Summary: This study aimed to evaluate the safety and effectiveness of endoscopy in individuals with CDH1 variants, particularly in those who declined prophylactic total gastrectomy. The study found that endoscopy had a certain level of safety and effectiveness in early detection and interception of gastric cancer.

Summary: Evaluation of performing totally laparoscopic distal gastrectomy without prophylactic drains in selected patients showed that omitting prophylactic drainage may reduce surgery time and result in faster recovery. Routine prophylactic drains are not necessary in selected patients, but may be useful in high-risk patients.

Summary: Individuals with germline E-cadherin mutations have a high risk of developing diffuse gastric cancer. Prophylactic total gastrectomy is the only life-saving treatment. Studies show that 85.4% of individuals have gastric cancer detected after PTG, with a higher rate of no cancer identified in the US (19.6%). PTG remains the best treatment for individuals with CDH1 mutations.

Summary: A 41-year-old woman was admitted to the hospital for epigastralgia and was diagnosed with gastric adenocarcinoma with multiple liver metastases. Her genetic testing revealed a point mutation in the APC gene. Two older brothers and their mother also had the same genomic mutation and were diagnosed with GAPPS, undergoing prophylactic surgery.

Summary: The frequency of CDH1 germline mutations in Chinese patients with HDGC is 7.4%, with new variants identified as N405Y and W409X. Among the 21 patients, up to 28.6% of CDH1 mutations were c.1775G>C (E551Q).

Summary: Prophylactic total gastrectomy performed on patients with CDH1 mutations has shown high satisfaction rates and improved quality of life postoperatively. The procedure effectively prevents the development of advanced diffuse gastric cancer in these high-risk individuals.

Summary: E-cadherin germline mutations, particularly CDH1 gene mutations, are associated with hereditary diffuse gastric cancer syndrome. The frequency of CDH1 mutations varies across different geographical areas, with a higher incidence in low-prevalence countries. The type and relative frequency of CDH1 mutations also differ between study groups and regions, with missense variants more common in high-incidence areas. Identifying individuals with clinically significant CDH1 mutations is crucial for effective genetic screening and patient management in gastric cancer.

Summary: Radical gastrectomy with metastasectomy may have a potential role in stage IV gastric cancer patients. Laparoscopic gastrectomy with metastasectomy in selected stage IV gastric cancer patients is safe and feasible.

Summary: Laparoscopic proximal gastrectomy (LPG) can be considered as an alternative to laparoscopic total gastrectomy (LTG) for patients with proximal gastric cancer, with comparable safety and efficacy, especially when performed with the double tract anastomosis/double-flap technique (LPG-DT/DFT).

Summary: This study analyzed patients with gastric cancer who underwent laparoscopic pylorus-preserving gastrectomy (LPPG) and found that reflux esophagitis is one of the mid- to long-term complications. The study identified several factors associated with postoperative reflux esophagitis, including male sex, preoperative reflux esophagitis, body mass index ≥ 23 kg/m2 at 1 year postoperatively, postoperative hiatal hernia, and long-term stasis. Therefore, careful attention should be paid in performing LPPG and in postoperative management for gastric cancer patients with these risk factors.

Summary: The study aims to define the standard of care for follow-up after prophylactic total gastrectomy (PTG) through a combination of literature review and two-round Delphi consensus. The guidelines provide a framework to best manage people following PTG, although the evidence supporting them is limited. They could also support the collection of information on the long-term effects of PTG.

Summary: This study compared the long-term outcomes of robotic and LG approaches in patients with gastric cancer. The results showed that after balancing covariates, there were no significant differences in long-term survival and recurrence-free survival between the two surgical methods, and robotic gastrectomy showed advantages in reducing blood loss.

Summary: The study found that robotic gastrectomy (RG) is associated with significant advantages in treating gastric cancer, including shorter operative time, less blood loss, lower postoperative morbidity, and shorter postoperative hospital stay when compared to laparoscopic gastrectomy (LG).

Summary: In a multicenter randomized trial conducted in the Netherlands, laparoscopic gastrectomy did not show a significant advantage over open gastrectomy in terms of hospital stay, postoperative complications, and oncological outcomes for Western patients with predominantly advanced gastric cancer undergoing multimodality treatment. Both procedures demonstrated similar results in postoperative complications and oncological efficacy.
