Summary: In low surgical risk patients with symptomatic severe aortic stenosis, the PARTNER 3 trial showed superiority of transcatheter aortic valve replacement (TAVR) over surgery for the primary endpoint at 1 year. However, at 2 years, differences in death and stroke between TAVR and surgery were no longer significant, and TAVR patients had an increased risk of valve thrombosis.

Summary: This study compared the 5-year risk-adjusted outcomes of S3 TAVR and SAVR in intermediate-risk patients and found similar rates of death and disabling stroke between the two procedures. However, the incidence of paravalvular regurgitation was higher after S3 TAVR.

Summary: The study compared outcomes of transcatheter aortic valve replacement (TAVR) and surgical aortic valve replacement (SAVR) in patients with rheumatic aortic stenosis (AS), as well as TAVR in nonrheumatic AS. The results showed similar mortality rates between TAVR and SAVR for rheumatic AS patients, suggesting that TAVR could be a viable alternative to traditional surgery in this patient population.

Summary: This study evaluated the clinical and echocardiographic outcomes of transcatheter aortic valve replacement (TAVR) compared with surgery in low-risk patients at 3 years. It found that TAVR showed durable benefits compared with surgery in terms of reducing the incidence of all-cause mortality or disabling stroke, and also significantly improved valve hemodynamics in patients.

Summary: This study found that there is a higher risk associated with redo surgery after transcatheter aortic valve replacement (TAVR), and the number of patients requiring reoperation after TAVR is increasing. Therefore, patients with a longer life expectancy and unsuitable anatomy for redo-TAVR should consider surgical aortic valve replacement (SAVR) as the first-line treatment option.

Summary: A meta-analysis comparing TAVI with SAVR in lower-risk patients showed a lower risk of death and death or disabling stroke at 1 year with TAVI. However, there were no significant differences in main outcomes between the two procedures in higher-risk patients.

Summary: This study aimed to evaluate the 5-year incidence and clinical outcomes of hemodynamic structural valve deterioration (SVD) in patients undergoing self-expanding transcatheter aortic valve implantation (TAVI) or surgery. The results showed a lower rate of SVD in the TAVI group and its association with worse clinical outcomes.

Summary: This study compares 1-year outcomes between TAVR patients with BAV and TAV morphology. The results show that TAVR with the SAPIEN 3 device demonstrates similar outcomes to TAV patients in low-surgical-risk, severe aortic stenosis cases.

Summary: This study aimed to evaluate the risk of noncardiac surgery after TAVI. The results suggest that noncardiac surgery can be performed early after successful TAVI, and device performance may impact the risk of adverse outcomes after surgery.

Summary: Severe PPM is more common following SAVR compared with TAVR and only affects mortality in patients with LF following TAVR and LF and low ejection fraction following SAVR, while severe PPM in normal flow is not associated with poor outcomes.

Summary: This study compared the outcomes of transcatheter aortic valve implantation (TAVI) and surgical aortic valve replacement (SAVR) in octogenarians. The results showed similar clinical outcomes in terms of clinical endpoints in low- and medium-risk groups, but differences were observed in certain indicators such as paravalvular leak, acute kidney injury, and new onset atrial fibrillation.

Summary: TAVR is rapidly expanding its indications, but faces technical challenges and knowledge gaps when dealing with patients with bicuspid aortic valve (BAV). Physicians need to understand the BAV anatomy and TAVR procedure planning comprehensively to guide treatment decisions.
