Summary: The study presented the institutional experience of using individually designed fully covered self-expandable metal stents (FCSEMSs) for treating refractory benign esophageal strictures (RBESs) in pediatric patients. The results demonstrated that the use of FCSEMS in selected pediatric patients with RBESs was safe and effective in improving symptoms and quality of life with proper follow-up and treatment management.

Summary: Despite significant progress in PCI procedures, malapposed or overlapped stents can disrupt the delicate balance of vascular biology, potentially leading to thrombosis and restenosis. This study used 2D axisymmetric parametric CFD simulations to analyze the hemodynamic effects of malapposition and stent overlap, evaluating the risks of restenosis and thrombus based on TAWSS and OSI thresholds. The results showed that small malapposition distances caused adverse outcomes with low TAWSS and high OSI, while overlap configurations produced areas with low WSS and high OSI, with the effects being more sensitive for strut size in overlap configurations compared to well-applied stents.

Summary: The mechanical responses of endovascular stents play a crucial role in determining arterial homeostasis and vulnerability of atherosclerotic plaque. Stainless steel stents were found to apply higher stress to plaques and vessel walls, while magnesium alloy stents with similar design generated less stress. Minor improvements in magnesium alloy stents, such as increasing strut thickness, can enhance performance and reduce applied stresses, making them an optimal choice for stenting applications.

The results showed that dual-layered stents significantly reduced the ipsilateral cerebral embolism rate and NIRS-derived lipid signals after stenting, particularly in highly lipidic lesions.

The results showed immediate relief of symptoms in a majority of patients after stent placement, significant improvements in dyspnea scores, performance scores, and performance status, with a majority of patients still alive at 6 months without major complications.
