Summary: This study aimed to identify risk factors for lymph node metastasis (LNM) in mucosal undifferentiated early gastric cancer (EGC) and evaluate the feasibility of endoscopic submucosal dissection (ESD) for its treatment. The results showed that LNM was present in mucosal undifferentiated EGC patients who satisfied the expanded indication of ESD, and lymphovascular invasion (LVI) was a significant risk factor for LNM.

Summary: Although the incidence and mortality rate of gastric cancer seem to be decreasing, it remains a significant global health issue. Early gastric cancer can be effectively treated with endoscopic resection, which has high rates of success and fewer adverse effects compared to surgical resection. However, proper patient selection and surveillance are crucial for ensuring optimal outcomes and detecting potential metachronous lesions. This review summarizes the current evidence on the endoscopic treatment of early gastric cancer, including short- and long-term outcomes and post-resection management.

Summary: This study aimed to clarify the long-term clinical outcomes and risk factors of metachronous gastric neoplasm (MGN) 5 years after endoscopic submucosal dissection (ESD) for early gastric cancer (EGC). The study found that male sex, severe intestinal metaplasia, tumor-positive lateral margin, <5 mm lateral safety margin, and synchronous adenoma were positive predictive factors for MGN after 5 years post-ESD. In addition, successful eradication of Helicobacter pylori was a negative predictive factor for MGN.

Summary: Gastric cancer is a widespread disease globally, with high mortality and morbidity rates. Early diagnosis and treatment are crucial for improved prognosis and survivability. However, unlike colorectal cancer, the diagnostic and therapeutic pathways for gastric cancer are not widely shared, which negatively impacts the opportunity for curative endoscopic resection.

Summary: This study investigated the prevalence and risk factors of lymph node metastasis (LNM) in undifferentiated early gastric cancer (UD-EGC) cases meeting the expanded indication for endoscopic submucosal dissection (ESD). The results showed that the risk of LNM is higher than expected when using the current expanded indication for UD-EGC. Histological evaluation could provide useful clues for reducing the risk of LNM.

Summary: The study compared the long-term outcomes of ESD followed by additional surgery and initial surgery in UD EGC patients. The surgery group had significantly longer OS than the ESD + OP group, especially in PDA patients. However, the ESD-only group had a shorter RFS compared to the ESD + OP and surgery groups. The ESD + OP group showed comparable RFS to the surgery group.

Summary: With the increasing incidence of early gastric cancer (EGC) in the elderly population, it is important to determine the indications for endoscopic resection (ER) in late-elderly patients in terms of life expectancy. This study found that ER for EGC in late-elderly patients may improve life expectancy, especially in those with a good Eastern Cooperative Oncology Group performance status (ECOG-PS), high Prognostic Nutritional Index (PNI), and non-eCura C-2 risk.

Summary: In this study, it was found that PM-isolated Pseudomonas stutzeri (PMPS) induces higher levels of inflammatory responses in lung epithelial cells, alveolar cells, and macrophages compared to Pseudomonas stutzeri (PS). The pathogenicity of PMPS is mediated by Toll-like receptors (TLRs) signaling pathways, which also regulate the expression of the antibiotic peptide beta-defensin 3 (BD3) that suppresses PMPS proliferation. Furthermore, pretreatment with PM enhances inflammatory responses and tissue damage caused by PMPS, while reducing BD3 expression.

Summary: This study aimed to evaluate the prevalence of gastrointestinal symptoms and post-COVID-19 disorders of gut-brain interaction among hospitalized patients with SARS-CoV-2 infection. The results showed that patients with COVID-19 had higher rates of gastrointestinal symptoms on hospital admission and lower rates of constipation and hard stools at 12 months post-infection compared to the control group. Additionally, the rate of irritable bowel syndrome was significantly higher in patients with COVID-19 than in controls.

Summary: This study conducted a comprehensive investigation on the failure to rescue (FTR) after radical gastrectomy for gastric cancer patients and explored the associations between FTR and clinicopathologic factors, operative features, and complication types. The study found that older age, high ASA score, and serosa exposure were associated with FTR. Furthermore, patients who underwent gastrectomy in recent years had lower mortality rates when complications occurred. This study is important as it reveals the risk factors for FTR and provides reference for clinical treatment.

Summary: This study evaluates the real-world practice of H. pylori management in Southeast Asia and finds that multidrug-resistant strains are emerging in the region. It reveals that inappropriate regimens containing antibiotics with high resistance rates are not uncommon, especially among non-gastroenterologist physicians. Furthermore, there are significant issues with the conventional first-line treatment in countries with high clarithromycin resistance.

The results showed that the detection rate of CHIP in BD patients was 11.1%, which was not significantly different from the control group. Among the BD patients, DNMT3A mutations were the most common. CHIP carriers with BD had higher levels of inflammatory markers and older age compared to non-CHIP carriers with BD.

Summary: This study explored the use of probiotics and factors associated with their prescription in patients with inflammatory bowel disease (IBD). The results showed that increased disease activity may be associated with fewer prescriptions of probiotics. However, physicians prescribed probiotics to control symptoms in both UC and CD patients. The use of MTX and iron, as well as a history of IBD-related surgeries, were associated with more frequent probiotic prescriptions in CD patients.
