Summary: This study aimed to investigate pain trajectories after posterolateral and axillary thoracotomies from postoperative days 0-5 and identify potential factors related to the worst trajectory. The results revealed four pain trajectories, with preoperative pain being a major factor for the worst trajectory of early postoperative pain.

Summary: This study identified that factors such as younger age, chronic opioid use, talc use, and multi-port VATS were associated with higher levels of acute postoperative pain. Furthermore, it found that a history of mental health condition, chronic use of opioids, urgent operations, and postoperative tramadol use were independently associated with a higher risk of chronic pain. It is important to consider modifying surgical practices associated with higher levels of pain and informing patients of non-modifiable risk factors for pain postoperatively.

Summary: This systematic review and meta-analysis investigated the prevalence and characteristics of persistent postoperative pain (PPP) after thoracic surgery. The results showed that 38.1% of thoracic surgery patients developed PPP, with 40.6% experiencing moderate-to-severe pain and 56.5% requiring opioid analgesics. Therefore, adequate pain treatment and follow-up are needed for patients undergoing thoracic surgery.

Summary: This study found that VATS procedures lasting over 5 hours and conversions to thoracotomy were associated with postoperative neuropathic pain. For cases involving bronchoplasty, angioplasty, or large tumors, a thoracotomy approach may be more appropriate depending on the surgeon's expertise. In cases of local or vascular adhesions, setting a time limit and considering patient safety first may be necessary before deciding to convert to thoracotomy.
