Endoscopic red flags for the detection of high-risk serrated polyps: an observational study

Background and study aims: In routine practice, colonoscopy may fail to prevent colorectal cancer (CRC), especially in the proximal colon. A better endoscopic recognition of serrated polyps is important, as this pathway may explain some of the post-colonoscopy cancers. In this study, the endoscopic characteristics of serrated polyps were examined. Patient and methods: This was a cross-sectional, single-center study of all consecutive patients referred for elective colonoscopy during 1 year. The endoscopists were familiarized with the detection and treatment of nonpolypoid colorectal lesions. Serrated polyps were classified into high risk serrated polyps, defined as dysplastic or large (>= 6mm) proximal nondysplastic serrated polyps, and low risk serrated polyps including the remaining nondysplastic serrated polyps. Advanced colorectal neoplasms were defined as multiple (at least three),>= 10mm in size, high grade dysplastic adenomas or CRC. Results: A total of 2309 patients were included (46.1% men, mean age 58.4 years), of whom 2.5% (57) had at least one high risk serrated polyp and 13.9% (322) had at least one advanced neoplasm. Overall, serrated polyps were more often nonpolypoid than adenomas (16.2% vs. 11.1 %; P = 0.002). In total, 65 high risk serrated polyps were found, of which 43.1% (28) displayed a nonpolypoid endoscopic appearance. Patients with advanced neoplasms were more likely to have synchronous high risk serrated polyps than patients without advanced neoplasms: OR 3.66 (95% CI 2.03-6.61, P < 0.001). Conclusions: High risk serrated polyps are frequently nonpolypoid and are associated with synchronous advanced colorectal neoplasms. Advanced colorectal neoplasms may therefore be considered red flags for the presence of high risk serrated polyps. Detection, diagnosis, and treatment of high risk serrated lesions may be important targets to improve the quality of colonoscopic cancer prevention.

Rebubbling and graft failure in Descemet membrane endothelial keratoplasty: a prospective Dutch registry study

Summary: This Dutch registry study identified risk factors for rebubbling and early graft failure in DMEK. Rebubbling was associated with higher graft failure, while older recipient age and surgical complications were risk factors for rebubbling. Early graft failure risk factors included transplantation before 2016 and surgical complications. Rebubbling was also associated with significantly lower endothelial cell densities.

Placebo response in pharmacological trials in patients with functional dyspepsia-A systematic review and meta-analysis

Summary: This study conducted a systematic review and meta-analysis to investigate the placebo response in pharmacological trials for functional dyspepsia (FD). The study found that the pooled placebo response rate ranged from 35.5% to 39.6% depending on the responder definition used. Lower baseline symptom scores were significantly associated with higher placebo response rates. The study suggests considering entry criteria based on symptom severity and separate reporting of core FD symptoms in future trials.

Validity and Reliability of the Assessment of Burden of Chronic Conditions Scale in the Netherlands

The impact of adipose tissue distribution on endometrial cancer: a systematic review

Further well-designed prospective studies are needed to explore these differences more specifically and understand how it can contribute to the prediction and therapy of EC.

Summary: Computer-aided diagnosis systems (CADx) can improve colorectal polyp (CRP) optical diagnosis. An explainable AI CADx was developed to automatically generate textual descriptions of CRPs, and its performance was analyzed using BLI images of 55 CRPs. The CADx showed high performance for surface descriptors, while the description of size and pits-distribution need improvement. Explainable AI can enhance the understanding and trust in CADx.

Participant Characteristics as Moderators of the Effects of Cognitive Behavioral Interventions on Concerns About Falling: Secondary Analyses of Two Randomized Controlled Trials

Summary: This study examined whether participant characteristics moderated the effects of two cognitive behavioral interventions on concerns about falling in older community-dwelling individuals. Secondary analyses of two RCTs were conducted, involving the group intervention A Matter of Balance - Netherlands (AMB-NL) (n = 540) and individual AMB - Home (n = 389) intervention. Marginal models were used for moderation assessment, including single and multiple moderator models. A total of 19 characteristics were assessed. Moderating effects were found for living situation, fall history, symptoms of depression, perceived general health, ADL disability, cognitive status, and consequences of falling-loss of independence subscale. Effects varied by intervention, time point, and type of model.

Summary: This study found that patients with ankylosing spondylitis (AS) have a higher mortality rate compared to the general population, and healthy individuals carrying the HLA-B27 gene also have a similar risk. Additionally, female patients have a lower life expectancy. However, in the overall European population, the presence of the HLA-B27 gene does not affect survival or the risk of death from cardiovascular disease.

Visceral obesity measured using computed tomography scans: No significant association with mortality in critically ill patients
