Early reduction in tumour [18F]fluorothymidine (FLT) uptake in patients with non-small cell lung cancer (NSCLC) treated with radiotherapy alone

Purpose Changes in tumour 3'-deoxy-3'-[ F-18] fluorothymidine (FLT) uptake during concurrent chemo-radiotherapy in patients with non-small cell lung cancer (NSCLC) have been reported, at variable time points, in two pilot positron emission tomography (PET) studies. The aim of this study was to assess whether FLT changes occur early in response to radiotherapy (RT) without concurrent chemotherapy and whether such changes exceed test-retest variability. Methods Sixteen patients with NSCLC, scheduled to have radical RT, underwent FLT PET once/twice at baseline to assess reproducibility and/or after 5-11 RT fractions to evaluate response. Primary and nodal malignant lesions were manually delineated on CT and volume, mean and maximum standardized uptake values (SUVmean and SUVmax) estimated. Analysis included descriptive statistics and parameter fitting to a mixed-effects model accounting for patients having different numbers of evaluable lesions. Results In all, 35 FLT PET scans from 7 patients with a total of 18 lesions and 12 patients with a total of 30 lesions were evaluated for reproducibility and response, respectively. SUVmean reproducibility in primary tumours (SD 8.9 %) was better than SUVmax reproducibility (SD 12.6 %). In nodes, SUVmean and SUVmax reproducibilities (SD 18.0 and 17.2 %) were comparable but worse than for primary tumours. After 5-11 RT fractions, primary tumour SUVmean decreased significantly by 25 % (p= 0.0001) in the absence of significant volumetric change, whereas metastatic nodes decreased in volume by 31 % (p= 0.020) with a larger SUVmean decrease of 40 % (p< 0.0001). Similar changes were found for SUVmax. Conclusion Across this group of NSCLC patients, RT induced an early, significant decrease in lesion FLT uptake exceeding test-retest variability. This effect is variable between patients, appears distinct between primary and metastatic nodal lesions, and in primary tumours is lower than previously reported for concurrent chemo-RT at a similar time point. These results confirm the potential for FLT PET to report early on radiation response and to enhance the clinical development of novel drugradiation combinations by providing an interpretable, early pharmacodynamic end point.

Summary: This study analyzes the overall survival of patients with inoperable stage II or III NSCLC treated with radiotherapy, and for the first time includes left ventricular ejection fraction as a metric of cardiac function. The results indicate that there is a region within the base of the heart that is significantly associated with worse overall survival, and an interaction between baseline cardiac health and heart base dose was identified.

Support and information needs for patients with non-small cell lung cancer receiving concurrent chemo-radiotherapy treatment with curative intent: Findings from a qualitative study

Summary: The purpose of this study was to identify the support and information needs of patients and their informal caregivers along the CCRT pathway. Semi-structured interviews were conducted to identify physical, psychological, and practical support needs, as well as information needs at different time points. The results showed that the need for disease and treatment information and support remains consistent throughout CCRT and beyond, and additional support for regular activities may also be desired.

Emerging therapies targeting the delta-like ligand 3 (DLL3) in small cell lung cancer

Summary: Small cell lung cancer (SCLC) is a highly aggressive neuroendocrine carcinoma with limited treatment options. DLL3, a Notch inhibitory ligand, is overexpressed on SCLC cells, making it an attractive therapeutic target. This article discusses the clinical experience with a DLL3-targeting antibody-drug conjugate called Rova-T, as well as other DLL3-targeting agents currently in development, including T-cell engager molecules and CAR T-cell therapy. The challenges and opportunities for DLL3-targeting therapies, such as using DLL3 as a biomarker and exploring combinatorial approaches, are also discussed.

Summary: Through longitudinal evolutionary analysis of 126 non-small cell lung cancer tumors, it was found that 25% of cases showed early metastatic divergence before the last clonal sweep in the primary tumor, which was enriched in patients who were smokers at the time of initial diagnosis. Polyclonal dissemination was associated with tumor recurrence, while primary lymph node disease played a limited role in metastatic relapse. Our study highlights the importance of selection in metastatic clone evolution within untreated primary tumors.

Summary: This study analyzed 1,644 tumor regions and found various gene mutations and evolutionary processes in lung adenocarcinoma. It was discovered that clonal expansion and whole genome doubling play important roles in the development and relapse of non-small cell lung cancer. The findings also revealed similarities in etiology and pathogenesis between lung adenocarcinoma in smokers and never-smokers.

Summary: Cancer-associated cachexia (CAC) is a significant factor in the morbidity and mortality of non-small cell lung cancer patients. The distribution of skeletal muscle and adipose tissue at the time of diagnosis is associated with survival rates. Individuals who develop CAC exhibit distinct tumor genomic and transcriptomic profiles, including enrichment of inflammatory signaling and epithelial-mesenchymal transitional pathways. The protein GDF15 is identified as a potential therapeutic target for managing CAC.

Higher-grade tumors have increased chromosomal complexity and tend to evolve towards higher-grade patterns. Association between radiotherapy protocol variations and outcome in the CONVERT trial

Summary: This study conducted comprehensive contouring, dosimetry, and treatment delivery quality assurance in radiotherapy, and examined the impact of protocol variations on patient outcomes. Non-compliant heart contours were associated with increased dose delivered to the heart organ at risk (OAR), but unacceptable variations did not affect toxicity or survival outcomes.
