I'm not the doctor; I'm just the patient: Patient agency and shared decision-making in naturally occurring primary care consultations

Summary: This study explores the interactional processes of clinical decision-making during medical consultations, focusing on how patients show agency by proposing and opposing actions, as well as the normative dimensions and role expectations involved. The findings reveal that patients more frequently oppose actions than propose them, and their opposition is more direct. Patients reveal their values when explaining their proposals and opposition. The study also suggests that assertive patients have the most influence when they directly oppose actions and provide elaboration. However, patients' role-performance changes throughout the consultations, and they usually defer to the authority of general practitioners in the final stages of decision-making. The practice implications highlight the importance for clinicians to be attentive to patients' engagement in decision-making throughout the entire consultation, considering the normative dimensions and institutional constraints on patient actions.

Through interviews with a (sub)clinical sample, the study identifies themes related to body surveillance, fixation on movement and intake, food scarcity, and body vulnerability. The study also highlights the differences in expression of these themes based on different identities.

Prevalence of Undernutrition and Risk of Undernutrition in Overweight and Obese Older People

Summary: Obese and overweight elderly people are not immune to undernutrition, which can be caused by a decline in appetite and an increased risk of chronic diseases. A study in France found that 2% of respondents with a BMI over 25 were undernourished and 23% were at risk of undernutrition. Specifically, 18% of overweight individuals and 29% of obese individuals were at risk of undernutrition. The study highlights the need for further research and strategies to address nutritional risk in obese and overweight older adults.

People tend to be selective in their honesty, being more likely to choose honesty when it is linked to selfishness. The research suggests that people's preferences for honesty are more in line with genuine needs, and even without selfish motives, honesty can still result in harm to interpersonal relationships.

Summary: Pre-frailty is a multidimensional risk state along the frailty continuum, potentially leading to frailty development, with factors including physical impairment, cognitive decline, nutritional deficiencies and socioeconomic disadvantages. Experts agree that pre-frailty is an age-associated, multi-factorial, multi-dimensional and non-linear process that does not inevitably lead to frailty, and targeted interventions can reverse or attenuate its development.

Building resilience and reversing frailty: a randomised controlled trial of a primary care intervention for older adults
