Robotic Partial Nephrectomy Versus Laparoscopic Cryoablation for the Small Renal Mass

Background: Open partial nephrectomy (OPN) remains the gold standard for treatment of small renal masses (SRMs). Laparoscopic cryoablation (LCA) has provided encouraging outcomes. Robotic partial nephrectomy (RPN) represents a new promising option but is still under evaluation. Objective: Compare the outcomes of RPN and LCA in the treatment of patients with SRMs. Design, setting, and participants: We retrospectively analyzed the medical charts of patients with SRMs (<= 4 cm) who underwent minimally invasive nephron-sparing surgery (RPN or LCA) in our institution from January 1998 to December 2010. Intervention: RPN and LCA. Measurements: Perioperative complications and functional and oncologic outcomes were analyzed. Results and limitations: A total of 446 SRMs were identified in 436 patients (RPN, n = 210; LCA, n = 226). Patients undergoing RPN were younger (p < 0.0001), had a lower American Society of Anesthesiologists score (p < 0.001), and higher baseline preoperative estimated glomerular filtration rate (eGFR) (p < 0.0001). Mean tumor size was smaller in the LCA group (2.2 vs 2.4 cm; p = 0.004). RPN was associated with longer operative time (180 vs 165 min; p = 0.01), increased estimated blood loss (200 vs 75 ml; p < 0.0001), longer hospital stay (72 vs 48 h; p < 0.0001), and higher morbidity rate (20% vs 12%, p = 0.015). Mean follow-ups for RPN and LCA were 4.8 mo and 44.5 mo, respectively (p < 0.0001). Local recurrence rates for RPN and LCA were 0% and 11%, respectively (p < 0.0001). Mean eGFR decrease after RPN and LCA was insignificant at 1 mo, at 6 mo after surgery, and during last follow-up. Limitations include retrospective study design, length of follow-up, and selection bias. Conclusions: Both techniques remain viable treatment options in the management of SRMs. A higher incidence of perioperative complications was found in patients undergoing RPN. However, the technique was not predictive of the occurrence of postoperative complications. Early oncologic outcomes are promising for RPN, which also seems to be associated with better preservation of renal function. Long-term follow-up and well-designed prospective comparative studies are awaited to corroborate these findings. (C) 2012 European Association of Urology. Published by Elsevier B. V. Summary: This study aimed to identify trends, costs, and predictors in the use of different surgical procedures for post-radical prostatectomy incontinence (PPI). The results showed that male slings were the most common procedure, while the use of an artificial urinary sphincter (AUS) was the most expensive option. Patient's comorbidity history, radiation therapy, and neurological comorbidities were found to be predictors of incontinence surgery.

Summary: This study aimed to develop a model based on mp-MRI and target biopsy (TB) alone to predict the risk of lymph-node invasion (LNI). Using logistic regression analysis, a model was created and a nomogram was generated to predict the risk of LNI. The model showed good discrimination and was able to identify patients with LNI risk above 7%.

A systematic review of primary care-focused, self-reported medication adherence tools

Summary: This systematic review aimed to identify validated, self-reported medication adherence tools applicable to primary care and summarize selected features as an assessment of clinical feasibility and applicability. The study found considerable variability in the characteristics of self-reported adherence tools, and clinicians should consider their measurement validity and practicality when selecting a tool. There is a critical need for adherence tool developers to establish a key list of tool characteristics to facilitate practical comparisons among tools.

The Impact of Psychedelic Drugs on Anxiety and Depression in Advanced Cancer or other Life-threatening Disease A Systematic Review With Meta-analysis

Summary: This study systematically reviewed and conducted meta-analyses on the use of validated anxiety and depression scales in patients with cancer or other life-threatening diseases. The findings suggest that psychedelics may have potential in reducing anxiety and depressive symptoms, but there are concerns regarding increased blood pressure and heart rate. Further research is needed to determine the optimal psychedelic, dosage, number of sessions required, and the response of psychedelic-naive patients before considering widespread clinical use of this therapy.

Summary: Globally, clear-cell renal cell carcinoma (ccRCC) is the most prevalent type of kidney cancer. Surgery is key in treatment, but a significant number of patients have metastatic ccRCC or experience recurrence. Molecular-target-based agents like TKIs and ICIs are recommended for advanced cancer treatment. The tumor microenvironment (TME) plays an essential role in cancer development and can be targeted for therapy. Understanding immune cell interactions with cancer and other TME elements is crucial for enhancing immunotherapies and reducing resistance.

Summary: In this systematic review and meta-analysis, the cumulative bladder cancer (BC) recurrence rates and efficacy profile of different Bacillus of Calmette-Guerin (BCG) strains were compared. The study found that BCG Tokyo 172 had the lowest recurrence rate among studies with <= 3-year recurrence-free survival (RFS). No significant difference was observed among strains in >3-year RFS outcomes. The genetically different BCG strains RIVM, Tice, and Tokyo 172 showed some potential enhanced benefits.

Summary: This study compares the outcomes of robotic-assisted partial nephrectomy (RAPN) and minimally invasive radical nephrectomy (MIS-RN) for complex renal masses (CRM). The results indicate that RAPN is not associated with increased risk of complications or worsened oncological outcomes compared to MIS-RN, and may be preferred when clinically indicated.
