Gastric linitis plastica: which role for surgical resection?

The role of surgery for gastric linitis plastica (GLP) is questioned. This study aimed to analyze our experience in the surgical treatment of GLP with specific reference to the resectability rate, prognosis, and mode of recurrence. Results of surgery were analyzed in 102 patients with GLP. Of the 102 patients, 92 underwent surgical exploration, with resection performed in 60 cases. R2 resection was carried out in 20 patients and R1 in 12 patients, while the resection was considered potentially curative (R0) in 28 (27.5%). Overall, the median (95% confidence interval [CI]) survival time was 5.7 (3.7-7.5) months, with none of the patients alive at the end date of the study. For R0 patients the median (95% CI) survival time was 15.8 (11-20.7) months. The great majority of recurrences were intra-abdominal (peritoneal and/or locoregional), with a systemic component of the relapse that was rarely observed (5 cases). After primary surgery, GLP showed a poor prognosis without regard to the extent or type of resection. The failure of surgical treatment related mainly to the peritoneal spread of the disease. Specifically designed multimodality treatment protocols should be tested in this setting.

SARS-CoV-2 infection and venous thromboembolism after surgery: an international prospective cohort study

Summary: Surgical patients with peri-operative or recent SARS-CoV-2 infection are at increased risk of postoperative venous thromboembolism compared to those without a history of SARS-CoV-2. However, the optimal prophylaxis and treatment for venous thromboembolism in this patient population remains unknown. Venous thromboembolism is independently associated with 30-day mortality.

The study concluded that staging laparoscopy is a useful tool to guide the most appropriate treatment strategy for gastric cancer patients.

Consequences of the COVID-19 pandemic on the diagnosis and treatment of gastric cancer in referral centers in Italy

Summary: The coronavirus pandemic had a significant impact on the diagnosis and treatment of gastric cancer in nine Italian research centers. The number of advanced or metastatic cases increased after the spread of SARS-CoV-2 infection, particularly after the full lockdown periods. During the pandemic, shorter timeframes were required to complete the diagnostic work-up or refer patients to the best treatment option once they were referred to the centers.

In 2015, the EGCA launched a project to establish a list and definitions of post-operative complications specific for gastrectomy. This study investigated 386 cases of post-operative complications in Italy, with non-surgical infections and anastomotic leaks being the most frequent complications.

Summary: In a prospective observational cohort study, the frailty status of patients at the emergency department admission was evaluated for predicting in-hospital death risk stratification for those needing urgent cholecystectomy. Frailty and open surgery were found to be independent predictors of major complications. The evaluation of functional status in the emergency department could predict the risk of major complications and the need for open surgical approach in elderly patients undergoing urgent cholecystectomy.

Soft pancreatic texture and a Wirsung diameter <3 mm are identified as prognostic factors for PPAP onset, while a pancreatic duct <= 3 mm significantly influences a more severe course of PPAP.

Summary: This study examines the potential impact of post-pancreatectomy acute pancreatitis (PPAP) on long-term outcomes after pancreaticoduodenectomy (PD). The occurrence of PPAP is associated with a worse post-operative clinical course, but there is no significant difference in overall survival between patients with and without PPAP. However, patients with PPAP may have worse disease-free survival compared to those without PPAP, although the difference is not statistically significant.

The study found that different metastatic sites did not affect survival rates, but multiple metastatic sites were associated with worse survival. Patients who could undergo curative resection had better survival rates. A more accurate diagnostic workup and staging had a favorable impact on survival.
