Summary: Biologic drugs target specific cytokine signaling pathways to treat autoimmune diseases. Despite their generally safe profiles, paradoxical reactions have been reported in some cases. This study presents a case of infliximab-induced psoriasis vulgaris in a patient with Crohn's disease. Discontinuing infliximab and introducing low-dose steroids and methotrexate successfully controlled both the psoriasis and bowel condition.

Summary: Paradoxical psoriasis induced by tumor necrosis factor alpha antagonists presents with significant pustular lesions and may have a protracted and complicated clinical course. Cases with moderate-to-high anti-nuclear antibody titers may be at risk for this condition and extensive pustular presentation could indicate a poor prognosis refractory to therapy.

Summary: This study revealed that the IL-36 pathway plays a significant role in the pathogenesis of generalized pustular psoriasis. Treatment with spesolimab, an anti-IL-36 receptor antibody, resulted in rapid improvement of skin and pustular symptoms in patients with GPP. The treatment also modulated dysregulated molecular pathways associated with GPP and PPP, leading to improved clinical outcomes.

Summary: Generalised pustular psoriasis (GPP) is a rare and severe form of psoriasis characterized by visible sterile primary pustules on non-acral skin. This article reviews the gastrointestinal (GI) and liver disorders associated with GPP. GPP is often accompanied by extracutaneous manifestations such as neutrophilic cholangitis and abnormal liver function tests. There is a possible genetic relationship between GPP and conditions like hepatocellular carcinoma and inflammatory bowel disease.

Summary: Palmoplantar pustular psoriasis (PPPP) and non-pustular palmoplantar psoriasis (NPPP) are localized, debilitating forms of psoriasis. This study found that NPPP shows more pronounced T helper 1-mediated inflammation, while PPPP exhibits stronger neutrophil-associated activity.

Summary: This study investigated the relationship between body composition parameters and disease severity in Crohn's disease (CD) patients treated with infliximab (IFX). The results showed significant differences in subcutaneous adiposity index (SAI), visceral adiposity index (VAI), and skeletal muscle mass index (SMI) among patients with different disease activity statuses. SMI was identified as a reliable indicator of disease activity status.

Summary: The study revealed that pediatric CD patients have gut microbial dysbiosis and altered metabolism after IFX therapy, which is related to treatment outcomes. The effects of IFX treatment may be mediated by enriching bacteria taxa that produce SCFAs and BSH, thereby inhibiting inflammation and restoring bile acid metabolism. Some fecal bacteria and metabolites may serve as predictors of IFX therapy outcomes for pediatric CD patients.

Summary: "Generalized pustular psoriasis (GPP)" is a type of pustular psoriasis characterized by recurring or persistent outbreaks of primary sterile pustules on non-acral areas. These eruptions can occur with or without systemic inflammation. Various factors, such as medications, stress, and viral infection, have been identified as potential triggers for GPP flares. While some cases have reported GPP-like eruptions in individuals with coronavirus disease 2019 (COVID-19) infection, few have examined the relationship between infection and biologic use in the development of GPP. In this case, we present the history and management of a 45-year-old male patient with a previous diagnosis of spondyloarthropathy treated with a tumor necrosis factor-& alpha; inhibitor who recently experienced a new, spreading pustular rash following COVID-19 infection.

Summary: This study investigated the adherence to Infliximab (IFX) among Chinese Crohn's disease patients and evaluated the association between medication belief and adherence. The results showed that medication concerns may be a predictive factor of adherence, and by enhancing knowledge and relieving concerns, patients' adherence to IFX can be increased.

Understanding the epidemiology and clinical data of GPP is challenging due to its rarity. This review article provides an overview of GPP prevalence estimates in key demographics, genetic mutations associated with the disease, incidence of comorbidities, mortality rate, and causes of death.

Summary: This study investigates the relationship between the shape of time-intensity curve on dynamic gadolinium-enhanced MRI and the type of ovarian tumor (benign, borderline, or malignant) classified as indeterminate based on ultrasonography, aiming to enable accurate pre-surgical diagnosis. Significant associations were found between the curve types and the lesion types, indicating that dynamic contrast-enhanced MRI can be useful for classifying indeterminate ovarian tumors as benign, borderline, or malignant.
