Summary: The study investigated whether early CT imaging in the ED altered the diagnosis or management of patients with acute pancreatitis. It was found that CT led to a change in diagnosis or management in some patients, but abdominal ultrasound may be a more sensitive screening study.

Summary: This study evaluated the association between advanced imaging in the emergency department (ED) and subsequent outpatient imaging and revisits among abdominal pain patients. The study found that patients who did not receive imaging in the ED were more likely to have outpatient imaging and revisit the ED within 30 days after discharge.

Summary: A study in Taiwan on patients with NSAP found that older age, male sex, and the use of NSAIDs and opioids during the index visit were associated with increased risk of return ED visits. On the other hand, CT scans were associated with decreased return visits, especially for older patients with underlying diseases who required pain control during the index visit.

Summary: This study investigated the factors influencing pain management and clinical outcomes in adult patients with appendicitis in the emergency department. The results showed that patients with higher pain levels were more likely to receive analgesia and opioids. Male patients were less likely to receive analgesia but more likely to receive opioids. Older age and Sunday presentations were associated with less opioid treatment. Patients who received analgesia had longer wait times for imaging, longer stays in the emergency department, and longer hospitalizations.

Summary: This clinical policy is a revised version of the 2010 Clinical Policy, focusing on the evaluation and management of emergency department patients with suspected appendicitis. A systematic review was conducted to provide evidence-based recommendations for the use of clinical prediction rules, diagnostic imaging modalities, and the addition of contrast in CT scans. Recommendations were made based on the strength of the available data.

Summary: This study compared the acquisition and findings of advanced diagnostic imaging (ADI) in emergency department (ED) patients with limited English proficiency (LEP) and English proficiency (EP). The results showed that after adjusting for demographic differences, the acquisition of ADI was similar between LEP and EP patients. Further research is needed to determine the optimal interpretation modalities to prevent unnecessary imaging in this setting.

Summary: Vermiform appendix duplication, also known as double appendix, is a rare anomaly with only about 100 reported cases since 1892. This study presents a case of a 10-year-old boy with two separate appendices, one arising from the cecal tinea coli, and one retrocecal. Awareness of this rare anomaly is crucial for clinicians in diagnosing patients with symptoms consistent with recurrent appendicitis after a previous appendectomy.

Summary: Replacing chest X-ray with ultra-low-dose chest CT (ULDCT) in the diagnostic work-up of patients suspected of non-traumatic pulmonary disease at the emergency department has similar effects on short-term functional health, hospital admissions, and length of stay. However, more incidental findings were found in the ULDCT group.

Summary: This article provides a systematic review of the available evidence on the diagnostic accuracy of biparametric (bp)-MRI and multiparametric (mp)-MRI for solid renal masses in differentiating malignant from benign, aggressive from indolent, and clear cell renal cell carcinoma (ccRCC) from other histology. The review reveals deficits in the available evidence, with limited studies and high risk of bias, as well as heterogeneity in definitions, imaging techniques, diagnostic criteria, and outcome measures for solid masses.

Summary: The EU In-Vitro Diagnostic Device Regulation (IVDR) aims to ensure transparent validation of diagnostic devices, traceability of results, and post-market surveillance. Laboratory-developed tests (LDTs) may be used in the absence of suitable commercial devices. The implementation of IVDR may lead to the need for urgent replacement of commercial tests with LDTs, and the workload will depend on various factors.

Summary: This study applied covariate-specific receiver operating characteristic (ROC) curve analysis to individual patient data on D-dimer testing for excluding pulmonary embolism. The results showed significant differences in D-dimer concentration and performance among age groups, while the distribution of results was similar for sex. Summary: This study aimed to explore the feasibility of lymphatic mapping in patients with locally advanced cervical cancer (LACC) and evaluate the impact of lymphatic mapping on radiotherapy for at-risk lymph nodes. The results showed that lymphatic mapping could identify potential metastases in at-risk lymph nodes, while the current radiotherapy regimen had suboptimal treatment for some nodes. Including at-risk lymph nodes in radiotherapy may improve treatment outcomes in LACC.

Summary: This study investigated the feasibility of magnetic resonance elastography (MRE) in measuring fibrotic disease involvement of the mesentery in small bowel Crohn's disease (CD). The results showed that MRE can detect increased stiffness and decreased fluidity in the mesentery of CD patients, which are correlated with the extent of mesenteric fibrosis. MRE may potentially guide clinical decision-making regarding extended mesenteric resection in CD.

Summary: For patients with colorectal cancer liver metastases, the identification of genetic mutations, such as KRAS mutations, is crucial. This study investigates the potential of radiomics in identifying KRAS mutations using CT scans. While the radiomics model shows good accuracy in the internal test set, it performs poorly in the external validation set. These results highlight the importance of external validation in assessing the clinical applicability of radiomics models.

Summary: Imaging tests are commonly used in healthcare, but concerns about their overuse have overshadowed their benefits in clinical decision-making. This study demonstrates the importance of considering patient-centered outcomes (PCOs) when evaluating the value of imaging tests. The PROD framework categorizes PCOs into four main domains and highlights the interplay between them and the factors that can modify them.
