Acromegaly is predominantly caused by a pituitary adenoma, which secretes an excess of GH resulting in increased IGF1 levels. Most of the GH assays used currently measure only the levels of the 22 kDa form of GH. In theory, the diagnostic sensitivity may be lower compared with the previous assays, which have used polyclonal antibodies. Many GH-secreting adenomas are plurihormonal and may co-secrete prolactin, TSH and alpha-subunit. Hyperprolactinaemia is found in 30-40% of patients with acromegaly, and hyperprolactinaemia may occasionally be diagnosed before acromegaly is apparent. Although trans-sphenoidal surgery of a GH-secreting adenoma remains the first treatment at most centres, the role of somatostatin analogues, octreotide long-acting repeatable and lanreotide Autogel as primary therapy is still the subject of some debate. Although the normalisation of GH and IGF1 levels is the main objective in all patients with acromegaly, GH and IGF1 levels may be discordant, especially during somatostatin analogue therapy. This discordance usually takes the form of high GH levels and an IGF1 level towards the upper limit of the normal range. Pasireotide, a new somatostatin analogue, may be more efficacious in some patients, but the drug has not yet been registered for acromegaly. Papers published on pasireotide have reported an increased risk of diabetes mellitus due to a reduction in insulin levels. Pegvisomant, the GH receptor antagonist, is indicated - alone or in combination with a somatostatin analogue - in most patients who fail to enter remission on a somatostatin analogue. Dopamine-D2-agonists may be effective as monotherapy in a few patients, but it may prove necessary to apply combination therapy involving a somatostatin analogue and/or pegvisomant.

Sex difference in patients with controlled acromegaly-A multicentre survey

Summary: Objective: The study aimed to explore the sex differences in growth hormone (GH) and insulin-like growth factor 1 (IGF-I) patterns, clinical features, and disease control in patients with controlled acromegaly. The results showed that postmenopausal females with controlled acromegaly exhibited higher GH levels than males despite comparable IGF-I levels, which also translated into impaired metabolic health and well-being.

Psychiatric Morbidity in Women With Previous Gestational Diabetes Mellitus: A Nationwide Register-Based Cohort Study

Summary: This study investigated the association between previous gestational diabetes mellitus (GDM) and incident psychiatric morbidity, and explored the role of subsequent diabetes development in psychiatric morbidity risk. The findings indicated that GDM was associated with an increased risk of depression and anxiety disorders, and subsequent diabetes played a significant role in future psychiatric morbidity risk.

Summary: This study investigated the association between maternal cortisol levels in the third trimester of pregnancy and offspring blood pressure (OBP). The results showed negative associations between maternal cortisol levels and OBP, specifically in boys. This suggests that maternal cortisol is not a risk factor for higher blood pressure in offspring up to the age of 5.

Maternal cortisol levels in third trimester and early language development: A study of 1093 mother-child pairs from the Odense Child Cohort

Summary: This prospective observational study aimed to evaluate the association between maternal third trimester cortisol and early longitudinal offspring language development, and to test whether there were sex differences in the association. The results showed that higher maternal cortisol levels in the third trimester were positively associated with early language development in children, with boys showing effects on productive vocabulary and girls showing effects on receptive vocabulary.

Summary: Hypogonadism is common during opioid treatment and is associated with cardiovascular disease. The effects of testosterone replacement therapy (TRT) on the coagulation system in opioid-treated men were investigated. TRT was found to have an anticoagulant effect by suppressing the tissue factor pathway in men with opioid-induced hypogonadism.

The Buss-Perry Aggression Questionnaire: Translation and Preliminary Validation in a Danish Population Sample

Summary: The aim of this study was to translate and validate Danish versions of the Buss-Perry Aggression-Questionnaire and the Short Aggression-Questionnaire. The results showed that the SH-AQ had better fit-parameters, and the Alternative-SH-AQ had the best model-fit. Men had higher scores than women on all dimensions except for Hostility. The study recommends using the Alternative-SH-AQ for aggression research in Denmark.

Summary: This study aimed to describe diagnostic trends for all patients with CAH in Denmark. The prevalence of CAH combined was 15.1 and 9.0 per 100,000 newborn females and males, respectively. Diagnosis of NC-CAH increased significantly during the course of the study.

Anthropometrics of neonates born to mothers with PCOS with metformin or placebo exposure in utero

Summary: This study explores the effects of maternal polycystic ovary syndrome (PCOS) and metformin therapy on fetal growth. The results indicate that intrauterine metformin exposure has an impact on the birth anthropometrics of infants born to women with PCOS, particularly in terms of head circumference. The study also suggests that maternal factors such as pre-pregnancy body mass index and PCOS phenotype may modify the effects of metformin. The study is rated 8 out of 10 for its importance.

Summary: This study found that low serum selenium levels during pregnancy, particularly low selenoprotein P (SELENOP) and glutathione peroxidase 3 (GPX3) activity, were associated with an increased risk of gestational diabetes mellitus (GDM) and large for gestational-age offspring. Offering selenium status assessment during pregnancy can identify high-risk women for GDM who may benefit from selenium substitution.
