Limitations of the QuantiFERON®-TB Gold test in detecting Mycobacterium tuberculosis infection in immunocompromised patients

Four cases are presented, immunosuppressed by at least three different mechanisms: one HIV-positive patient with a CD4 count of 0.29 x 10(6)/ml, one malnourished patient, and two kidney-transplanted patients. All patients had a negative interferon (IFN)-gamma test for suspected tuberculosis (TB), but a positive culture. We conclude that a negative IFN-gamma test does not exclude TB disease in immunosuppressed patients. (c) 2007 European Federation of Internal Medicine. Published by Elsevier B.V. Estimating Renal Function Following Lung Transplantation

Summary: Patients undergoing lung transplantation (LTx) experience a rapid decline in glomerular filtration rate (GFR) in the acute postoperative period. This study compared different equations for estimating GFR in LTx patients and found that equations based on plasma creatinine underestimated GFR decline, while equations based on plasma cystatin C overestimated GFR decline after LTx. The 2021 CKD-EPI combination equation had the most accurate estimation of GFR in both pre-LTx and post-LTx periods. Therefore, caution should be exercised when interpreting renal function based on GFR estimation equations in the acute postoperative period following LTx.

Endotrophin Levels Are Associated with Allograft Outcomes in Kidney Transplant Recipients

Summary: This study evaluated endotrophin (ETP) as a prognostic marker in kidney transplant recipients. It found that ETP levels in the blood on the first day after transplantation were strongly associated with delayed graft function, while ETP levels in the urine at the third month after transplantation were significantly correlated with kidney graft function at twelve months. The measurement of collagen type VI formation could aid in predicting graft function in kidney transplant recipients.
