Survey on the Current Trends in Managing Intrauterine Growth Restriction

Objective: To provide a snapshot of the current trends in managing intrauterine growth restriction (IUGR) and to assess the agreement on the gestational age and the way of delivery in different clinical scenarios. Methods: A PubMed search was performed to identify all original articles on IUGR in the last 6 years. The most active 20 authors were selected as experts and were invited to respond to a survey on their preferred gestational age for elective delivery in several IUGR cases depending on Doppler measurements (including umbilical artery (UA), middle cerebral artery, cerebroplacental ratio, uterine artery and ductus venosus), biophysical profile and cardiotocography. Results: 15 of the 20 selected experts agreed to participate in the survey, of which 3 failed to meet the deadline to complete the survey. Management of IUGR was relatively uniform for abnormal UA, uterine artery or cerebroplacental ratio. Although average gestational age at delivery reflected a clear progression with accepted markers of severity, discrepancies of up to 4 weeks were found for abnormal middle cerebral artery Doppler and absent end-diastolic velocity in the UA, and of up to 8 weeks for reverse end-diastolic velocity in the UA and abnormalities in the ductus venosus Doppler. Conclusions: Management of IUGR is still far from being uniform among centers, with most controversy surrounding the management of early-onset IUGR. Intra- and interobserver reproducibility of second trimester ultrasound cervical length measurement in a general population

Summary: This study evaluated the reproducibility of ultrasound cervical length (CL) measurement in the second trimester and found moderate agreement among intra- and inter-observers. However, wide disagreements were observed when using specific cutoffs to classify patients, highlighting the importance of training and quality assessments when considering the universal application of CL measurement for screening.

Summary: The study successfully constructed nomograms for fetal cardiac, ventricular, and atrial relative size and geometry parameters from 18 to 41 weeks' gestation using a low-risk population of singleton pregnancies. The results showed regular patterns of changes in these parameters throughout pregnancy.

Summary: The study aimed to evaluate corpus callosum size in fetuses with isolated major congenital heart defects using neurosonography. Findings revealed that fetuses with major CHD had a smaller corpus callosum compared to controls, particularly in the subgroup with expected poorer brain oxygen supply. This suggests sonographic corpus callosum size could serve as a clinically feasible marker of abnormal white matter development in congenital heart defects.

Summary: This study is the first randomized trial evaluating the impact of maternal lifestyle interventions during pregnancy on perinatal outcomes. The interventions focus on promoting Mediterranean diet and stress reduction using mindfulness techniques, aiming to reduce the prevalence of FGR and improve neurodevelopment and cardiovascular profile in children. Compliance to the interventions will be evaluated with biomarkers to ensure their effectiveness.

Summary: This analysis suggests that fetoscopic endoluminal tracheal occlusion improves survival in cases of moderate and severe lung hypoplasia. The timing of balloon insertion may impact the treatment effect, but further research is needed to determine its precise influence due to the limited sample size and confounding factors. In particular, early insertion of the balloon greatly increases the risk of preterm delivery.

Summary: COVID-19 has a lesser impact on children compared to adults, but they can still get infected and transmit the virus. A study evaluated the use of high-throughput Luminex assays to measure SARS-CoV-2 antibodies in saliva and compared them to serum/plasma samples in infected children and adults. The results showed a correlation between saliva and serum/plasma antibody levels, and infected children had higher antibody levels, suggesting stronger clinical immunity. Therefore, measuring saliva antibodies could be a useful tool to determine COVID-19 prevalence and transmission in pediatric populations.
