Association of Nonacute Opioid Use and Cardiovascular Diseases: A Scoping Review of the Literature

Jade H. Singleton, Erin L. Abner, Peter D. Akpunonu, Anna M. Kucharska-Newton

Summary: This scoping review identified 23 original articles from the PubMed database investigating the relationship between nonacute opioid use (NOU) and cardiovascular outcomes. The studies showed a positive association between community prevalence of injection drug use (with opioids being the most commonly injected type of drug) and infective endocarditis, as well as between (primarily medically indicated) NOU and myocardial infarction. However, there was less consensus about the relationship between NOU and congestive heart failure, cardiac arrhythmia, and stroke.

The Toxicology Investigators Consortium 2020 Annual Report

Meghan B. Spyres, Kim Aldy, Lynn A. Farrugia, A. Min Kang, Jennifer S. Love, Sharan L. Campleman, Shao Li, Alexandra Amaducci, Evan Schwarz, Paul M. Wax, Jeffery Brent

Summary: The Toxicology Investigators Consortium (ToxIC) Registry was established in 2010 by the American College of Medical Toxicology. The 2020 report summarized data from 6668 cases, including demographics, toxic agents, clinical symptoms, treatments, and outcomes. Non-opioid analgesics were the most commonly reported agents, with acetaminophen being the most common. Additionally, there were 80 fatalities, accounting for 1.2% of all cases in the registry.

Intravenous phytonadione administered orally in reducing warfarin-related coagulopathy

Jordan H. Rice, Peter Akpunonu, George A. Davis, Adam Dugan, Jamie Litteral, Ashley N. Webb, Alexandra Wiegand, Abby Bailey, Regan A. Baum

Summary: The study compared the efficacy of phytonadione intravenous solution given orally and commercially available phytonadione tablets for reversing warfarin-related coagulopathy, finding that both forms performed similarly in achieving an INR <1.5 at 24 hours.

Pizon, Sharan L. Campleman, Shao Li, Kim Aldy, Yaron Finkelstein

Summary: This study analyzed data from 165 children with opioid intoxication and found that exposure to fentanyl and age ≥ 10 years were independent predictors of severe outcomes in children with opioid toxicity, highlighting the importance of targeting these risk factors for prevention efforts.

Jennifer S. Love, Dana L. Karshenas, Meghan B. Spyres, Lynn A. Farrugia, A. Min Kang, HoanVu Nguyen, Sharan L. Campleman, Shao Li, Paul M. Wax, Jeffery Brent, Kim Aldy

Summary: The report summarizes the data of the ToxIC Core Registry in 2021, including 8552 cases. It provides information on demographic data, drug exposure and clinical characteristics, as well as mortality rate. The report also analyzes new demographic characteristics, the impact of the COVID-19 pandemic, and novel substances of exposure.

Using natural language processing to identify opioid use disorder in electronic health record data

Jade Singleton, Chengxi Li, Peter D. Akpunonu, Erin L. Abner, Anna M. Kucharska-Newton

Summary: This study aims to determine the prevalence of opioid use disorder (OUD) in electronic health records using two methods: natural language processing (NLP) and ICD-10-CM diagnostic codes. The results show that combining NLP with ICD-10-CM codes provides the most comprehensive ascertainment of OUD in electronic health records.

Brown recluse spider envenomation with systemic loxoscelism and delayed hemolysis: A case report

Blake DiPaola, Jason Davis, Regan A. Baum, Peter Akpunonu, Andrew Micciche

Summary: A rare case of delayed hemolysis six days after envenomation by a brown recluse spider is presented, resulting in three emergency department visits and two hospitalizations. This case highlights the importance of continuous monitoring and/or follow-up in cases of systemic loxocelism.

Human exposures to Phytolacca americana in Kentucky

Jordan A. Woolum, Peter Akpunonu, Meredith Johnson, Ashley N. Webb

Summary: This study describes the demographics, treatments, and outcomes of pokeweed exposures in the state of Kentucky between 2000 and 2019. A total of 1669 cases were reported, with young males being the most heavily implicated. Oral ingestion was the most common exposure route, with gastrointestinal symptoms being the most frequently reported. Cutaneous exposures were found to be an underappreciated route. Treatments were mainly supportive in nature.

Summary: This study aims to evaluate the efficacy of varespladib-methyl in the treatment of snakebite envenoming (SBE) worldwide. The study is a multicenter, randomized, double-blind, placebo-controlled trial that compares the outcomes of patients receiving varespladib-methyl plus standard of care with those receiving placebo plus standard of care. The primary outcome measure is the modified Snakebite Severity Score (SSS).
