. Two competing structural analytic models of depressive symptoms and platelet hyperactivity with three yearly assessments were compared. Results: Although depressive symptoms were generally in the subclinical range, their persistent elevation was associated with heightened platelet reactivity and recovery at all three-years while the change in depressive symptoms from the previous year did not predict platelet activity. Limitations: These results focus on caregivers providing consistent home care, while future studies may extend these results by modeling major caregiving stressors. Conclusions: Enduring aspects of negative affect, even among those not suffering from clinical depression are related to hemostatic changes, in this case platelet reactivity, which might be one mechanism for previously reported increase in CVD risk among elderly Alzheimer caregivers. Steriod-associated psychiatric burden in cancer patients

Summary: This study investigates the role of steroid treatment in identifying cancer patients who are distressed or mentally disordered. By analyzing the medical records of 12,298 cancer patients, the study identifies four subgroups of patients using different dosages of prednisone equivalents. The study finds that patients with higher dosages of prednisone are more likely to receive psychotropic drug administration, while patients with lower dosages have a slightly increased probability of psychiatric assessment and psychotropic drug administration. The subgroup with the lowest probability of receiving steroid treatment also has the lowest likelihood of receiving psychiatric assessment and psychotropic drug administration.

Sympathetic nervous system responses to acute psychosocial stress in male physicians with clinical burnout

A short screening tool identifying systemic barriers to distress screening in cancer care

Summary: International guidelines recommend screening for cancer patients’ distress, but rates are insufficient. This study developed a survey to assess systemic barriers to distress screening. Through a three-step approach, an initial survey with 53 questions was reduced to a final set of 14 validated questions using qualitative content analysis and factor analysis. Major barriers identified include timing of screening, lack of capacity, patient and staff overload, and refusal of patients to be referred to support services. The validated questions enable quick identification and improvement of screening programs.

Summary: The COVID-19 pandemic has negatively affected the mental health of ICD patients, leading to fear, anxiety, depression, and PTSD. Factors such as unemployment, younger age, low social support, prior COVID-19 infection, and inadequate knowledge about COVID-19 predicted these mental health issues. Fear of COVID-19 had an impact on anxiety, depression, and PTSD, with positive and negative affects mediating these relationships. Psychological and educational interventions are crucial for vulnerable subgroups of ICD patients.

Early impact of the COVID19 pandemic on psycho-oncological support: a latent class analysis

Summary: This study is the first to examine the impact of the COVID-19 pandemic on the provision of psycho-oncological care, stage of cancer at first diagnosis, and duration of hospitalizations. The retrospective latent class analysis of 4639 electronic patient files revealed that the pandemic had no effect on the provision of psycho-oncological support.

Summary: This study aims to explore factors associated with declining psycho-oncological support in order to increase nurses' efficiency in screening patients. The developed model has a high sensitivity in detecting patients who decline psycho-oncological support, with older patients, patients with lower distress levels, fewer comorbidities, fewer physical problems, and those who do not feel sad, afraid, or worried being more likely to refuse psycho-oncological support.

Summary: This study comprehensively characterized a pair of monozygotic female twins, one of whom experienced TTS. The twin sister with TTS exhibited higher levels of anxiety, vital exhaustion, social inhibition, and alexithymia, and lower levels of quality of and meaning in life. Additionally, the twin with TTS experienced more-and also more severe-stressors involving life-threatening and dangerous situations over the life course.

The role of family accommodation in child posttraumatic stress symptoms and functional impairment in the context of cognitive behavioral therapy

Summary: This study examined the relationship among family accommodation (FA), posttraumatic stress symptoms (PTSS), and functional impairment. The results showed that FA was significantly associated with PTSS and functional impairment. Baseline FA partially mediated the relationship between baseline PTSS and functional impairment. Changes in FA from pre- to post-treatment were associated with relevant outcome variables at post-treatment and 12-month follow-up.
