Olfactory function and neuropsychological profile to differentiate dementia with Lewy bodies from Alzheimer's disease in patients with mild cognitive impairment: A 5-year follow-up study

Background: Mild cognitive impairment (MCI) is a well-known precursor of Alzheimer's disease (AD) but often also precedes dementia with Lewy bodies (DLB). The early differentiation of DLB from AD is important to delay disease progression. Olfactory dysfunction is a well-known early sign of both AD and Lewy body disorders, including Parkinson's disease (PD) and DLB. Thus, the aim of the present study was to determine whether olfactory and neuropsychological tests can aid in the differentiation of DLB from AD at the MCI stage. Methods: The present study included 122 MCI patients who were monitored until they developed dementia or until their condition stabilized; the follow-up period averaged 4.9 years (range: 3.9-62 years). Baseline olfactory function as measured with the Cross-Cultural Smell Identification (CCSI) test and neuropsychological data were compared. Results: During the follow-up period, 32 subjects developed probable AD (MCI-AD), 18 had probable DLB (MCI-DLB), 45 did not convert to dementia (MCI-stable), and eight developed a non-AD/DLB dementia. The mean CCSI score (95% confidence interval [CI]) in patients with MCI-DLB (4.6; 95% CI: 4.0-5.3) was significantly lower than that of MCI-AD patients (6.4:95% CI: 6.0-6.7, p < 0.001) and MCI-stable patients (73; 95% Cl: 6.9-7.8, p < 0.001). The area under the curve of the receiver operating characteristic to discriminate MCI-DLB from MCI-AD using CCSI scores was (0.84; 95% CI: 0.72-0.97). Frontal-executive function and visuospatial ability was worse in patients with MCI-DLB, while verbal recognition memory impairment was greater in those with MCI-AD. Conclusion: Olfactory and neuropsychological tests can help predict conversion to DLB or AD in patients with MCI. (C) 2015 Elsevier B.V. All rights reserved.

Association of plasma α-synuclein with cardiac 123I-MIBG scintigraphy in early Parkinson's disease

Summary: In early Parkinson's disease patients, plasma alpha-synuclein levels were found to correlate with cardiac sympathetic denervation but not with nigrostriatal degeneration. This suggests that plasma alpha-synuclein levels may more readily reflect the peripheral deposition of Lewy bodies rather than their central deposition.

Factors associated with motor severity in vascular parkinsonism with normal dopamine transporter imaging

Summary: In a study investigating determinants of motor severity in vascular parkinsonism (VaP), it was found that regional white matter intensity (WMH) burden and comorbidities such as periventricular hyperintensities (PVH) and diabetes were significantly associated with motor severity in patients with normal dopamine transporter (DAT) imaging. These results suggest that diabetes and white matter hyperintensities may play a significant role in the development of motor symptoms in VaP.

Summary: This study compared the specific patterns of cortical hypoperfusion, dopamine transporter uptake, and cardiac MIBG uptake between glucocerebrosidase gene mutation Parkinson's disease (GBA-PD) and sporadic Parkinson's disease (sPD). The findings showed decreased regional perfusion in the bilateral posterior parietal and occipital cortex in GBA-PD patients and a higher prevalence of cardiac sympathetic denervation and non-motor symptoms in GBA-PD compared to sPD.

Nelonemdaz for Patients With Acute Ischemic Stroke Undergoing Endovascular Reperfusion Therapy: A Randomized Phase II Trial

Summary: This study evaluated the safety and efficacy of nelonemdaz in patients with acute ischemic stroke. While there was no significant difference in the proportion of patients achieving modified Rankin Scale scores of 0-2 at 12 weeks among the groups, patients treated with nelonemdaz showed a favorable tendency towards achieving these scores without serious adverse effects. A large-scale phase III trial is warranted for further validation.

Deep brain stimulation in Parkinson disease with valosin-containing protein gene mutation

Summary: This case report presents a 53-year-old patient with Parkinson's disease and a VCP mutation who developed motor complications. The patient underwent subthalamic nucleus deep brain stimulation at 56 years old, but myopathy emerged 1.5 years after the surgery. The variability of VCP phenotype should be taken into consideration when evaluating the long-term outcomes of deep brain stimulation.

Summary: This study found that plasma neurofilament light chain (NfL) and cardiac metaiodobenzylguanidine (MIBG) uptake may be useful biomarkers in predicting phenoconversion of isolated rapid-eye-movement sleep behavior disorder (iRBD) patients. Elevated plasma NfL levels may suggest imminent phenoconversion to multiple system atrophy (MSA), whereas low cardiac MIBG uptake suggests phenoconversion to Lewy body disease (LBD).

The effect of levodopa treatment on vascular endothelial function in Parkinson's disease

Summary: In this study, a new classification system for intracranial large vessel occlusion (LVO) patients undergoing endovascular treatment (EVT) was proposed. The system differentiated between extracranial and intracranial atherosclerosis, classified cryptogenic embolism, and identified other determined etiologies. The ICAS group had a longer median onset-to-puncture time and a lower rate of complete recanalization after EVT.
