Summary: This study evaluated the diagnostic value of combining transthoracic echocardiography (TTE) and fluoroscopy (F) in patients with mechanical aortic and mitral prosthetic valve dysfunction. The results showed that TTE demonstrated good sensitivity and specificity, while F had high specificity but low sensitivity. Integrating both TTE and F significantly improved accuracy, making it useful for patients with suspected PVD.

Summary: This study describes the utilization of both two-dimensional (2D) and three-dimensional (3D) transesophageal echocardiography (TEE) in successfully performing transcatheter mitral valve (MV) in patients with thrombus in left atrial appendage (LAA) and/or left atrium (LA) body, which is considered a contraindication for the procedure. The procedure was successful in all patients with improvement of symptoms and no complications over a mean follow-up of 21 months.

Summary: The aim of this study was to compare the accuracy of two-dimensional and three-dimensional transesophageal echocardiography (TEE) in detecting pannus and thrombus in left mechanical valve obstruction (LMVO). The results showed that three-dimensional TEE had higher sensitivity, specificity, and accuracy compared to two-dimensional TEE in diagnosing thrombus and pannus. Three-dimensional TEE can be a reliable imaging modality for identifying the causes of LMVO.

Summary: In patients with severe aortic stenosis and moderate aortic regurgitation, the transvalvular flow velocity and mean pressure gradient are significantly affected by the regurgitation, while the effective orifice area and the ratio between maximum velocity of the left ventricular outflow tract and transvalvular flow velocity are not. Borderline cases of aortic stenosis severity should be verified using geometric orifice area.

Summary: This study aimed to evaluate the outcomes of thrombolytic therapy (TT) and surgery as the first-line treatment strategy in patients with obstructive prosthetic valve thrombosis (PVT). The results showed that low-dose and slow/ultraslow infusion of tissue plasminogen activator (t-PA) were associated with low complications and mortality and high success rates.

Understanding the complexity of the tricuspid valve anatomy and pathology is crucial for surgical planning. Three-dimensional echocardiography provides detailed images of the tricuspid valve and surrounding structures, aiding in the assessment of disease etiology and severity as a supplement to two-dimensional imaging.

Summary: Although transcatheter valve therapy is rapidly evolving, surgical valve replacement is still required in many patients with severe left-side valve stenosis or regurgitation, and lifelong efficient anticoagulation of these patients remains problematic. The availability of multimodal imaging techniques enables accurate diagnosis of mechanical valve thrombosis and aids in establishing the treatment method and timing. This article provides an overview of the essential role of non-invasive exploration in the treatment of mechanical prosthetic valve thrombosis.
