Summary: We study the welfare effects of offering choice over coverage levels in regulated health insurance markets. We find that heterogeneity in efficient coverage level is not sufficient to motivate choice. Vertical choice is efficient only if consumers with higher willingness to pay have a higher efficient level of coverage. Empirical investigation shows that as long as a minimum coverage level can be enforced, the welfare gains from vertical choice are either zero or economically small.

Summary: Resection of adenomatous colorectal polyps has been proven to reduce mortality from colorectal cancer. Cold snare polypectomy (CSP) is a simpler and safer method for the resection of colorectal polyps smaller than 10mm, with advantages in shortening procedure time, reducing bleeding risk, and lowering treatment costs compared to conventional methods. This review summarizes previous studies on the efficacy and safety of CSP, as well as discusses potential expansions of its indications and optimal treatments for recurrent lesions.

Summary: This article discusses the debate on whether videolaryngoscopy (VL) should be the standard of care for tracheal intubation. Dr Aziz argues in favor of VL being the standard of care, while Dr Berkow challenges that assertion. The benefits of VL include improved first-pass success rates, reduced risk of intubation failure and esophageal intubation, and benefits in difficult airway patients. However, VL is not without complications and does not guarantee a 100% success rate. The transition to VL as the standard of care may lead to a decline in competency in other airway techniques.

Bariatric endoscopic and surgical therapies are considered effective strategies for improving quality of life, reducing cardiovascular risk, and enhancing metabolic profile, despite being costly. This review suggests considering these therapies early, along with liver fibrosis evaluation, in order to achieve long-term cost-effectiveness when lifestyle modifications and pharmacological treatments fail.
