Impact of COVID-19 and suspension of colorectal cancer screening on incidence and stage distribution of colorectal cancers in the Netherlands

Esther Toes-Zoutendijk, Geraldine Vink, Iris D. Nagtegaal, Manon C. W. Spaander, Evelien Dekker, Monique E. van Leerdam, Sabine Siesling, Iris Lansdorp-Vogelaar, Marloes A. G. Elferink

Summary: This study assessed the impact of the COVID-19 pandemic on colorectal cancer (CRC) incidence and stage distribution in the Netherlands. The findings showed a significant decrease in CRC incidence during the pandemic, particularly for stage I tumors. However, after gradually resuming screening, the number of CRC diagnoses increased.

Colectomy and desmoid tumours in familial adenomatous polyposis: a systematic review and meta-analysis

Arthur S. Aelvoet, Daphne Struik, Barbara A. J. Bastiaansen, Willem A. Bemelman, Roel Hompes, Patrick M. M. Bossuyt, Evelien Dekker

Summary: This systematic review and meta-analysis aimed to examine the relationship between the type, approach, and timing of colectomy and the risk of Desmoid tumors (DT) in patients with familial adenomatous polyposis (FAP). The results indicate that the current literature does not provide conclusive evidence regarding the impact of colectomy type, approach, or timing on DT risk in FAP patients.

Gastric metaplasia could initiate the serrated neoplasia pathway in CRC

Joep E. G. IJspeert, Evelien Dekker

Summary: This new study reveals that a subset of colorectal cancers originates from sessile serrated lesions, and the tumorigenic process through the serrated pathway is initiated by gastric metaplasia induced by damage and repair triggered by disadvantageous microbiota composition in the proximal colon.

Summary: This study evaluated the changes in colorectal cancer (CRC) incidence, mortality, and treatment before and after the introduction of a screening programme in the Netherlands. Results showed a decrease in overall and advanced-stage CRC incidence after the introduction of the screening programme, with more screening-detected tumors located in the left side of the colon and detected at an early stage.

Management of familial adenomatous polyposis and MUTYH-associated polyposis; new insights

Arthur S. Aelvoet, Francesco Buttitta, Luigi Ricciardiello, Evelien Dekker

Summary: Familial adenomatous polyposis (FAP) and MUTYH-associated polyposis (MAP) are rare inherited polyposis syndromes with a high risk of colorectal cancer (CRC). Patients require frequent endoscopic surveillance, polypectomy, and chemopreventive treatment to reduce the burden of polyps in the colon and upper gastrointestinal tract.

Summary: The study suggests that in LS carriers, complete colonoscopies with adequate bowel preparation and chromoendoscopy use improve adenoma detection, while surveillance intervals of less than 3 years are associated with a reduction in PCCRC incidence. High-quality colonoscopy surveillance is crucial for preventing CRC in LS.

Summary: This study compared the accuracy of an optical diagnosis system with endoscopists in real-time colonoscopy for the detection of diminutive colorectal polyps. The results showed that there was no significant difference in accuracy between the computer-aided diagnosis system and the endoscopists.

Factors associated with interval colorectal cancer after negative FIT: Results of two screening rounds in the Dutch FIT-based CRC screening program

Emilie C. H. Breekveldt, Esther Toes-Zoutendijk, Hilliene J. van de Schootbrugge-Vandermeer, Lucie de Jonge, Arthur I. Kooyker, Manon C. W. Spaander, Anneke J. van Vuuren, Folkert J. van Kemenade, Christian Ramakers, Evelien Dekker, Iris D. Nagtegaal, Monique E. van Leerdam, Iris Lansdorp-Vogelaar

Summary: The interval colorectal cancer (CRC) rate after negative fecal immunochemical testing (FIT) is an important quality indicator of CRC screening programs. Our study analyzed the outcomes of a FIT-based CRC screening program in the Netherlands, showing low incidence of interval CRC and high FIT-sensitivity. Risk-stratified CRC screening based on prior fecal hemoglobin concentration (f-Hb) may further optimize the screening program.

Appendiceal lesions in serrated polyposis patients are easily overlooked but only seldomly lead to colorectal cancer

David E. F. W. M. van Toledo, Joep E. G. Ijspeert, Arne G. C. Bleijenberg, Barbara A. J. Bastiaansen, Carel J. M. van Noesel, Evelien Dekker

Summary: This study assessed the prevalence and clinical relevance of appendiceal lesions in a large cohort of Serrated polyposis syndrome (SPS) patients. The results showed that although appendiceal lesions were common in SPS patients, the proportion of advanced lesions was low, indicating limited clinical significance.

High quality colonoscopy: using textbook process as a composite quality measure

Karlijn J. Nass, Sascha C. van Doorn, Paul Fockens, Colin J. Rees, Maria Pellise, Manon van der Vlugt, Evelien Dekker

Summary: This study establishes the textbook process (TP) as a new composite measure for the optimal colonoscopy process and evaluates its attainment in clinical practice and variation between endoscopists. TP was defined through a Delphi consensus process with international expert endoscopists. In the two endoscopy services studied, TP was achieved in 72.5% of colonoscopies, with significant variation between the 48 endoscopists performing the procedures.

Impact of delayed screening invitations on screen-detected and interval cancers in the Dutch colorectal cancer screening programme: individual-level data analysis

Esther Toes-Zoutendijk, Lucie de Jonge, Carola Adriana van Iersel, Manon C. W. Spaander, Anneke J. van Vuuren, Folkert van Kemenade, Christian R. Ramakers, Evelien Dekker, Iris D. Nagetaal, Monique E. van Leerdam, Iris Lansdorp-Vogelaar

Summary: The study assessed the impact of delayed invitation on screen-detected and interval colorectal cancers (CRC) in a FIT-based CRC screening program. Individual-level data from individuals who participated in 2017 and 2018 with a negative FIT and were eligible for screening in 2019 and 2020 were analyzed. The results showed that the first COVID-19 wave had a modest impact on screening yield, and a small proportion of FIT negatives had interval CRC, possibly due to an extended invitation interval.

Colorectal cancer risk after removal of polyps in fecal immunochemical test based screening

D. E. F. W. M. van Toledo, J. E. G. IJspeert, M. C. W. Spaander, I. D. Nagtegaal, M. E. van Leerdam, I. Lansdorp-Vogelaar, E. Dekker

Summary: By analyzing the results of colonoscopy, it was found that the simultaneous presence of high-risk serrated polyps and high-risk adenomas, or the presence of only high-risk serrated polyps, is associated with an increased risk of metachronous colorectal cancer.
