Summary: G-POEM has been shown to be a safe and effective treatment for refractory gastroparesis, with adverse events typically being mild and managed conservatively. The experience of the endoscopist plays a significant role in reducing the incidence of adverse events.

Summary: Endoscopic submucosal dissection (ESD) is a highly effective treatment for superficial gastric neoplasia in North America, with low risk of local recurrence. Close endoscopic surveillance post-ESD is crucial for identifying and treating metachronous lesions.

Gastric per-oral endoscopic myotomy (G-POEM) for refractory gastroparesis: results from an international prospective trial

Kia Vosoughi, Yervant Ichkhanian, Petros Benias, Larry Miller, A. Aziz Aadam, Joseph R. Triggs, Ryan Law, William Hasler, Nicole Bowers, Dalton Chaves, Alberto M. Ponte-Neto, Peter Draganov, Dennis Yang, Maan El Halabi, Omid Sanaei, Olaya Isabella Brewer Gutierrez, Robert Stephen Bulat, John Pandolfino, Mouen Khashab

Summary: Although G-POEM is considered a safe procedure, it only showed modest overall effectiveness in the treatment of refractory gastroparesis. High baseline GCSI Score, high baseline gastric retention, and early response to G-POEM were independent predictors of clinical success at 12 months. Further studies are needed to identify the best candidates for G-POEM.

Factors Associated With Inpatient Endoscopy Delay and its Impact on Hospital Length-of-Stay and 30-Day Readmission

Chelsea C. Jacobs, Johnny F. Jaber, Michael Ladna, Sandeep Ponniah, Ishaan Madhok, Nicole Ruiz, Haley Podeschi, Francesca Gesiotto, Walid Khan, Nabeel Moon, April Mathews, Vikas Patel, Brett Styskel, Tony S. Brar, Peter V. Draganov, Dennis Yang

Summary: This study investigated inpatient endoscopy delay and found that factors such as older age, female sex, use of specific medications, and certain types of procedures are associated with delay. Delayed endoscopy prolonged hospital length-of-stay and was identified as an independent predictor of 30-day readmission.

Endoscopic submucosal dissection vs. endoscopic mucosal resection for early Barrett's neoplasia in the West: a retrospective study

Lady Katherine Mejia Perez, Dennis Yang, Peter Draganov, Salmaan Jawaid, Amitabh Chak, John Dumot, Omar Alaber, John J. Summary: The study compared the outcomes of endoscopic submucosal dissection (ESD) and endoscopic mucosal resection (EMR) for early Barrett's esophagus neoplasia, finding that ESD had higher en bloc and R0 resection rates, lower recurrence/residual disease rates, and less need for repeat endoscopic treatments compared to EMR.

Outcomes for endoscopic submucosal dissection of pathologically staged T1b esophageal cancer: a multicenter study

Summary: This study evaluated the technical outcomes and recurrence rates of ESD in treating T1b esophageal cancer. It found that EUS staging had poor accuracy, R0 resection rates were low, and R1 resection was associated with recurrent disease. Close surveillance is necessary for noncurative ESD resection patients who cannot undergo surgery.

Dennis Yang, Peter Draganov, William King, Nanlong Liu, Ahmed Sarheed, Adnan Bhat, Peter Jiang, Michael Ladna, Nicole C. Ruiz, Jake Wilson, Venkata S. Gorrepati, Heiko Pohl

Summary: In this single-center historical control study, EMR-MM of large nonpedunculated colorectal polyps reduced the recurrence risk by 80% compared with conventional EMR. This simple technique may provide an alternative to margin ablation.

Effect of endoscopic submucosal dissection on histologic diagnosis in Barrett's esophagus visible neoplasia

Jawaid, Yaseen B. Perbtani, Brenda J. Hoffman, Ashwin S. Akki, Alexander Schlachterman, Roxana M. Coman, Andrew Y. Wang, Peter V. Draganov

Summary: This study found that ESD can change the diagnosis in patients with BE visible neoplasia, particularly in those with suspected invasive disease. Lesions in the distal esophagus and gastroesophageal junction, as well as prior radiofrequency ablation, were predictors of change in histologic diagnosis.

Aparicio, Guilherme Macedo, Peter V. Draganov, Peter Vilmann, Leandro Pecchia, Alessandro Repici, Stefano Francesco Crino

Summary: This study identified independent predictors for adverse events (AEs) related to endoscopic ultrasound-guided through-the-needle biopsy (TTNB) of pancreatic cystic lesions (PCLs). Age, number of TTNB passes, complete aspiration of the cyst, and diagnosis of intraductal papillary mucinous neoplasm (IPMN) were found to be significant predictors. Recursive partitioning analysis (RPA) classified patients into high-risk, low-risk, and middle-risk classes. The findings support the selective use of TTNB in the evaluation of patients with IPMN.

Endoscopic submucosal dissection for colorectal dysplasia in inflammatory bowel disease: a US multicenter study

Saowanee Ngamruengphong, Hiroyuki Aihara, Shai Friedland, Makoto Nishimura, David Faleck, Petros Benias, Dennis Yang, Peter V. Draganov, Nikhil A. Kumta, Zachary A. Borman, Rebekah E. Dixon, James F. Marion, Lionel S. D'Souza, Yutaka Tomizawa, Simran Jit, Sonmoon Mohapatra, Aline Charabaty, Alyssa Parian, Mark Lazarev, Esteban J. Figueroa, Yuri Hanada, Andrew Y. Wang, Louis M. Wong Kee Song

Summary: ESD is a safe and effective treatment for dysplastic colorectal lesions in patients with inflammatory bowel disease. Despite the high prevalence of submucosal fibrosis, en bloc resection was achieved in nearly all cases. Careful endoscopic surveillance is necessary to monitor for local recurrence and metachronous lesions after ESD.
