Endovascular Repair for Acute Type B Aortic Dissection With Unfavorable Proximal Landing Zone

Summary: Despite advancements in the prevention, diagnosis, and treatment of acute aortic dissection, it remains a complex cardiovascular event with high mortality and morbidity rates. Recent research has improved our understanding of the disease's pathophysiology and led to reevaluations of existing classifications. There is a shift towards a unified naming system based on morphology and function. The location and extent of the initial tear, as well as the affected aortic segments, determine the appropriate management approach - emergency surgery, endovascular intervention, or medical treatment. Ongoing scientific evidence continues to shape the management and follow-up of acute aortic dissection, the most severe form of acute aortic syndrome.

Short-term follow-up of proximal aorta remodeling after zone 2 thoracic endovascular aortic repair for acute type B aortic dissection

The study found that compared with common stent-graft implantation, Castor single-branched stent-graft (CSS) implantation provided a safer and more effective treatment for promoting the remodeling of the proximal aorta.

Transcarotid Approach for Ascending Aortic Endovascular Repair

Dynamic Morphology of the Ascending Aorta and Its Implications for Proximal Landing in Thoracic Endovascular Aortic Repair

Summary: This study assessed the dynamic segmental anatomy of the entire ascending aorta to determine a suitable landing zone and aortic sizing for thoracic endovascular aortic repair. Using electrocardiogram-gated computed tomography angiography, the dimensions of the aorta in systolic and diastolic phases were measured. The results showed that the ascending aorta had larger dimensions during systole, and the mid-ascending and distal-ascending segments had favorable forms for TEVAR.

Postoperative acute kidney injury after thoracic endovascular aortic repair for acute type B aortic dissection

Summary: This study aimed to characterize acute kidney injury (AKI) after thoracic endovascular aortic repair (TEVAR) in patients with acute type B aortic dissection (ATBAD). The incidence of postoperative AKI was found to be 16.2% in patients undergoing TEVAR for ATBAD. Patients with postoperative AKI had a higher rate of in-hospital morbidities and mortality than those without AKI. A history of chronic kidney disease (CKD) and preoperative AKI were independently associated with postoperative AKI.

The fate of aortic root and aortic regurgitation after supracoronary ascending aortic replacement for acute type A aortic dissection

Summary: The study aimed to analyze the outcomes of thoracic endovascular aortic repair for complicated and uncomplicated acute type B aortic dissections. The data from WL Gore's Global Registry for Endovascular Aortic Treatment showed that the 30-day mortality and perioperative complications were equally low for both complicated and uncomplicated type B aortic dissections, and the midterm outcome was positive.

Summary: Middle-aged men experience decreased sexual quality of life after coronary artery bypass grafting. Preoperative sexual quality of life, general health, and higher-quality counseling positively affect postoperative sexual quality of life, whereas sternal pain, fear, and erectile dysfunction play a negative role. Pre- and postoperative care guidelines should be improved. Further prospective large cohort studies for males and females are required.

Infrared Thermographic Imaging of Chest Wall Perfusion in Patients Undergoing Coronary Artery Bypass Grafting

In this study, infrared thermography was used to observe 42 patients, and it was found that the surface temperature of the chest wall significantly dropped after harvesting the internal mammary artery, but returned to normal after 24 hours.

Combined Minimally Invasive Mitral Valve Surgery and Percutaneous Coronary Intervention: A Hybrid Concept for Patients with Mitral Valve and Coronary Pathologies

Summary: The feasibility of hybrid percutaneous coronary intervention (PCI) and minimally invasive mitral valve surgery (MIMVS) was evaluated in patients with concomitant coronary and mitral disease. Ten patients, with a median age of 75 years, underwent MIMVS and PCI with a 100% success rate. There were no postoperative myocardial infarctions or strokes, but complications such as delirium and bleeding occurred. The 30-day survival rate was 100%. The study suggests that a hybrid approach can be feasible in patients with both mitral valve and single vessel coronary disease.
