Summary: Lichen sclerosus (LS) is an inflammatory mucocutaneous condition primarily affecting postmenopausal women, with unknown etiology. LS is associated with factors such as hormonal status, trauma, and autoimmune diseases. LS pathogenesis involves genetic predisposition, immune-mediated mechanisms, tissue remodeling genes, and oxidative stress. LS presents as chronic whitish atrophic patches with itching and soreness in the genital and anal regions. Treatment options include topical corticosteroids and calcineurin inhibitors. LS is a common dermatological disease with an incompletely understood pathogenesis and limited treatment options.

Summary: This study describes the differences in the urinary microbiome of patients with pathologically confirmed lichen sclerosus (LS) urethral stricture disease (USD) compared to non-lichen sclerosus (non-LS) USD pre- and post-operatively. The findings show significant alterations in diversity and differential abundance of urine microbiota in LS USD patients. These findings could guide further research on the role of the urinary microbiome in LS USD pathogenesis, severity, and stricture recurrence.

The use of antiplatelet/anticoagulant therapy, diabetes, coronary heart disease, smoking (current/former), and stricture length were identified as potential contributing factors to complications.

Summary: Lichen sclerosus is a chronic inflammatory condition that affects the genital and perianal areas, characterized by irritation and dyspigmentation. It is most commonly seen in postmenopausal women and prepubertal children. The exact cause of lichen sclerosus is unknown, but it is believed to be a result of various factors such as genetics, environment, infection, and autoimmune factors. Long-term treatment is necessary for remission. We present a case of lichen sclerosus in an elderly female patient involving the vulvar, perineal, and perianal regions, which improved with topical clobetasol.

This study found that men with LS-USD may have a unique microbiologic richness compared to those with non-LS USD, with a distinct profile of bacteria enriched in the urine. Further research is needed to determine the clinical significance of these differences in the urinary microbiome.

Summary: This study evaluated the factors predicting recurrence after treatment and the best rescue option for patients failing buccal mucosa graft (BMG) urethroplasty. The findings showed that stricture length >= 5 cm was the only predictor of failure. Direct visual internal urethrotomy (DVIU) was a good option as a rescue treatment, but its success rate decreased with the number of treatments performed. On the other hand, open urethroplasty improved outcomes in most patients.

Summary: The preliminary results of this study suggest that restaging transurethral resection (ReTUR) may be safely avoided in T1 non-muscle-invasive bladder cancer (NMIBC) patients with complete resection at initial transurethral resection (TUR). Longer follow-up and larger sample size are needed to investigate the long-term oncological outcomes of this alternative approach.
