Khan, Vindya Kumarapeli, Andrew E. Moran, Margaret Mswema Silwimba, Brian Rayner, Apichard Sukonthasan, Jing Yu, Nizal Saraffzadegan, K. Srinath Reddy, Taskeen Khan

Summary: Hypertension is a major global cause of cardiovascular disease and deaths, with less than 14% of patients having controlled blood pressure. The World Health Organization has developed new guidelines for pharmacological treatment of hypertension in nonpregnant adults, including thresholds, targets, follow-up intervals, and healthcare worker involvement.

Early identification of frailty: Developing an international delphi consensus on pre-frailty

Duygu Sezgin, Mark O'Donovan, Jean Woo, Karen Bandeen-Roche, Giuseppe Liotta, Nicola Fairhall, Angel Rodriguez-Laso, Joao Apostolo, Roger Clarnette, Carol Holland, Regina Roller-Wirnsberger, Maddalena Illario, Leocadio Rodriguez Manas, Miriam Vollenbroek-Hutten, Burcu Balam Dogu, Cafer Balci, Francisco Orfila Pernas, Constanca Paul, Emer Ahern, Roman Romero-Ortuno, William Molloy, Maria Therese Cooney, Diarmuid O'Shea, John Cooke, Deirdre Lang, Anne Hendry, Siobhan Kennelly, Kenneth Rockwood, Andrew Clegg, Aaron Liew, Ronan O'Caoimh

Summary: Pre-frailty is a multidimensional risk state along the frailty continuum, potentially leading to frailty development, with factors including physical impairment, cognitive decline, nutritional deficiencies and socioeconomic disadvantages. Experts agree that pre-frailty is an age-associated, multi-factorial, multi-dimensional and non-linear process that does not inevitably lead to frailty, and targeted interventions can reverse or attenuate its development.

Building resilience and reversing frailty: a randomised controlled trial of a primary care intervention for older adults

John Travers, Roman Romero-Ortuno, John Langan, Fergal MacNamara, Darren McCormack, Christopher McDermott, Jude McEntire, Joanne McKiernan, Sean Lacey, Peter Doran, Dermot Power, Marie-Therese Cooney

Summary: This study examined the effectiveness of an optimised exercise and dietary protein intervention in combating frailty and improving resilience in older people. The combination of exercise and dietary protein significantly reduced frailty and improved self-reported health after 3 months of intervention.

Testing the feasibility of a primary-care exercise intervention to prevent and reverse early frailty and build resilience in community-dwelling older adults

John Travers, Roman Romero-Ortuno, Marie-Therese Cooney

Summary: This study tested the feasibility of an optimized exercise intervention for mild or pre-frailty in a primary-care setting and provided design guidance for future research. The results showed high uptake and adherence to frailty intervention, with participants reporting physical and mental health benefits.

Involving older people in co-designing an intervention to reverse frailty and build resilience

John Travers, Roman Romero-Ortuno, Eidin Ni She, Marie-Therese Cooney

Summary: This study aimed to involve older people in co-designing an intervention to reverse frailty and build resilience. Through group discussions, telephone interviews, and online workshops, older people actively participated in the design of home-based resistance exercises, frailty intervention format, and dietary guidance. The study demonstrated that inclusive involvement can be achieved during a pandemic.
