Cardiomyopathy in Duchenne Muscular Dystrophy: Current Value of Clinical, Electrophysiological and Imaging Findings in Children and Teenagers

Background: Progressive cardiomyopathy (CMP) is one main cause of death in DMD. This cross-sectional assessment of different cardiac diagnostic procedures focusses on preterm diagnosis of cardiac dysfunction. Patients: 39 male DMD patients aged 6-20 years were included. 6 patients were still ambulatory, 21 patients received corticosteroid therapy. Methods: All patients were investigated by ECG, Holter ECG and heart rate variability (HRV), B-type natriuretic peptide (BNP), echocardiography (TTE), tissue Doppler Imaging (TD) and magnetic resonance imaging (MRI) with Late Gadolinium enhancement (LE) and segmental wall motion analysis (WMA). Results: 56% of the patients showed repolarization abnormalities and 76% altered HRV. Subnormal ventricular function was found in 25% by TTE and in 34% by MRI. TD differed from normal controls only in the apical septum. In MRI 89% of the patients showed different distribution and intensity of LE and WM restriction. The extent of LE was less in patients after steroid treatment (p < 0.05). Discussion: MRI with segmental LE- and WM-analysis seems to be superior to TTE and TD in exploring regional distribution and severity of damage of the myocardium. ECG and HRV abnormalities are common in DMD-patients but not tightly predictive for segmental and global left ventricular dysfunction. Targeted treatment of CMP in DMD needs prospective evaluation. Conclusion: A timely cardiac MRI is the most sensitive investigation for the identification of early myocardial changes in DMD which is a prerequisite for early interventions and therapeutic strategies.

New kids on the CPET: age-appropriate outdoor cardiopulmonary exercise testing in preschoolers

Summary: This study compares the results of a mobile device-based outdoor running test with a treadmill test. The results show that the outdoor test is significantly shorter in duration and yields higher values for almost all cardiopulmonary variables. Testing very young children with a mobile device is a new alternative that allows for individualized and standardized exercise testing.

The Congenital Cardiology Cloud: Proof of feasibility of Germany's first tele-medical network for pediatric cardiology

In order to bridge the gap between surgical and interventional cardiology data, the ECHSA has collaborated with The Association for European Paediatric and Congenital Cardiology to expand the database and capture data about interventional cardiology procedures. This new module aims to provide centers with access to their own outcome data as well as national and international aggregate data for benchmarking, and facilitate comparison between surgical and catheter interventional outcomes.

Impact of premature birth on cardiopulmonary function in later life

Summary: Pulmonary function is reduced in children after preterm birth, regardless of the presence of bronchopulmonary dysplasia and mechanical ventilation history. However, the impact on cardiopulmonary function in these children is unclear. This study found that moderate to late premature birth did not lead to limitations in cardiopulmonary function in comparison with term-born controls.

Summary: This study evaluated the performance and durability of pulmonary valve replacement procedures in Germany, including both transcatheter and surgical valve replacements. The results showed that transcatheter valves had equivalent performance to surgical valves and extended durability. Different types of valves had different levels of durability, with homografts performing the best and Matrix P-valves performing the worst. The interval between procedures shortened significantly in most age groups below 40 years.
