A randomized, double-blind multicentre clinical trial comparing the efficacy of calcium dobesilate with placebo in the treatment of chronic venous disease

Objective. To assess the efficacy of calcium dobesilate on the quality-of-life (QoL) of patients with chronic venous disease (CVD). Design. Randomised, parallel, double blind, placebo-controlled clinical trial. Methods. Patients were recruited from vascular surgery clinics and randomised to 500 mg capsules of calcium dobesilate twice a day for 3 months or placebo. The primary outcome measure was 'QoL after 3 months' treatment measured by the specific Chronic Insufficiency Venous International Questionnaire (CIVIQ). Secondary outcomes were QoL at 12 months and assessment of the CVD signs and symptoms. The principal analysis was undertaken on the intention-to-treat (ITT) data. Results. Five hundred and nine patients were recruited (246 to calcium dobesilate and 263 to placebo). The analysis of the 'QoL after 3 months' showed no significant differences between groups (p = 0.07). For secondary outcomes, oedema and symptoms of CVD, there were no significant differences between groups. In a multi-factorial analysis, the 'QoL at 12 months' was better in the calcium dobesilate group than in placebo group (p = 0.02). Conclusions. Treatment with calcium dobesilate was not found to be superior to placebo on the QoL of CVD patients. The sustained effect of calcium dobesilate observed after treatment should be confirmed in future studies. (C) 2007 European Society for Vascular Surgery. Published by Elsevier Ltd. Summary: This study provides an overview of the effectiveness of nonpharmacological interventions in preventing adverse events in the intensive care unit. The findings suggest that some interventions, such as subglottic secretion drainage and semirecumbent position, are effective in preventing specific adverse events. However, the low methodological quality of the included reviews should be taken into consideration when interpreting the results. Rating: 7.5/10.

Quality of Life and Post-Surgical Complications in Patients on Chronic Antiplatelet Therapy with Proximal Femur Fracture: 12-Month Follow-Up after Implementing a Strategy to Shorten the Time to Surgery

Summary: This study evaluated a strategy to shorten the time from admission to surgery in patients with proximal femur fractures on chronic antiplatelet therapy and reported a 12-month follow-up on complications and quality of life. Patients were randomized to either early platelet function-guided surgery or delayed surgery. Surgical wound complications occurred in 5.7% of patients after hospital discharge, and 55.9% had medical complications, with 42.7% experiencing serious adverse events. Quality of life improved significantly after surgery, with the best scores at the six-month follow-up. There were no differences in outcomes between early and delayed surgery groups.

Summary: Hepatobiliary cancers are usually treated with systemic oncological treatments, but the trade-off between these treatments and supportive care is not clear and other outcomes beyond survival should be considered. This study aimed to assess the effects of systemic oncological treatments versus supportive care for advanced hepatobiliary cancers. The results showed that these treatments tend to improve survival but with greater toxicity, and other patient-important outcomes were not reported.

Delayed surgery and health related quality of life in patients with proximal femoral fracture

Summary: This study aimed to identify factors associated with delayed surgery in patients with proximal femoral fracture and evaluate their health-related quality of life (HRQoL) after surgery, including mortality within 6 months. It was a single-center, observational, prospective cohort study that included 163 patients. The main factor associated with delayed surgery was administrative authorization. The HRQoL of patients improved 6 months after surgery.

Summary: This study aimed to assess the impact of montelukast on pediatric patients with asthma/allergic rhinitis and found that montelukast was effective in controlling asthma symptoms when compared with placebo, but inhaled corticosteroids were superior in controlling symptoms. Summary: This systematic review analyzed the performance of sentinel lymph-node biopsy (SLNB) in women with node-positive breast cancer at diagnosis and node-negative tumor after neoadjuvant therapy compared to axillary lymph-node dissection. The results showed that SLNB has a low false-negative rate, high lymph-node identification rate, good negative predictive value, and high accuracy. The performance of SLNB can be improved with double mapping and marking the affected node.

Assessing the methodological strengths and limitations of the Spanish Society of Medical Oncology (SEOM) guidelines: a critical appraisal using AGREE II and AGREE-REX tool

Frequency of anticancer drug use at the end of life: a scoping review

Summary: The purpose of this scoping review was to describe the methods and extent of anticancer drug use at the end of life. Through systematic searches, 341 eligible publications were selected, including 69 articles on all cancer types published within the last 5 years. The results showed variability in the frequency of anticancer drug use across different end of life periods.

Dual neoadjuvant blockade plus chemotherapy versus monotherapy for the treatment of women with non-metastatic HER2-positive breast cancer: a systematic review and meta-analysis

Furthermore, dual treatment may lead to improved survival outcomes and tumor clinical response. For neoadjuvant treatment of HER2-positive breast cancer, the use of dual blockade with trastuzumab and pertuzumab is recommended.
