Summary: This study provides the first integrative genomic and transcriptomic profiles of primary gastric linitis plastica (GLP), a distinct subtype of gastric cancer. Dysfunction in the Hippo pathway and mutations in genes such as MUC6 and CDH1 are key characteristics of GLP. Additionally, GLP exhibits high immunodeficiency and low AMPK pathway activity. Up-regulation of PI3K-AKT pathway-related genes, IGF2BP3 and MUC16, is associated with PI3K-AKT pathway activity in GLP.

Summary: This study found abnormal changes in vasculature in patients with Linitis plastica (LP) using probe-based Confocal Laser Endomicroscopy (pCLE). The abnormal features included vessel enlargement, tortuosity, and leakage associated with the affected submucosal layer. This provides a new endoscopic approach and strategy for diagnosing LP.

Summary: This study utilized a national database to investigate the clinical characteristics, treatment modalities, and outcomes of gastric linitis plastica (LP). The findings showed that surgical resection and chemotherapy were associated with improved survival, while advanced-stage disease and lack of surgical resection and chemotherapy were associated with increased risk of death.

Summary: This study suggests that an observed tumor size greater than 8 cm may serve as a critical value for distinguishing the prognosis of Borrmann III and IV gastric cancer. It is proposed as a quantitative diagnostic criterion for gastric linitis plastica regardless of Borrmann type.

Summary: EUS-FNA biopsy showed a diagnostic ability of 76.9% in patients with negative endoscopic biopsy findings for GLP, contributing to diagnosis in 18.5% of cases without adverse events. The absence of superficial ulcerations was identified as a predictor of false-negative endoscopic biopsy findings in patients with GLP.

Summary: Gastric linitis plastica (GLP) is a type of gastric adenocarcinoma with a poor prognosis. The infiltrating nature of this tumor leads to low diagnostic yield from superficial mucosal biopsies. This study investigated the role of endoscopic ultrasound-fine needle biopsy (EUS-FNB) in diagnosing GLP. The results showed that EUS-FNB had a sensitivity of 90% in diagnosing GLP in patients with suspected disease and normal mucosal biopsies, and it was also helpful in diagnosing other serious conditions.

Summary: This retrospective study examined the role of adjuvant chemotherapy (ACT) in patients with pT2N0M0 gastric cancer who underwent radical resection. The study found that ACT was associated with higher 5-year overall survival and disease-specific survival rates. Patients with pT2N0M0 gastric cancer, regardless of high-risk factors, may be suitable for ACT.

Summary: We present a case of rectal linitis plastica secondary to prostate adenocarcinoma, and describe the MRI, Ga-68-PSMA-11 PET/CT, and PET/MRI findings. The rectal linitis plastica was the initial manifestation of the prostate adenocarcinoma, with the rectum being the sole site of metastasis. Imaging showed circumferential thickening of the rectal wall on MRI, and intense Ga-68-PSMA-11 uptake on PET/CT and PET/MRI. Familiarity with these imaging features can aid in the recognition of this rare condition.

Summary: By establishing 5FU+CDDP resistant organoid lines from gastric cancer patients, this study reveals that JAK/STAT signaling and its downstream target ADAR1 are upregulated, and ADAR1 contributes to chemoresistance and self-renewal through RNA editing. The study also finds hyper-edited lipid metabolism genes in the resistant lines, and ADAR1-mediated editing on SCD1 promotes lipid droplet formation, alleviating chemotherapy-induced ER stress and enhancing self-renewal. Inhibition of SCD1 reverses chemoresistance and reduces tumor-initiating cell frequency, and higher levels of ADAR1 and SCD1 or a high SCD1 editing/ADAR1 mRNA signature predict worse prognosis.

Summary: Surgical patients with peri-operative or recent SARS-CoV-2 infection are at increased risk of postoperative venous thromboembolism compared to those without a history of SARS-CoV-2. However, the optimal prophylaxis and treatment for venous thromboembolism in this patient population remains unknown. Venous thromboembolism is independently associated with 30-day mortality.

Summary: This study aimed to investigate the benefits of staging laparoscopy for gastric cancer patients. The results showed that tumor type, tumor location, and staging could provide additional diagnostic information for patients undergoing laparoscopic surgery. The study concluded that staging laparoscopy is a useful tool to guide the most appropriate treatment strategy for gastric cancer patients.

Summary: The coronavirus pandemic had a significant impact on the diagnosis and treatment of gastric cancer in nine Italian research centers. The number of advanced or metastatic cases increased after the spread of SARS-CoV-2 infection, particularly after the full lockdown periods. During the pandemic, shorter timeframes were required to complete the diagnostic work-up or refer patients to the best treatment option once they were referred to the centers.

Summary: Gastrectomy for gastric cancer in Western countries still has high rates of morbidity and mortality. In 2015, the EGCA launched a project to establish a list and definitions of post-operative complications specific for gastrectomy. This study investigated 386 cases of post-operative complications in Italy, with non-surgical infections and anastomotic leaks being the most frequent complications.

Summary: In a prospective observational cohort study, the frailty status of patients at the emergency department admission was evaluated for predicting in-hospital death risk stratification for those needing urgent cholecystectomy. Frailty and open surgery were found to be independent predictors of major complications. The evaluation of functional status in the emergency department could predict the risk of major complications and the need for open surgical approach in elderly patients undergoing urgent cholecystectomy.

Summary: This study evaluates the clinical significance of the novel ISGPS definition of PPAP. The results indicate that PPAP occurrence increases the risk of more severe complications, delayed gastric emptying, and post-operative pancreatic fistula. Soft pancreatic texture and a Wirsung diameter <3 mm are identified as prognostic factors for PPAP onset, while a pancreatic duct <= 3 mm significantly influences a more severe course of PPAP.

Summary: This study examines the potential impact of post-pancreatectomy acute pancreatitis (PPAP) on long-term outcomes after pancreaticoduodenectomy (PD). The occurrence of PPAP is associated with a worse post-operative clinical course, but there is no significant difference in overall survival between patients with and without PPAP. However, patients with PPAP may have worse disease-free survival compared to those without PPAP, although the difference is not statistically significant.

Summary: Despite its decreasing incidence, gastric cancer remains an important global healthcare problem. Neoadjuvant chemotherapy has been recommended for gastric cancer in Europe and has potential benefits in downstaging the tumor and improving survival. The collection of at least twenty-five lymph nodes after neoadjuvant therapy can reduce the likelihood of recurrence.

Summary: The Italian Research Group for Gastric Cancer developed a prospective database to evaluate the impact of a pragmatic attitude on the management of stage IV gastric cancer patients. The study found that different metastatic sites did not affect survival rates, but multiple metastatic sites were associated with worse survival. Patients who could undergo curative resection had better survival rates. A more accurate diagnostic workup and staging had a favorable impact on survival.
