Sorting out measures and definitions of screening participation to improve comparability: The example of colorectal cancer

Participation is a key indicator of the potential effectiveness of any population-based intervention. Defining, measuring and reporting participation in cancer screening programmes has become more heterogeneous as the number and diversity of interventions have increased, and the purposes of this benchmarking parameter have broadened. This study, centred on colorectal cancer, addresses current issues that affect the increasingly complex task of comparing screening participation across settings. Reports from programmes with a defined target population and active invitation scheme, published between 2005 and 2012, were reviewed. Differences in defining and measuring participation were identified and quantified, and participation indicators were grouped by aims of measure and temporal dimensions. We found that consistent terminology, clear and complete reporting of participation definition and systematic documentation of coverage by invitation were lacking. Further, adherence to definitions proposed in the 2010 European Guidelines for Quality Assurance in Colorectal Cancer Screening was suboptimal. Ineligible individuals represented 1% to 15% of invitations, and variable criteria for ineligibility yielded differences in participation estimates that could obscure the interpretation of colorectal cancer screening participation internationally. Excluding ineligible individuals from the reference population enhances comparability of participation measures. Standardised measures of cumulative participation to compare screening protocols with different intervals and inclusion of time since invitation in definitions are urgently needed to improve international comparability of colorectal cancer screening participation. Summary: This study aimed to determine the impact of occupation and work-related factors on lung cancer survival. The findings showed that the skill level required for the occupation had an impact on lung cancer survival, with lower skill levels associated with worse survival for both men and women. Workers in paid employment without specific information on occupation were identified as the most at-risk socio-professional category in nearly all models.

Summary: This study provides a comprehensive examination of worldwide variation and trends in survival from brain tumors in adults by analyzing individual data from 2000 to 2014. The results show significant regional differences and temporal changes in survival rates, especially for the most common sub-types, with remarkable improvements since 2005 due to the uptake of chemoradiation. However, there are still some countries that lag behind in terms of survival rates.

Comparison of statistical models to predict age-standardized cancer incidence in Switzerland

Summary: This study compared the performance of different statistical methods for predicting cancer incidence, and found that ARIMA models had the best overall performance, followed by linear regression models. The widely used APC and BAPC models were found to be suboptimal for prediction, especially when there was a trend reversal in incidence. The study suggests that it is better to regularly update cancer incidence predictions rather than making predictions for distant future periods.

Summary: The study aims to address the impact of organizational and cultural barriers on breast cancer screening, specifically low attendance in Southern Italy and underscreening and overscreening in Italy as a whole. Investing in mobile screening units to increase adherence in Southern Italy is cost-effective. Harmonizing the screening interval and investing in measures to reduce opportunistic screening and mobile screening units can save costs while maintaining the same benefits.

Estimating 10-year risk of lung and breast cancer by occupation in Switzerland

Summary: The study aims to estimate the 10-year risk of lung and breast cancer by occupation and smoking status and to create easy to use age-, and sex-specific 10-year risk charts. The results show that the risks of lung and breast cancer increase with age and are the highest for current smokers. There are significant differences in the 10-year risk between occupational categories, and smoking has a greater impact on the risk than occupation. The risk charts can be used as public health indicators and to inform policies to protect workers.

Summary: Swiss health insurance reimburses screening for colorectal cancer (CRC) with colonoscopy or fecal occult blood test (FOBT). A study was conducted to explore the association between CRC testing status of primary care physicians (PCP) and the testing rate among their patients. The results showed that patients of PCPs who were tested for CRC had a higher testing rate, providing important information for future interventions.

Summary: New non-invasive tests for colorectal cancer (CRC) are emerging, making it challenging to conduct trials with mortality reduction as the main goal. A formal consensus approach involving experts resulted in the revision of previously established principles and the emergence of twelve new principles. The effectiveness of a new test can be evaluated by comparing it with an established non-invasive test, with the faecal immunochemical test considered as the appropriate comparator. Flexible adjustment of the new test's positivity threshold is desirable to meet different screening goals and requirements. A four-phased approach is proposed for the evaluation of new tests, including small studies, prospective estimation of accuracy, evaluation in screening populations, and monitoring for missed lesions.

Summary: Canton Vaud, Switzerland implemented an organised colorectal cancer screening programme, leading to greater increases in colorectal cancer testing rates compared to neighbouring cantons and the rest of Switzerland. The faecal occult blood test use increased in Vaud and neighbouring cantons, while it decreased in the rest of Switzerland. By 2018, a significant proportion of colonoscopies and faecal occult blood tests occurred within the screening programme.

Research on occupational diseases in the absence of occupational data: a mixed-method study among cancer registries of Western Switzerland

Summary: The contribution of occupation-related diseases to the global burden of disease is often underestimated, especially in Switzerland. To address this issue, a research project was conducted to assess the need for and quality of occupation registration in Swiss cancer registries, and to find a suitable strategy for collecting this information. The accuracy of occupational information was found to be lower in registries that actively searched and verified this data. The use of a standardized questionnaire to improve the quality of collected occupation data was recommended.

Summary: In this study, genetic alterations in resectable pleural mesothelioma tissues and blood samples were analyzed, and it was found that high tissue tumor mutational burden, tissue median minor allele frequency, blood tumor mutational burden, and specific mutations were correlated with outcomes in patients with resected PM. These findings suggest that molecular profiling could help identify longer survivors in patients with resected PM.

Summary: This study evaluated the safety and activity of liposomal irinotecan in the perioperative treatment of resectable pancreatic ductal adenocarcinoma (rPDAC) patients. The results showed that NALIRIFOX has manageable and active outcomes, and should be further investigated in randomized trials comparing it to standard upfront surgery followed by adjuvant therapy.

The results show that sentinel-lymph-node mapping did not reach the expected sensitivity, but ultra-staging protocol improved the accuracy of diagnosis for patients.

Immunotherapy response in microsatellite stable metastatic colorectal cancer is influenced by site of metastases

Summary: This study explores the impact of metastatic sites on treatment outcomes for chemotherapy-refractory colorectal cancer patients. Summary: The study reveals that nullpeptides can serve as biomarkers for cancer detection and treatment, particularly in highly recurrent cancer patients. These nullpeptides primarily occur in highly expressed genes, particularly in specific loci of oncogenes and tumor suppressors. Recurrent nullpeptides are more likely to be found in neoantigens, which play a significant role in immunotherapy.
