Preconception folic acid treatment affects the microenvironment of the maturing oocyte in humans

Objective: To investigate the influence of folic acid supplementation on the follicular fluid concentrations of folate and total homocysteine and their relationship to the diameter of the follicle. Design: Observational study. Setting: Tertiary referral fertility clinic at the Erasmus MC, University Medical Center, Rotterdam, The Netherlands. Patient(s): Thirty-seven women undergoing IVF or intracytoplasmic sperm injection treatment. Intervention(s): No interventions other than routine stimulation treatment and the recommendation of folic acid supplementation. Main Outcome Measure(s): Concentrations of folate and total homocysteine in monofollicular and pooled follicular fluid and the diameter of the follicle. Result(s): Folic acid supplementation significantly increased folate and decreased total homocysteine concentrations in pooled follicular fluid. In monofollicular fluid, folate concentrations only were significantly increased in supplemented women. The total homocysteine concentration appeared to be significantly correlated with the diameter of the follicle (r = 0.27). Samples from single follicles were less prone to artifacts in the measurements of the folate and total homocysteine concentration. Conclusion(s): Preconception folic acid supplementation significantly alters both folate and total homocysteine concentrations in follicular fluid. The correlation between the diameter of the follicle and total homocysteine concentration in follicular fluid warrants further investigation.

The results show that the program can improve vegetable intake-related attitudes and practices in women undergoing ART treatment. However, there is no significant effect on fruit intake and smoking behavior.

Summary: Periconceptional maternal obesity is associated with adverse maternal and neonatal outcomes. Identifying biomarkers of pathways affected by maternal obesity can provide insights into pathophysiological mechanisms and identify individuals at risk of adverse clinical outcomes. This systematic review identified biomarkers in the endocrine, inflammatory, and one-carbon metabolic pathways influenced by maternal obesity.

Circulating maternal prorenin and oocyte and preimplantation embryo development: the Rotterdam Periconception Cohort

Summary: This study suggests that there is a difference in embryonic morphological development between ongoing pregnancies and live pregnancies ending in a miscarriage. Miscarriages tend to have smaller embryos and slower heart rates. This finding is important in terms of estimating the likelihood of a pregnancy continuing and providing support for those at risk of recurrent pregnancy loss.

Computational methods for the analysis of early-pregnancy brain ultrasonography: a systematic review

Summary: Based on the analysis of 2575 studies, this systematic review found that automatic and learning-based methods show great potential in early-pregnancy brain ultrasound analysis. To bring these methods into clinical practice, it is recommended that studies use routine clinical data depicting both normal and abnormal development, make their dataset and program source code publicly available, and be attentive to the influence of confounding factors.

First-trimester maternal renin-angiotensin-aldosterone system activation and the association with maternal telomere length after natural and IVF/ICSI conceived pregnancies: the Rotterdam periconception cohort

Summary: This study aimed to investigate the associations between maternal determinants of renin-angiotensin-aldosterone system (RAAS) activation in the first trimester and telomere length (TL) in pregnancies conceived naturally and after IVF/ICSI. Renin, prorenin, and aldosterone concentrations were measured in maternal blood at 9 weeks gestational age (GA) in 145 pregnancies of the Rotterdam Periconception cohort. TL was measured using qPCR at 20 weeks GA. The results showed a significantly negative correlation between renin and TL, which was attenuated for prorenin and not observed for aldosterone. Maternal TL was significantly shorter in pregnancies conceived after IVF/ICSI compared to natural pregnancies. Conclusion: The negative association between first-trimester maternal renin and maternal TL, as well as the shorter maternal TL in women after IVF/ICSI treatment compared to natural pregnancies, substantiates the role of excessive RAAS activation.

Summary: This study found significant associations between DNA methylation and birth season at birth and in childhood. The differential methylation regions (DMRs) identified were related to genes involved in tumorigenesis, psychiatric/neurological disorders, inflammation, and immunity. Furthermore, the associations between birth season and DNA methylation were influenced by birth latitude, particularly in higher latitude regions.

Summary: This study evaluated the current practice of preconception care in the Netherlands and the perceptions of birth care professionals regarding preconception care. The findings suggest that although birth care professionals acknowledge the importance of preconception care, there are variations in the implementation and identification of risk factors across different organizations. Furthermore, there is a low demand for preconception care among prospective parents.
