Endoscopic resection for undifferentiated early gastric cancer

Background and Objective: Endoscopic resection (ER) has become an important curative option for early gastric cancer (EGC). However, the application of ER for undifferentiated EGC remains controversial. The aim of this study was to evaluate the clinicopathologic outcomes of ER performed in undifferentiated EGC with special reference to histopathologic subtypes to examine the feasibility of ER in undifferentiated EGC. Design and Setting: Retrospective, single-center study. Patients: From January 2001 to April 2007, 58 lesions in 58 patients with undifferentiated EGC (17 poorly differentiated adenocarcinoma; 41 signet-ring cell carcinoma) were treated by ER at Severance Hospital, Seoul, Korea. Main Outcome Measurements: The therapeutic efficacy of ER was assessed according to en bloc resection, histologic complete resection (CR), lateral or vertical cut end-positive (including submucosal invasion), and recurrence rates in 3- to 65-month follow-up periods. Results: The en bloc resection and CR rates were 84.5% and 67.2%, respectively. The en bloc and CR rates in poorly differentiated were 82.4% and 58.8%, whereas those in signet-ring cell were 85.4% and 70.7%, respectively. There were no significant differences between poorly differentiated and signet-ring cell. However, all (100%) of the histologic incomplete resections in poorly differentiated were vertical cut end-positive, whereas 83.3% of these resections in signet-ring cell were lateral cut end-positive. The recurrence rate was 5.1% in CR during the follow-up period. Limitations: Retrospective, short-term follow-up period. Conclusions: ER may be a feasible local treatment for undifferentiated EGC if CR can be achieved. However, a different approach is necessary between poorly differentiated and signet-ring cell before ER to prevent incomplete resection.

Summary: This study investigated the mediating effects of illness perception on psychological distress in patients with newly diagnosed gastric cancer. It also identified the factors influencing illness perception in these patients. The results showed that social support, physical symptoms, and satisfaction with patient education significantly influenced illness perception, and illness perception fully mediated the relationship between these variables and psychological distress.

Pseudomonas stutzeri PM101005 inhaled with atmospheric particulate matter induces lung damage through inflammatory responses

Summary: In this study, it was found that PM-isolated Pseudomonas stutzeri (PMPS) induces higher levels of inflammatory responses in lung epithelial cells, alveolar cells, and macrophages compared to Pseudomonas stutzeri (PS). The pathogenicity of PMPS is mediated by Toll-like receptors (TLRs) signaling pathways, which also regulate the expression of the antibiotic peptide beta-defensin 3 (BD3) that suppresses PMPS proliferation. Furthermore, pretreatment with PM enhances inflammatory responses and tissue damage caused by PMPS, while reducing BD3 expression.

Summary: This study aimed to evaluate the prevalence of gastrointestinal symptoms and post-COVID-19 disorders of gut-brain interaction among hospitalized patients with SARS-CoV-2 infection. The results showed that patients with COVID-19 had higher rates of gastrointestinal symptoms on hospital admission and lower rates of constipation and hard stools at 12 months post-infection compared to the control group. Additionally, the rate of irritable bowel syndrome was significantly higher in patients with COVID-19 than in controls.

Changes in failure to rescue after gastrectomy at a large-volume center with a 16-year experience in Korea

Summary: This study conducted a comprehensive investigation on the failure to rescue (FTR) after radical gastrectomy for gastric cancer patients and explored the associations between FTR and clinicopathologic factors, operative features, and complication types. The study found that older age, high ASA score, and serosa exposure were associated with FTR. Furthermore, patients who underwent gastrectomy in recent years had lower mortality rates when complications occurred. This study is important as it reveals the risk factors for FTR and provides reference for clinical treatment.

Furthermore, there are significant issues with the conventional first-line treatment in countries with high clarithromycin resistance.

Role of prognostic nutritional index in postoperative radiotherapy for non-small cell lung cancer

Summary: This study aimed to investigate the relationship between the prognostic nutritional index (PNI) and survival and recurrence in NSCLC patients who underwent postoperative radiotherapy (PORT). 97 NSCLC patients were included in the study, and higher post-RT PNI was significantly associated with better overall survival. This finding suggests that PNI can be used as a prognostic marker in this patient population.

Validation of the simplified magnetic resonance index of activity by using DWI without gadolinium enhancement to evaluate bowel inflammation in Crohn's disease

Summary: This study aimed to validate the modified simplified magnetic resonance index of activity (sMARIA) score using DWI on non-contrast magnetic resonance enterography (MRE) to evaluate active inflammation in patients with Crohn's disease (CD), compared to the original sMARIA scoring system. The study found that the modified sMARIA had higher accuracy in detecting active inflammation and showed comparable performance to sMARIA using contrast-enhanced MRE.

Interplay between chronic inflammation and clonal haematopoiesis of indeterminate potential in Behcet's disease

Summary: This study investigated the emergence rate of clonal haematopoiesis of indeterminate potential (CHIP) and its association with inflammatory markers in patients with Behcet's disease (BD). The results showed that the detection rate of CHIP in BD patients was 11.1%, which was not significantly different from the control group. Among the BD patients, DNMT3A mutations were the most common. CHIP carriers with BD had higher levels of inflammatory markers and older age compared to non-CHIP carriers with BD.

Factors associated with the prescription of probiotics in patients with inflammatory bowel disease: a cross-sectional study

Summary: This study explored the use of probiotics and factors associated with their prescription in patients with inflammatory bowel disease (IBD). The results showed that increased disease activity may be associated with fewer prescriptions of probiotics. However, physicians prescribed probiotics to control symptoms in both UC and CD patients. The use of MTX and iron, as well as a history of IBD-related surgeries, were associated with more frequent probiotic prescriptions in CD patients.
