Summary: This study retrospectively reviewed patients who underwent gastric electrical stimulation (GES) over a 10-year period and found that patients with gastroparesis had different clinical outcomes after GES therapy based on underlying etiology. Diabetic patients were more likely to continue therapy, while idiopathic patients were more likely to have the device removed.

Summary: This article reviews the use of bioelectric therapy of neuromodulation, via gastric electrical stimulation, for patients with drug refractory gastroparesis syndromes including surgical aspects of device placement and subsequent management. In addition to discussing newer concepts, the role of pyloric dysfunction in gastroparesis and the potential use of stimulation devices and pyloric therapies in concert are explored.

Summary: Gastric peroral endoscopic myotomy (G-POEM) is an effective 4-year treatment for refractory gastroparesis (RG), especially in diabetic gastroparesis (DG) patients, establishing a potential first-line therapy. Early diagnosis, nausea/vomiting, GCSI score at 6 months (1.5-2), and RP4H <10% at 6 months are predictive factors for long-term success.

Summary: Gastroparesis, a disorder characterized by delayed stomach emptying, has limited treatment options. Gastric electrical stimulation (GES) has shown promise in treating the signs and symptoms of gastroparesis by stimulating the stomach with high-frequency electrical impulses. In this case, a 43-year-old woman with refractory gastroparesis underwent laparoscopic implantation of a GES device. Further studies are needed to improve patient selection, technique, and long-term outcomes of GES. Patients with refractory gastroparesis who have not responded to traditional therapy should be considered for GES, with treatment decisions based on individual clinical presentation and patient preferences.

Summary: This meta-analysis compared the efficacy and complication rates of different modalities of pyloromyotomy and gastric electrical stimulation for the treatment of gastroparesis. The results suggest that double per-oral pyloromyotomy has better clinical success compared to the other interventions, while gastric electrical stimulation may lead to higher recurrence rates.

Summary: Gastric electrical stimulation and pyloric surgery have been shown to be successful treatments for gastroparesis, with durable improvement in nausea and vomiting symptoms. The choice of operation should be guided by patient characteristics and discussion of surgical risks and benefits. Combination GESPP does not appear to confer an advantage over GES or PP alone.

Summary: This study evaluated the effects of deep brain stimulation of the anterior nucleus of the thalamus (ANT-DBS) on systemic inflammatory responses in patients with drug-resistant epilepsy (DRE). The results showed a significant decrease in the IL-6/IL-10 ratio following ANT-DBS, indicating potential anti-inflammatory properties of the treatment. Further research is needed to explore the use of proinflammatory cytokine IL-6, anti-inflammatory cytokine IL-10, and their ratio as biomarkers for evaluating therapeutic response to DBS treatment.

Summary: YouTube is the most popular platform for surgical trainees to prepare cases. However, the most viewed laparoscopic cholecystectomy videos on YouTube were found to lack critical safety standards and educational value. Senior residents aligned with attending surgeons in video assessments, while junior trainees tended to overrate video quality and value.

Summary: According to the study on patients undergoing laparoscopic sleeve gastrectomy, it was found that the combination of bougie and esophagogastroduodenoscopy (EGD) can reduce the incidence of dehydration events, while the use of bougie alone may increase the risk of dehydration. Not using any sizing device showed similar outcomes to using various combinations of sizing devices. These findings emphasize the need for standardization of sizing devices in laparoscopic sleeve gastrectomy and suggest that foregoing sizing devices may be a management option without early adverse sequelae.
