Searching for E-cadherin gene mutations in early onset diffuse gastric cancer and hereditary diffuse gastric cancer in Korean patients

The impact of CDH1 gene mutations and large deletions on hereditary diffuse gastric cancer (HDGC) and early onset diffuse gastric cancer (EODGC) has not been determined in Asians. We investigated the mutation status of the CDH1 gene in 25 Korean EODGC patients younger than 50 years and 23 HDGC patients who met the clinical criteria for HDGC. Polymerase chain reaction-direct sequencing was performed, and multiplex ligation-dependent probe amplification (MLPA) was used to evaluate the patients with negative sequencing results. We determined that 2 of 25 (8 %) EODGC patients had CDH1 germline mutations. One was a nonsense mutation (c.1003C > T, p.Arg335*, exon 7) in a 41-year-old female with no family history of cancer. The other was a missense mutation (c.715G > A, p.Gly239Arg, exon 6) in a 28-year-old male with no family history of cancer. One of 23 (4.3 %) HDGC patients had a CDH1 germline mutation (c.1003C > T). The patient's brother and sister died of stomach cancer. The MLPA results revealed no deletion or duplication in any patient. More research is needed to determine additional genetic targets that trigger HDGC. More comprehensive methods such as next-generation sequencing might be a good approach that can be used to identify the genetic causes of pathogenetically unexplained disorders.

Usefulness of EndoFLIP in Diverticular Peroral Endoscopic Myotomy for Symptomatic Epiphrenic Diverticulum

Summary: This study aimed to report our experience with diverticular peroral endoscopic myotomy (D-POEM) and investigate the usefulness of endoscopic functional luminal imaging probe (EndoFLIP) in determining the need for cardiomyotomy with septotomy for symptomatic epiphrenic diverticulum. The results showed that EndoFLIP may help decide whether to perform combined cardiomyotomy and septotomy for the treatment of an epiphrenic diverticulum. Further large-scale studies are needed to confirm these results.

Efficacy of tegoprazan-based bismuth quadruple therapy compared with bismuth quadruple therapy for Helicobacter pylori infection: A randomized, double-blind, active-controlled study

Summary: This study compared the efficacy and safety profiles of tegoprazan-based quadruple therapy and lansoprazole-based quadruple therapy for Helicobacter pylori eradication. The results showed that tegoprazan-based quadruple therapy had higher eradication rates than lansoprazole-based quadruple therapy in intention-to-treat, modified intention-to-treat, and per-protocol analysis. There was no significant difference in the rate of adverse events between the two groups. Therefore, tegoprazan might be a substitute for proton pump inhibitors in H. pylori eradication regimens.

Comparison between single and serial computed tomography images in classification of acute appendicitis, acute right-sided diverticulitis, and normal appendix using EfficientNet

Summary: This study aimed to develop a CNN using the EfficientNet algorithm for automated classification of acute appendicitis, acute diverticulitis, and normal appendix on CT images, and evaluate its diagnostic performance. The results showed that the RGB serial image method had slightly higher sensitivity, accuracy, and specificity for classifying normal appendix and acute diverticulitis compared to the single image method. Additionally, the mean areas under the ROC curve were significantly higher with the RGB serial image method for all three conditions. Therefore, the model accurately distinguished these conditions on CT images, particularly when using the RGB serial image method.

Development of a New Liquid Type Rapid Urease Test Kit (Helicotest®): Comparison with Other Commercial Kits

Summary: This study developed a liquid-type medium, Helicotest (R), to enable faster detection of Helicobacter pylori. Compared to other commercial kits, Helicotest (R) showed the fastest reaction time of 5 minutes for detecting H. pylori. Therefore, it is expected to facilitate faster diagnosis in clinical practice.

The optimal interval of surveillance gastroscopy after endoscopic resection for gastric neoplasia: a multicenter cohort study

Summary: This study aimed to determine the optimal interval for surveillance gastroscopy after endoscopic resection for gastric neoplasia and investigate the risk factors for metachronous gastric neoplasia. A retrospective review of medical records identified 677 patients who underwent endoscopic resection, and it was found that severe atrophic gastritis was an independent risk factor for metachronous gastric adenocarcinoma.
