Summary: It is not possible to diagnose the cause of voiding difficulty based on medical history alone. Different urinary problems may have overlapping symptoms. To differentiate between underactive bladder and bladder outlet obstruction as the underlying cause of lower urinary tract symptoms, invasive urodynamic study of the bladder is often required.

Summary: A retrospective cohort study was conducted in 105 UK hospitals to determine the preoperative assessment and perioperative outcomes of men undergoing bladder outlet obstruction (BOO) surgery. The study found that completion rates of frequency-volume charts and urinary symptom questionnaires were low, with 5% of men experiencing complications within 30 days of surgery. Overall, the study concluded that BOO surgery treatments are safe and effective, but there is room for improvement in adherence to preoperative assessment guidelines.

Summary: This study investigated the therapeutic effect of combination treatment with a hydrogen sulfide (H2S) donor and tadalafil on bladder dysfunction induced by partial bladder outlet obstruction. The results showed that the combination treatment could normalize the abnormal bladder function by regulating the H2S and nitric oxide pathways and decreasing oxidative stress and hypoxia.

Summary: Male patients with bladder stones often experience sexual dysfunction and lower urinary tract symptoms, which can be significantly improved by surgical removal of the stones. Patients with BPH are more likely to have sexual dysfunction.

Summary: In a rat model of partial bladder outlet obstruction (PBOO), sodium butyrate (NaBu) was found to improve erectile function, prevent penile fibrosis, and normalize the expression of certain proteins and genes. NaBu may be a promising target for preventing erectile dysfunction (ED) in patients with benign prostate hyperplasia (BPH).

Summary: This study evaluated the impact of endoscopic enucleation surgery of the prostate on testosterone levels in hypotestosteronemic patients with bladder outlet obstruction. The results showed a significant improvement in testosterone levels after surgery, particularly in patients with lower preoperative testosterone levels.

Summary: This study aimed to evaluate glycogen deposition within the bladder wall in patients with bladder outlet obstruction (BOO) due to benign prostate hyperplasia (BPH) and its correlation with urodynamic findings. The study found that there was increased detrusor glycogen deposition in patients with BOO due to BPH, but the amount of deposition did not correlate with symptom severity and duration or urodynamic findings.

Summary: The study evaluated the efficacy of tamsulosin for male overactive bladder and found statistically significant reductions in voiding frequency, urgency scores, and nightly nocturia. Baseline AUA-7 SI total score and voiding subscale categories were associated with significant reduction in symptoms.

Summary: Goal Attainment Scaling (GAS) is a person-centered and collaborative approach used to assess the effectiveness of interventions on personally relevant goals. However, GAS is a heterogeneous group of methodologies with variations and lack of consensus on high quality GAS. This communication aims to provide updated information on GAS use in PRM practice and research, address methodological challenges, guide the integrated use of GAS in rehabilitation, and offer resources for self-directed learning.

Summary: The study found that MBOOI was the only baseline parameter correlated with improvements in IPSS, QoL, Qmax, and overall efficacy in predicting surgical efficacy for men receiving TURP. Additionally, ROC analysis confirmed that MBOOI was more optimal than BOOI, TZV, and TZI in predicting surgical efficacy.

Summary: The objective of this study was to assess the objective association between bladder outlet obstruction (BOO) and abnormal sensation parameters during filling cystometry in men. The results indicated a strong association between bladder oversensitivity and BOO in men. Men with first desire to void (FDV) <140 mL had a significantly increased probability of being obstructed.

Summary: Non-invasive biomarkers are still needed to identify patients with bladder outlet obstruction (BOO)-related dysfunction. In a study involving rats and BPH patients, it was found that FGF2 is involved in the development of bladder hypertrophy in partial BOO and is positively correlated with prostate volume in BPH patients. BDNF, on the other hand, showed no correlation with prostate volume, age, or BMI.

Summary: This study investigated the correlation between nitric oxide (NO) and urodynamics in men with bladder outlet obstruction (BOO). The results showed that nitric oxide synthase (NOS) expressed in the urothelium was associated with urodynamic parameters, and neuronal NOS (nNOS) may reflect changes in detrusor function.

Summary: This study developed a clinical outcome assessment tool through a multi-step process including expert workshops and patient-centered individual interviews, resulting in a neurogenic bladder/bowel dysfunction goal menu consisting of 25 specific goals for this population. Most participants identified Impact on Life goals as most important, with main themes including challenges posed by incontinence, limitations on everyday life, and the need for personalized care.

Summary: Prevention strategies have been effective in many areas of human health, yet have not been utilized for lower urinary tract symptoms (LUTS) or bladder health (BH). The NIH-sponsored Prevention of Lower Urinary Tract Symptoms Research Consortium (PLUS) aims to enhance traditional prevention science approaches through a transdisciplinary team science approach, mixed methodology, and community engagement. Their foundational work includes developing clear definitions of BH and disease, as well as a BH measurement instrument that will be validated for use in the general population, adolescents, and Latinx and Spanish-speaking women.

Summary: Traditionally, the bladder has been considered sterile, but microbial communities have been detected in both adult females and males. These urobiomes may influence the development of lower urinary tract disorders, and there is potential for treating symptoms by modifying the urobiome in the future.

Summary: Research found that vaginal intercourse shifts the lower urinary tract microbiota, particularly noting an increase in Streptococcus mitis abundance. Isolates of S. mitis from both partners were genetically similar, indicating potential transmission. This post-coitus bloom in S. mitis may provide a window of opportunity for uropathogens to colonize the urinary tract.

Summary: The advancement of women leaders in obstetrics and gynecology does not match the changes in the physician workforce. Gender equity should be a core value in the culture of obstetrics and gynecology, and actions should be taken to prioritize and demonstrate commitment to gender equity. This commentary proposes principles and leadership practices to promote gender equity in academic leadership roles within obstetrics and gynecology.

Summary: This study aims to understand the distribution of bladder health and the factors that promote bladder health and prevent LUTS in women. It is a population-based longitudinal cohort study conducted in the United States, involving community-dwelling women. Data collection includes self-administered surveys and objective measures. The findings will contribute to the development of interventions for bladder health promotion and LUTS prevention in women.

Summary: This article presents the case of a 30-year-old woman who seeks specialty consultation for frequent episodes of urinary urgency, frequency, and dysuria. Although her home dipsticks suggest a urinary tract infection (UTI) and her urinalysis shows a moderate number of white blood cells, her urine cultures are always negative. The challenges of this clinical presentation are discussed along with evidence-based evaluation and treatment options.
