Risk of vascular diseases in patients with dermatitis herpetiformis and coeliac disease: a long-term cohort study

Summary: This cohort study investigated the risk for vascular diseases in dermatitis herpetiformis (DH) and coeliac disease patients. The results showed that compared to coeliac disease patients, DH patients had a decreased risk for cerebrovascular diseases, while coeliac disease patients had an increased risk for cerebrovascular and cardiovascular diseases. Coeliac disease, but not DH, was associated with an increased risk for venous thrombosis.

Infections in early life as risk factor for coeliac disease

Summary: Infections in the first 6 months of life increase the risk of developing CD, especially for respiratory infections, and particularly if there are 3 or more episodes. The use of antibiotics in this period of life was not found to be associated with an increased risk of CD.

The diagnosis of CeD usually precedes the diagnosis of IBD, and patients with both IBD and CeD receive more intensified treatment for IBD.

An update on coeliac disease from the NHS England National Centre for Refractory Coeliac Disease

Summary: Coeliac disease is a common autoimmune-mediated gastrointestinal disorder with a prevalence of around 1%, but many cases remain undiagnosed. The cornerstone of management is strict adherence to a gluten-free diet with support and education from a gastrointestinal dietitian, sometimes requiring re-evaluation of symptoms and repeat biopsies. Refractory CD affects a small subset of individuals and requires specialist intervention.

Summary: The review of human observational studies on microbes and coeliac disease found that early childhood infections were associated with increased risk of subsequent coeliac disease. There was some evidence for an association between certain microbial factors and coeliac disease, such as Helicobacter pylori, adenovirus, and enterovirus. Most studies on gut microbiota were cross-sectional and lacked prospective data. Larger prospective studies are needed to further understand the link between microbes and coeliac disease.

Case-control study of the concurrence of coeliac disease with inflammatory bowel disease

Summary: This case-control study used a large histopathology database to investigate the association between inflammatory bowel disease (IBD) and coeliac disease. The study found a lower prevalence of coeliac disease in IBD patients (0.4%) compared to the control population (0.7%), while the prevalence of coeliac disease in microscopic colitis (MC) patients was higher (4.4%). The results suggest a possible bias in previous studies that reported a positive association between coeliac disease and IBD due to the inclusion of MC cases in the IBD patient population.

What are the clinical consequences of 'potential' coeliac disease?

Summary: This study compared the clinical presentation of potential coeliac disease (PCD) with coeliac disease (CD). The findings showed that PCD patients had fewer extraintestinal manifestations and less haematinic deficiencies compared to CD patients. Summary: This study aimed to identify the benefit-risk trade-offs that IBD patients are willing to make in order to inform patient-centric treatment targets. The study found that efficacy, administration, and safety all had varying degrees of impact on treatment preferences. Factors such as abdominal pain, risks of mild-to-moderate side effects, and serious infections were considered important. Preferences varied among patients, with CD patients placing greater importance on abdominal pain and UC patients on bowel urgency.

Summary: This study evaluated the correlation and predictive value of a new and established index in ulcerative colitis. The results showed that PHRI had a similar correlation with endoscopy and was able to effectively stratify the risk of relapse. This study demonstrates that a neutrophil-only assessment is a simple and viable alternative.

Summary: As the patents for biologic originator drugs expire, biosimilars are emerging as cost-effective alternatives within healthcare systems. A global survey was conducted to shed light on physicians' knowledge, beliefs, practical approaches, and concerns related to biosimilar adoption. Based on the survey results, a consensus was reached on key statements covering biosimilar effectiveness, safety, indications, rationale, multiple switches, therapeutic drug monitoring, non-medical switching, and future perspectives. The consensus affirmed that biosimilars are equally effective and safe compared to originator drugs.
