The results show that epicardial lead placement has lower failure rate and better long-term performance compared to transvenous leads. Therefore, epicardial lead placement should be considered when transvenous lead placement fails or in challenging cases.

Summary: The benefit of implantable cardioverter-defibrillators (ICD) in non-ischemic systolic heart failure (NICM) patients is uncertain. This study aimed to investigate the incidence, predictors, and prognostic impact of ventricular arrhythmias in NICM patients with ICD. The results showed that ventricular arrhythmias occurred in approximately one-fourth of NICM and ICD patients, and were associated with worse prognosis compared to those without sustained ventricular arrhythmias.
