Effect of Low-dose, Enteric Coated Aspirin on Gastrointestinal Bleeding in Patients with Coronary Artery Disease

Background/Aims: This study was performed to determine whether low-dose aspirin and/or clopidogrel can induce gastrointestinal bleeding and gastroduodenal mucosal injury. Methods: A total of 387 patients who underwent coronary angiography at Seoul National University Bundang Hospital were assigned to one of three antiplatelet treatment groups: (1) control, (2) 100-mg enteric coated aspirin, and (3) 100-mg enteric coated aspirin plus clopidogrel. The incidences of gastroduodenal mucosal injury and gastrointestinal bleeding were prospectively evaluated, and risk factors for gastrointestinal bleeding were analyzed. Results: The rate of gastroduodenal mucosal injury was higher in the aspirin-plus-clopidogrel group than in the aspirin group (p=0.012), and higher in the aspirin group than in the control group (p=0.049). The rate of gastrointestinal bleeding was significantly higher in the aspirin-plus-clopidogrel group (9.4%) than in the control group (2.4%, p=0.048). The risk factors for gastrointestinal bleeding were older age (>= 60 years) and the presence of at least two comorbid disorders. Conclusions: Low-dose enteric coated aspirin was found to be safe in patients with coronary artery disease, but the addition of clopidogrel increased the rate of gastrointestinal bleeding. Combined clopidogrel and aspirin should be used with caution in older patients having at least two comorbid conditions. (Gut and Liver 2008;2:99-104)

Effect of Helicobacter pylori infection and its eradication on the expression of tight junction proteins in the gastric epithelium in relation to gastric carcinogenesis

Summary: This study evaluated the expression of tight junction proteins (TJPs), including claudin-1 and -2, occludin, and tight junction protein 1 (TJP1), in relation to Helicobacter pylori (Hp) infection and eradication in gastric carcinogenesis. The results showed that Hp infection affects TJP expression, with lower expression of TJP1 in Hp(+) controls and higher expression in Hp(+) gastric cancer (GC) patients. However, the high expression of TJP1 in Hp(+) GC patients was restored to control levels after Hp eradication.

Effect of Clostridium butyricum on High-Fat Diet-Induced Intestinal Inflammation and Production of Short-Chain Fatty Acids

Summary: This study aimed to investigate the response of rat colon to a high-fat diet (HFD) and the effect of Clostridium butyricum on HFD-induced intestinal inflammation and production of short-chain fatty acids (SCFAs) according to sex. The results showed that HFD significantly increased inflammation scores and fat proportions. Clostridium butyricum improved HFD-induced colonic inflammation, increased SCFA production, and enhanced the expression of tight junction proteins. These effects were more pronounced in male rats, showing a sex difference.

Rehabilitation Program for Improved Musculoskeletal Pain in Gastrointestinal Endoscopists: Multicenter Prospective Cohort Study

Summary: This study aimed to develop a rehabilitation program for musculoskeletal pain in gastrointestinal endoscopists and determine its usefulness. A multicenter cohort study was conducted with 118 participants, and it was found that 94% of them reported musculoskeletal pain at the beginning. The follow-up surveys revealed that musculoskeletal pain was associated with equipment/posture performance. Conclusion: Our rehabilitation program is effective and satisfactory in improving musculoskeletal pain in gastrointestinal endoscopists.

Prevalence and trends of multiple antimicrobial resistance of Helicobacter pylori in one tertiary hospital for 20 years in Korea

Summary: This study assessed the antimicrobial resistance rates and trends of Helicobacter pylori in Korean patients. The results showed an increasing resistance rate to clarithromycin, metronidazole, and fluoroquinolones. The prevalence of multidrug resistance also significantly increased.

Data from 14,603 patients with gastric cancer were analyzed, and the incidence of SMGC was found to be 4.04%, which was associated with male sex, age ≥65 years, and early gastric cancer. Unlike single gastric cancer, SMGC had a high survival rate.

Sex/gender differences in gastrointestinal endoscopy from the perspective of patients and gastroenterologists

Summary: The sex/gender of gastroenterologists has an impact on patients' satisfaction, compliance, and clinical outcomes. Female gastrointestinal (GI) endoscopist-patient gender concordance improves health-related outcomes, indicating the need for increased number of female GI endoscopists. While the number of women in the field of gastroenterology is increasing, it is not sufficient to meet the gender preferences of female patients. Gastroenterologists, particularly male endoscopists, are at a high risk of endoscopy-related injuries, with different distribution of injuries between males and females.

Sex Difference of Colon Adenoma Pathway and Colorectal Carcinogenesis

Summary: Colorectal cancer is a common cancer that affects both men and women, but there are significant sex differences. Men have a higher risk and earlier onset of CRC, suggesting a protective role of estrogen. Female patients are more likely to develop aggressive right-sided colon cancer. There are differences in carcinogenic mechanisms between left and right CRC, with sex-based biological and sociocultural factors playing a role.
