Summary: A 67-year-old woman presented with shortness of breath, and initial ventilation/perfusion scan showed mismatched bilateral apical defects indicating high probability of pulmonary emboli. However, investigation revealed that the defects were caused by incorrect patient positioning during Tc-99m-MAA administration. Repeat scan with correct patient positioning showed no defects. Incorrect patient positioning could have led to a misdiagnosis of pulmonary emboli and inappropriate treatment.

Summary: For patients evaluated for PE, a history of asthma in the past 10 years does not affect the diagnostic performance of imaging techniques such as VQ or CTPA. Patients with asthma are not more likely to have inconclusive results, require repeat exams, or experience false negatives compared to patients without asthma.

Summary: The occurrence of pulmonary embolism in pregnant women is increasing, possibly due to improved healthcare and an increase in high-risk groups. Obstetricians should be vigilant for venous thrombosis and pulmonary embolism and provide preventive measures, early diagnosis, and prompt treatment. Postpartum management is also crucial to address subsequent health issues.

Summary: This study evaluated the pregnancy outcomes in women with congenital heart disease (CHD) with and without pulmonary hypertension (PH). The results showed that women with CHD-associated mild PH had better pregnancy outcomes compared to those with moderate-to-severe PH, with similar outcomes to women with CHD and no PH.

Summary: Multiple mechanisms contribute to essential hypertension, and antihypertensive drugs primarily target increased sympathetic nervous system activity, altered production of vasoactive mediators, vascular inflammation, fibrosis, and increased peripheral resistance. C-type natriuretic peptide (CNP) is an endothelium-derived peptide that influences vascular signaling through two receptors, NPR-B and NPR-C. This article summarizes the effects of CNP on the vasculature in relation to essential hypertension and proposes targeting the CNP system as a potential tool for managing long-term essential hypertension.

Summary: This study presents a 10-year follow-up of thalassemia patients with invasively documented pulmonary hypertension, revealing that pulmonary arterial hypertension (PAH) is highly lethal even with sub systemic pressures. The study also highlights the importance of echocardiography in predicting survival based on changes in predicted pulmonary artery pressure.

Summary: This study evaluated the changes in incidence, risk factors, and outcomes of ICH in children with ITP over different time periods. It found that persistent low platelet counts < 10 x 10^9/L were significant risk factors for ICH in both time periods, while head trauma and hematuria were more commonly associated with ICH in the most recent study. Outcome was found to be comparable in both time periods.

Summary: This study validated a previous prediction model for inhibitor development in hemophilia patients and developed a new model with four predictors. The new model had a C-statistic of 0.66 with moderate calibration, and positive and negative predictive values of 0.22 and 0.95 at a 10% cut-off point.

Summary: During the SARs-COV-2 pandemic, there was a decrease in ER visits for G6PD-deficient children with AHC, and most patients were able to tolerate lower hemoglobin levels. Telemedicine proved to be an efficient tool in supporting their families. A restrictive transfusion strategy was clearly demonstrated in this study.

Summary: This study analyzed 155 consecutive patients with clinical suspicion of hereditary erythrocyte defects, obtaining an overall diagnostic yield of 84% with monogenic inheritance seen in 69% and multi-locus inheritance in 15% of the tested patients. Some patients showed dual molecular diagnosis of different red blood cell disorders, indicating the importance of genetic testing for patients with complex modes of inheritance.
