To elucidate the mechanisms underlying peripheral neuropathic pain in the context of HIV infection and antiretroviral therapy, we measured gene expression in dorsal root ganglia (DRG) of rats subjected to systemic treatment with the anti-retroviral agent, ddC (Zalcitabine) and concomitant delivery of HIV-gp120 to the rat sciatic nerve. L4 and L5 DRGs were collected at day 14 (time of peak behavioural change) and changes in gene expression were measured using Affymetrix whole genome rat arrays. Conventional analysis of this data set and Gene Set Enrichment Analysis (GSEA) was performed to discover biological processes altered in this model. Transcripts associated with G protein coupled receptor signalling and cell adhesion were enriched in the treated animals, while ribosomal proteins and proteasome pathways were associated with gene down-regulation. To identify genes that are directly relevant to neuropathic mechanical hypersensitivity, as opposed to epiphenomena associated with other aspects of the response to a sciatic nerve lesion, we compared the gp120 + ddC-evoked gene expression with that observed in a model of traumatic neuropathic pain (L5 spinal nerve transection), where hypersensitivity to a static mechanical stimulus is also observed. We identified 39 genes/expressed sequence tags that are differentially expressed in the same direction in both models. Most of these have not previously been implicated in mechanical hypersensitivity and may represent novel targets for therapeutic intervention. As an external control, the RNA expression of three genes was examined by RT-PCR, while the protein levels of two were studied using western blot analysis. The physiological function of different voltage-gated sodium channels in pain

George Goodwin, Stephen B. McMahon

Summary: Evidence from human genetic pain disorders shows that voltage-gated sodium channel alpha-subtypes Nav1.7, Nav1.8 and Nav1.9 play important roles in peripheral pain signaling. Drug development targeting these channels requires a better understanding of their involvement in pain signal transmission.

Summary: Research has shown that ultra-low frequency biphasic current can rapidly generate conduction block in animal models, holding promise as an effective treatment for chronic pain. In clinical trials, epidural spinal ULF neuromodulation significantly improved pain ratings, demonstrating high therapeutic potential.

The association between pain-induced autonomic reactivity and descending pain control is mediated by the periaqueductal grey

Summary: This study investigates the interaction between the autonomic nervous system (ANS) and descending pain control mechanisms, revealing the role of the periaqueductal grey (PAG) in mediating this relationship. The results show a negative correlation between heart rate variability (HRV) and descending pain modulation, with PAG-vmPFC functional connectivity mediating this association. This multi-modal approach provides new evidence of body-brain interactions in pain modulation.

Summary: Na(v)1.7 is a promising drug target for pain treatment, but there is a mismatch between the analgesia produced by Na(v)1.7 loss-of-function and the peripherally restricted Na(v)1.7 inhibitors. The function of Na(v)1.7 in axonal propagation in nociceptors is not clearly defined.

Summary: This study examines alterations in morphometric similarity in chronic pain patients and identifies genetic and cellular correlates of these changes. The findings suggest that cortical remodeling in chronic pain may be influenced by multiple elements of the brain's cellular architecture and highlight potential pathways for future genetic association or drug development studies.
