Effect of Aging, Gender and Sensory Stimulation of TRPV1 Receptors with Capsaicin on Spontaneous Swallowing Frequency in Patients with Oropharyngeal Dysphagia: A Proof-of-Concept Study

Weslania Nascimento, Noemi Tomsen, Saray Acedo, Cristina Campos-Alcantara, Christopher Cabib, Marta Alvarez-Larruy, Pere Clave

Summary: The study found that age and gender have an impact on spontaneous swallowing frequency in healthy individuals, with older age associated with lower spontaneous swallowing frequency. Gender did not have a significant effect on spontaneous swallowing frequency. Sensory stimulation with TRPV1 agonist capsaicin significantly increased spontaneous swallowing frequency in patients with post-stroke oropharyngeal dysphagia, suggesting a potential therapeutic strategy for central pattern generator activation in these patients.

Recovery Focused Nutritional Therapy across the Continuum of Care: Learning from COVID-19

Emanuele Cereda, Pere Clave, Peter F. Collins, Anne Holdoway, Paul E. Wischmeyer

Summary: Targeted nutritional therapy is crucial for COVID-19 patients, starting early and continuing through recovery. While literature emphasizes the importance of nutrition during the acute phase, there is a lack of guidance for practices across the entire patient journey.

Assessment of Swallowing Disorders, Nutritional and Hydration Status, and Oral Hygiene in Students with Severe Neurological Disabilities Including Cerebral Palsy

Alicia Costa, Alberto Martin, Viridiana Arreola, Stephanie A. Riera, Ana Pizarro, Cristina Carol, Laia Serras, Pere Clave

Summary: The study evaluated the prevalence of oropharyngeal dysphagia, malnutrition, dehydration, and oral health in students at special needs schools. The results showed that cerebral palsy was the main diagnosis, with students commonly experiencing malnutrition, dysphagia, and poor oral health.

The Hydration Status of Adult Patients with Oropharyngeal Dysphagia and the Effect of Thickened Fluid Therapy on Fluid Intake and Hydration: Results of Two Parallel Systematic and Scoping Reviews

Paula Vinas, Mireia Bolivar-Prados, Noemi Tomsen, Alicia Costa, Sergio Marin, Stephanie A. Riera, Nuria Barcons, Pere Clave

Summary: This article aims to assess the hydration status in patients with oropharyngeal dysphagia (OD) and the effect of thickened fluid (TF) therapy on it. Results show that dehydration is a common complication in OD and TF therapy has a positive effect on hydration status, although strict monitoring of fluid intake is necessary.

A Comparative Study on the Effect of Acute Pharyngeal Stimulation with TRP Agonists on the Biomechanics and Neurophysiology of Swallow Response in Patients with Oropharyngeal Dysphagia

Noemi Tomsen, Omar Ortega, Daniel Alvarez-Berdugo, Laia Rofes, Pere Clave

Summary: This study compared the effects of increasing bolus viscosity and using TRPV1, TRPA1, or TRPM8 agonists on the swallow response in patients with oropharyngeal dysphagia (OD). The results showed that fluid thickening and substances like capsaicin and piperine significantly improved swallow safety and speed, providing potential candidates for the development of new pharmacological strategies.

A Comparative Study between the Three Waves of the Pandemic on the Prevalence of Oropharyngeal Dysphagia and Malnutrition among Hospitalized Patients with COVID-19

Paula Vinas, Alberto Martin-Martinez, Claudia Alarcon, Stephanie A. Riera, Jaume Miro, Cristina Amado, Pere Clave, Omar Ortega

Summary: The prevalence of oropharyngeal dysphagia (OD), malnutrition (MN), and mortality among COVID-19 patients was found to be very high. OD was independently associated with MN and mortality. Early and proactive multimodal nutritional intervention can improve the nutritional status of patients.

Economic Evaluation of Clinical, Nutritional and Rehabilitation Interventions on Oropharyngeal Dysphagia after Stroke: A Systematic Review

Sergio Marin, Omar Ortega, Mateu Serra-Prat, Ester Valls, Laia Perez-Cordon, Pere Clave

Summary: This study aimed to assess the efficiency of healthcare interventions in post-stroke oropharyngeal dysphagia (PS-OD) management. The findings suggest that screening and diagnosis of PS-OD can adjust and reduce hospitalization costs, while OD management after thrombolysis may not significantly reduce hospitalization costs. Videofluoroscopy is found to be the most cost-effective screening method. Rehabilitation programs and home enteral nutrition are also shown to be cost-effective. However, further evaluation and research on novel strategies are still needed.

Shear-Viscosity-Dependent Effect of a Gum-Based Thickening Product on the Safety of Swallowing in Older Patients with Severe Oropharyngeal Dysphagia

Mireia Bolivar-Prados, Yuki Hayakawa, Noemi Tomsen, Viridiana Arreola, Weslania Nascimento, Stephanie Riera, Satomi Kawakami, Kazuhiro Miyaji, Yasuhiro Takeda, Jun Kayashita, Pere Clave

Summary: The study aimed to determine the therapeutic effect of the xanthan-gum-based thickener Tsururinko Quickly (TQ) in older patients with severe oropharyngeal dysphagia (OD). The results showed that TQ greatly improved the safety of swallowing and reduced the occurrences of penetration and aspiration in a shear-viscosity-dependent manner. The therapeutic range for TQ was found to be 100-800 mPa·s, with 200 and 800 mPa·s being the optimal doses for older patients with OD.

Therapeutic Effect on Swallowing Function and on Hydration Status of a New Liquid Gum-Based Thickener in Independently-Living Older Patients with Oropharyngeal Dysphagia

Noemi Tomsen, Mireia Bolivar-Prados, Omar Ortega, Pere Clave

Summary: ThickenUp (R) Gel Express (TUGE) is a new liquid thickening product based on xanthan and acacia gum, which can improve swallowing difficulties in older adults without negative effects on oral and pharyngeal residues. During a 14-day treatment with TUGE-thickened fluids, older adults experienced increased fluid intake, improved hydration status, and good tolerance.

Characterization of Dysphagia Thickeners Using Texture Analysis-What Information Can Be Useful?

Raquel Baixauli, Mireia Bolivar-Prados, Kovan Ismael-Mohammed, Pere Clave, Amparo Tarrega, Laura Laguna

Summary: In addition to shear viscosity, other texture parameters such as adhesiveness and cohesiveness are important for safe swallowing in individuals with oropharyngeal dysphagia. Texture analyzers can provide valuable information on food properties, but the protocols and instruments for assessing adhesiveness and cohesiveness are limited. This study aimed to characterize four dysphagia thickeners using different texture analyzer settings and found that the choice of test type and probe influenced the results, while the test speed could be standardized to 1 mm/s.
