Severe and recurrent episodes of bronchiolitis obliterans organising pneumonia associated with indolent CD4+CD8+T-cell leukaemia

The present study describes an adult male who has had recurrent episodes of pulmonary infiltrates with severe acute respiratory failure over a period of 10 yrs. Clinical and pathological characteristics revealed bronchiolitis obliterans with organising pneumonia (BOOP) that responded dramatically to prednisone. BOOP is characterised by inflammation of the bronchioles and surrounding tissue in the lungs. It can mimic infectious pneumonia but diagnosis is suspected when there is no response to multiple antibiotic treatment, and blood and sputum cultures are negative for microorganisms. A high proportion of double-positive (DP)-T-cells was detected in peripheral blood and in bronchoalveolar lavage, expressing CD4 and CD8a beta heterodimer with memory phenotype. These DP-T-lymphocytes expressed specific homing molecules that could explain their tropism to lung tissue, giving rise to the clinical symptoms. The patient did not present organomegaly, lymphadenopathy, lymphocytosis or other features of malignancy. However, T-cell receptor V beta chain analysis indicated clonal rearrangement, and cytogenetic studies displayed chromosomic alterations that were similar to clonal proliferation observed in ataxia-telangiectasia and T-prolymphocytic leukaemia. The findings suggest a smouldering form of lymphoproliferation, the first sign of which was bronchiolitis obliterans organising pneumonia requiring constant corticoid treatment.

Summary: This study aimed to assess the feasibility of a validated assay of T-cell responses as a complement to antibody tests for evaluating the levels of protection generated by natural infection or SARS-CoV-2 vaccines. The results showed that the IGRA test detected T-cell responses in naturally exposed and vaccinated individuals, and there was a high correlation with specific antibody levels. These findings suggest that the IGRA test could be a promising tool for measuring the state of the immune response to SARS-CoV-2.

Summary: The study evaluated the long-term cellular and humoral immunogenicity induced by two and three doses of the mRNA-1273 SARS-CoV-2 vaccine in patients with hematologic malignancies and its relationship with the severity of breakthrough SARS-CoV-2 infection. The results showed a decline in seroconversion rate and anti-S titers after the second dose, especially in older patients or those with chronic lymphocytic leukemia. Cellular immunogenicity remained positive in most cases. A third vaccine dose improved the immune response in previously negative patients. COVID-19 severity was associated with impaired vaccine-induced immunogenicity.

Summary: Immune Checkpoint Receptors play a role in regulating immune responses and preventing tissue damage. Inhibiting the PD-1/PD-L1 receptor axis has shown promising results in cancer immunotherapy but can also lead to autoimmune adverse effects. This study investigates the role of PD-1/PD-L1 in the interaction between T cells and thyroid cells, and suggests that TFCs suppress T cell proliferation through the expansion of DN T cells, rather than PD-1/PD-L1. These findings contribute to our understanding of immune regulation and suggest alternative pathways for tumor evasion.

Summary: Chronic granulomatous disease (CGD) is an inborn error of immunity characterized by phagocytes' inability to produce reactive oxygen species, leading to dysregulated inflammatory responses and susceptibility to severe infections. The most common and severe form of CGD is X-linked CGD (XL-CGD), caused by mutations in the CYBB gene. This study illustrates the molecular challenges in the genetic diagnosis of XL-CGD through the analysis of three affected families, including heterozygous deletions of CYBB exon 13, hemizygous deletion of CYBB exon 7, and a novel intronic variant affecting splicing process.
